Miner was working in the 3200-033 raise. EE was bolting the hanging wall. A large wedge slipped off the wall and pinned the Miner to the foot wall causing fatal crushing injuries.

At 9:35 safety was notified that the #3 hoist, our primary escapeway was not operating properly. Crews were evacuated out the secondary escapeway. It was found there were bad sensors. The sensors were replaced and the hoist returned to service later that afternoon.
A seismic event occurred shortly after the blasted round in 37-281 stope. No one was in the area at the time of the event. The area was shut down to work and or travel until the next morning, when we could get into the area to investigate. upon inspection of the area, a section of cemented sand had taken damage during the event.
at 1350hrs an air blast occurred in the 46-146 old workings with the blast of the 46-136 heading. The air blast resulted in a ground fall beyond the anchorage points of the bolts. The ground fall occurred in an old working that was barricaded at the time of the event. No miners were in the area at the time.

A miner fractured their hand when it was pinched between a drill steel and the rib/wall. This occurred when a drill steel fell into the slushing lane and got stuck between the ribs. the miner tried to pry the steel free when the steel pinched their hand against the rib.
The employee bumped into the chute gate valve an activated the chute gate, which struck a scaling bar they were using to relieve muck. The end of the bar caught their left cheek and cut them.
A miner overheated during the shift, cycled round with minimal breaks. Started cramping and went to hospital to get iv fluids.
Miner was bolting the right rib installing a 6-ft dog when the glue set sooner that expected, causing the jackleg to spin out of EE's hands. The jackleg struck EE in the jaw, causing EE to fall and strike the back of EE's head. Miner lost consciousness briefly.
An LHD operator backed out of a head while mucking and ran over EE's partner's foot.
EE picked up a pump to place it back on a timber car. EE's foot was against the rail and when EE turned EE stumbled and felt a sharp stabbing pain in EE's back. EE did not want to see a DR. After EE's days off, EE stated EE's back wasn't getting better. EE was taken to the doctor on 09/23/2025 and was placed on lite duty by the physician.
Turning a piece of rail over and felt a pulling in left arm. No diagnosis at this time.
The employee was cutting timbers for repair work when debris blew around safety glasses into their eye. The injury was reported the next day, on 8/22/2025, and the EE was taken to the doctor, where they received antibiotics and was placed on restricted duty by the physician.
While scaling, loose material fell from the rib and rolled over the EE's ankle causing a sprain/strain of the ankle. The EE was placed on restricted duty by the physician.
While scaling, loose material fell from the hanging wall and landed on the scaling bar. The scaling bar was pulled from the EE's hand causing the EE's arm to get pushed back and twist. This resulted in a torn ligament in the left bicep. The EE was placed on restricted duty by the physician.
On May 7th a miner with a previously reported injury was sent to the doctor with complications from that injury. The visit resulted in a carpal tunnel diagnosis that requires surgery. the mine operator was not informed of the diagnosis until 07/02/2025
While using a hand grinder to cut a hinge, tha hand grinder kicked out striking the EE on the left knee causing a laceration. the laceration required sutures
While bolting the back, loose material fell causing the miner to move backwards. This caused miner's machine to fall toward miner, knocking backward. As miner fell backward, left hand came into contact with the post of the bolt deck which resulted in a hematoma on the hand. The miner was placed on restricted duty by the provider.
A labor support tripped while crossing a grizzly cover and fell causing a dislocation of the shoulder. The injured person was placed on light duty by the provider
Miner grabbed slusher cable near the block and got EE's hand stuck in the block causing a laceration of EE's left hand.
Miner was bolting when loose material fell from the back causing a laceration on the right forearm. The laceration required sutures.
While pulling a hitch pin from a loci, the hitch pin moved, pinching the motor operator's finger between the hitch pin and the wall of the loci. The incident resulted in a laceration that required stitches.
While moving a slusher block, the nipper slipped on the muck pile and fell causing EE's left number 5 finger to get pinched between the slusher cable and the slusher block. EE was taken to the doctor where EE received stitches.
While drilling an I-drift round, the injured miner's drill steel broke causing them to fall forward. As miner fell, miner reached out to catch themself on the rib and miner's hand contacted the edge of a bolt mat causing a laceration on the left palm/hand. The miner was taken to the doctor where miner received sutures.

While descending the stairs out of the main office after shift, the injured miner missed a step and fell down the stairs, twisting their knee. The injured miner declined going to the doctor. On December 26th the miner stated it wasn't getting better. We took EE to the doctor and the miner was placed on restricted light duty after being diagnosed with a sprain in the knee.
While barring down in the 49-173 heading, loose rock from the back fell out and struck the miner causing lacerations and contusions to the neck, shoulder and left elbow. They received sutures and was placed off work by provider.
A miner working in the 49-182 incline overheated and was taken to the local hospital where EE was diagnosed with dehydration. EE was given fluids and was placed off work the following shift by the provider.
The Miner was drilling a hole in the back to install roof bolts, as EE let down EE's machine, EE tripped on the oiler causing the machine to strike the miner on the right cheek. The Miner was taken to the local hospital and was treated with sutures for a laceration
While preparing to wet the muck down, the miner turned on the 1"" water line when the hose failed where it connected to the nozzle. The hose was pushed back and cut the miner's hand. The miner received sutures.
While fitting up a support for the fender of an LHD, debris fell from above the mechanic and entered EE's eye between EE's hard hat and safety glasses. The mechanic was taken to the eye doctor where a foreign body was removed from the eye and they were prescribed antibiotics for infection.
On September 4th the cager was pulling powder off the no. 3 skip when they felt a strain in lower groin area. The incident wasn't reported until 10/4/2024. The injured cager was taken to the doctor on 10/8/2024 where they were diagnosed with a hernia and placed on restricted duty.
While cleaning up muck from the track level, the motorman stepped over the hitch of a train car to get to the other side of the train when they slipped, twisting their ankle. This resulted in a sprained ankle
Miner was preparing to bolt when the air line on the jack leg failed. While trying to shut the air off to the hose, the miner slid against a mat on the rib causing a laceration to left forearm. The miner was taken to the hospital where the miner received stitches.
Miner was working in heading 5200-179 when they began vomiting. The miner stated they had drank approximately 1.5 gallons of their 2 gallon water jug. The miner left at the end of shift and reported the next day that they were still throwing up. On 08/16/2024 they were admitted to Shoshone Medical Center for fluids and observation.
Nipper was knocking a drill bit off of a jackleg steel. A chip of rock broke off and flew approximately 20' and hit the miner in the eye.
While bolting employee got some grit in left eye. Eye was flushed out at doctor's office and eye drops were prescribed.
While finishing drilling a small rock burst ejected a small amount of rock from the back striking the miner on right knee causing a laceration.
While bolting the miner was holding the end of a 6' mat in place while EE's partner was driving the bolt. Rock fell out of the wall and pushed the mat into EE's face causing a laceration on EE's face, EE then fell back and the rock struck EE's lower leg breaking EE's ankle.
Miner was riding on the draw head of the last car of a train being pulled; miner bent over the car and hit EE's head on the edge of the car suffering a concussion.
Employee was walking down ramp when EE tripped and caught self with left hand. This resulted in a broken wrist. Initial x-ray was negative. Did not receive confirmation of broken bone until 04/03/2024.
Mechanic was removing a seized bolt with an air hammer when a metal chip went under glasses and in EE's right eye.
Miner was driving a split set bolt, when a dust blew past safety glasses and got a rock in miner's left eye.
Drilling holes in the back with a jackleg; a piece of rock fell out of the back and pinched off the end of left thumb.
Miner was working in the 4900-149 stope. They cut their right elbow on a ground support plate (monster mat). The laceration required sutures. The miner was wearing arm guards.

Miner was working in the 3700-033 raise. They brushed against a monster mat. They received a small laceration of left wrist requiring 3 sutures and a tetanus shot at the Quick Care Clinic.
Climbing to top of raise, rocks fell out of the back, Miner ducked back into the raise and a rock lacerated Miner's left forearm.
Turning a locomotive wheel on the metal lathe, a metal chip cut through the leather gloves and lacerated the right index finger requiring stitches.
Miner slipped on air hose. EE fell backwards. EE's right forearm was cut by quartzite on the rib. Received 10 sutures.
The injured Miner was in basket of the Alimack raise climber, with a partner. The Partner was barring down. Miners hand was on the rail. A rock came down and hit the miners left hand. Injury required surgery.
Fabricator was working in the Morning Shop. A metal particle got into left eye. Employee was wearing safety glasses. Employee could not say exactly when the particle entered eye.
Mechanic received a small cut on finger. EE went to the Doctor on own. Received one very small stitch. EE did not report it until the end of the following shift (8/23).
3700 Motorman did not control speed and ran through the air door.
There was no damage to hoisting equipment. The Crews were stood down. No one was in any shaft conveyance. An air lock occurred in the oil accumulator for the hoist brakes. After 30 minutes the call was made to MSHA. The oil accumulator air lock was corrected. The #3 Hoist was up and running at 1802 hrs (6:02 pm).
Pulling on sand pipe, slipped in mud. Hurt knee.
A miner and partner were moving a 10""X10"" cap. As they were setting the cap in place one of the miners caught the tip of left index finger. The tip of the finger was de-gloved. Surgery was preformed the following day. Amputation to the first knuckle was required.
Nipper was assisting with loading the round. The nipper's hand was resting on the prell pot. A small rock came down and cut left wrist.
Miner was looking up and material entered EE's eye. Miner was wearing safety glasses. Eye Doctor removed foreign body from Miners left eye.
Miner was working in the 4900-366 ramp and became overheated.
#3 Hoist House. Hoist man was using a personal hand blender. EE did not unplug tool before cleaning. The blender was activated and cut EE's right thumb and forefinger. Emergency room sutures.
Miner was bolting the back in 4900-181 stope. A small rock fell and cut miner's chin. Laceration to chin required sutures.

Miner and EE's partner were breaking boulders on the 4900 #3 grizzly. A rock chip penetrated the tissue of EE's upper right thigh. ER visit and sutures.
Miner was bolting the drift on 5500 level. The bolt and plate fell out of hole in back. The plate came in contact with Miner's left forearm causing a laceration. Sutures were needed.
Shaft Repairman slipped on ice and injured left shoulder. EE was stepping off the cage on the top station of the Coeur shaft.
Engineer was painting survey points on the back in 49-360 ER. Dust entered EE's left eye. Miner was wearing safety glasses. Did not notice discomfort until that evening while removing contact lenses. Took EE to the eye doctor the next morning (9/15/22). Diagnosis was Keratitis. Antibiotic drops prescribed.
Shaft Crew was lowering replacement liner into the 3700 pocket. Section of liner bounced and hit Miner on the lower right leg. Miner was seen by Doctor the next day and and was given a full release (08/21/2022). Miner Worked the remainder of Miner's rotation. Miner started next rotation on 08/26/2022. Saw doctor on 08/30/2022 and was given a work release for 09/13/2022.
Miner was assisting a Mechanic with the removal of an hydraulic cylinder. The cylinder slipped and it crushed the tip of the Miners right index finger.
Miner was cleaning up at the 2400 lay-down was placing rubbish in EE's loader bucket. A second loader was tramming muck up the ramp. The second loader hit the miner who was cleaning up. Contact with the second loader grill caused a ""closed, Non-displaced left clavicle shaft fracture"".
Ground fall occurred after round was shot (determined after reviewing seismic data).
A large seismic event occurred at 2:47 pm today and located at the current face in 37-281. The event had a magnitude of 1.6 (moment magnitude) and 1.3 (local magnitude). It occurred 25 minutes after the round was shot and was preceded by at least one moderately large event that occurred immediately after blasting.
Miner was baring a rock out of a chute. Rock came loose knocking scaling bar out of EE's hands and hitting EE on the side of the jaw.
Miner was barring out a chute of the 3700-054 raise. The material came down and hit the bar. The bar then hit the miner in the left ear.
#3 hoist tripped out and it took intervention on behalf of Spencer Engineering to correct the issue.
Miner was on knees cleaning a drill hole. When they stood up and twisted, they said ""I reaggravated a lower back injury that happened in the early 2000's""
On January 2nd electrical sub-station (Avista) located 5-6 miles off site had two broken insulators caused by snow off-loading from power lines. January 6th same location Avista sub-station, part of the pole the power crew fixed on January 3rd broke and took out the power lines - this happened the evening of January 6th.
Electrical Sub-station located 5-6 miles from mine site (not associated with mine) lost an insulator from snow load and tripped out causing power outage to mine (total property)

No. 3 Hoist #2 Side faulted on over speed would not clear fault. Points on Lilly were closed, had to disconnect Lilly drive shaft and turn Lilly back to bring over speed cam off cam follower so hoist would reset.
Mine wide - lost power - snow load on power lines. Power restored at 1130PM; computer on #3 Hoist would not function - evacuated crews out through the Coeur Mine. #3 Hoist up and running at 2:30 AM December 22nd.
Lost power on site at 1416 hours. AVISTA Power informed us that a tree limb had fallen over power line. Power back on at 1500 hours.
4900-310 switch - move supplies by timber truck. Derailed timber truck; had to use draw bar to lift each side back on track. Felt ""pop"" to left shoulder. Employee did not report accident till Wednesday, October 27th. To clinic Monday November 1st.
The pump failed causing one of the fuses to blow in the pump starter, and because the starter did not trip it caused the motor to single-phase which caused the #3 Station Sub 600 amp disconnect to blow a fuse. The 600 amp disconnect in the Sub on the #3 station is burned up and needs to be replaced and 700HP pump burned up where fire started.
Galena Mill - Stairs by break room, employee tripped as employee was coming down stairs, caught self on railing. Went in to clinic on Friday October 22nd, released to full duty. Complained of pain and lack of motion, returned to clinic on October 28th. Taken off work till MRI is done on shoulder - Lost time started on Sunday, October 31st
No. 3 Hoist - 4 power bumps then AVISTA restored power. At approximately 1910 hrs. hoist kicked out - loop fault breaker fault. Electricians responded - removed damaged loop fault breaker and installed new one into system.
2400-081 Mucking out ore heading hit dag or rock while digging out the pile causing an abrupt halt bouncing miners body in mucker. Dismounting mucker noticed left abdominal pain, possible hernia.
Mill Maintenance man was repositioning a sheet of diamond plate on saw horse - felt a shoulder muscle pull (left shoulder). Surgery to left shoulder was done on Friday, November 26, 2021
2400-018W Miner scaled work area then set up bolt deck and started bolting, after a few bolts scaled area again, started bolting, rock came off rib hitting back of right boot, no cut or damage to boot, but miner had laceration to back of right ankle - two stitches.

Building metal latch boxes, felt eye irritation, tried to wash out with eye wash, still felt irritated - Right eye.

Cagers in 5500 pocket were pulling waste when a rock got hung up in the chute; overloaded the skip and #3 hoist tripped out trying to hoist skip. Miners were stood down, then evacuated out secondary escape way.
5200 Rock Pick, cleaning rocks by hand, grabbed rock to throw down grizzly, tripped over another rock ""tweaking"" lower Back. EE went to doctor on Monday, March 22nd.
Snowing, tree along sand line road fell across power line, shutting down power to the mine
5500-120 collaring hole with jack leg drill, rock came out of back hitting right knee. 1-1/2"" laceration.
Grinding on slusher bucket wearing glasses and clear face shield, spark bounced behind shield and fell in eye behind safety glasses
Cagers stopped at the 4900 #3 Station, exited the cage to call hoistman. Cagers told #3 Hoistman that they would be working on 4900 station and they would release the cage after they unloaded their gear off the cage. While one cager was unloading gear the cage moved up with doors open damaging the doors when they contacted the overhead cap. No injuries reported.
A windstorm caused loss of electrical power to the mine. The mine did not receive power until 0901 hrs. January 14th. 78 miners were directed to stand down at the shaft level stations. All miners safe and accounted for.

Coeur Hoist went down for electrical issues. Crews stood down. Problem was in sub floor transformer. North side transformer was not making full contact, loose bolts. Electricians inspected, made corrections.
Loading front part of a 4-yard bucket onto the cage the chain being used to lift the bucket slipped and bucket landed on miners left foot
4900-ER6 getting started bolting, tripped over bull hose landing on rock with right knee. Reported to supervisor on October 23rd, did not seek medical attention till November 9, 2020.
No. 3 Hoist - pulling muck from 4300 pocket, hoist kicked out, would not reset, called Spencer Engineering with computer faults. Electricians called, found two bad electrical components, replaced and safety checks conducted on hoist.
5200 Galena Station. Shaft Repair person went to un-chock a timber truck with their foot, slipped and sprained left ankle.
4900-081 lifting jackleg drill onto bolt deck, lower back started hurting. Going to get powder it got worse.
Electrical power (Avista Utilities - Spokane, Washington) went out at 3:35 PM from Osburn, Idaho to Mullan, Idaho. Power back on at 4:33PM. No. 3 Hoist back on line at 4:55PM.
#3 Hoist went down due to electrical issues in drive. Spencer Engineering was contacted - directed electrical department where to locate issue - replaced fuses and hoist re-started
4300 #3 Pocket, rock stuck in gate; muck overfilled the skip. No. 3 Hoist could not move skip. Skip cleaned out, No 3 Hoist up and running at 9:13 PM, 09/08/2020

No. 3 Hoist, Hoistman doing overspeed test, hoist shut down. Found overspeed buzzer underneath console shorted out causing hoist to shut down, replaced and relocated overspeed buzzer.
Coeur Hoist, scheduled maintenance to replace power switch, went to activate hoist, new switch would not energize. electricians traced out, bad fuse. Hoist up and running at 12:10 hrs.
Mechanic moving LHD tire, reported possible hernia on May 14th 2020. Mechanic thought it would get better over time - Requested to go see medical provider on July 14th 2020. Waiting for second opinion and surgical date.
3000-020 Chute - ore car overfilled while pulling muck, the handle on chute valve to open gate came off. While cleaning up mess, miner was using hands to pick up rocks. A rock fell from chute or ore car landing on miners left hand.

Coeur Hoist, when operating hoist, hoistman noticed had no dynamic braking. Basement electrical room the primary switch was found to be in the closed position. Crews were stood down during investigation.
Start of shift riding in conveyance in #3 Shaft. Miner experienced light-headedness, flushed feeling while riding down in the cage, miner fainted before reaching #3 4900 station
No. 3 Hoist kicked out - computer showed ""brakes not set"" Consulted with Spencer Engineering - mechanics reset limit switch on south brake ring - ran safety checks and shaft run, back up and running.
No. 3 Hoist - limit switch was out of adjustment on south brake track; was readjusted; during night muck hoisting limit switch was again out of adjustment due to expansion of brake track, computer shut hoist down.
Power bumps with heavy snow storm, No. 3 Hoist was pulling lead off 4900 pocket when it kicked out, would not reset. Contacted Spencer Engineering on computer faults. Electricians up replaced two electrical components.

Accident was reported on Wednesday July 31st. 5200-186 - pulling chutes, jumped off motor to pull chute twisting right knee when heel of boot caught on motor hitch.
5200-347 barring rock out of chute, felt sharp pain in groin, pain went slowly away, grizzling on 3700, moved piece of steel, pain came back.
5200-164 - Using powder knife to cut water hose; knife slipped and went into left thigh.
Avista power tripped out in town of Osburn, ID. Coeur Hoist powered up 1:00 PM, ran checks on hoist and shaft
No. 3 Hoist tripped out, would not reset. Called IT in Ontario Canada - long range dial stuck when taken out of man mode - reset manually to muck mode and re-set. Recommissioned shaft - back and running at 11:10 PM.
Hoistman moving cagers from 3700 pocket to 3700 Station to transfer more muck. Stopped at upper pocket the #3 hoist stalled out and would not reset. Brake limit switch had to be readjusted.
5200-346 Chute - barring overhead in chute, felt pain in right shoulder.
5200-184 Rock on face - went to pull it off, slipped and fell forward, rock slipped off face and caught middle finger of right hand between two rocks.
Operate Mucker 2-4 (250M) heading up incline to get another load of muck, looked back saw flames coming from end of mucker. Articulated mucker into rib and hit fire suppression system
3700 Galena Station - loading bolts onto cage, bundle of bolts was missing a band. Put gap filler in place to prevent bolts from falling down shaft. The bolts caught the filler going into the cage and the filler went down the shaft. Shaft was inspected and cleared at 30 minutes.

4900-210 Move wooden post with slusher, end of post caught rib came back hitting miner in lower right leg. Went to clinic - released to full duty on March 26th. Miner went back in on April 4th - doctor put on lost time till April 12th - elevate leg and have MRI done
5200 Grizzly - using double jack to break rocks on top of grizzly; felt strong pain in right hand.

Heavy wet snow caused tree limbs to fall and trip out power line coming to mine. No. 3 Hoist down from 10:30 PM January 20th to 12:45 AM January 21st.
Employee slipped and fell on ice/snow straining upper back, arm, and both legs. Went back on January 21st, Local clinic sent employee home for 3 days to rest and recover. *EE was diagnosed with fracture to left ankle on February 22nd; taken off work as of February 25th. Surgery scheduled after appointment on March 26th.

The air compressor used on hoist for pinion and accumulator is also used to aid in dumping stench. Valve was left open on stench manifold and compressor would not keep up creating air loss on the pinion brake, which faulted the hoist computer.
4900-132 Ramp - Mucker 2-11. Miner was operating LHD; mucking out heading when the LHD caught fire. The fire suppression system was activated and miner withdrew from heading and called the No. 3 Hoist. The mine was evacuated.
5200-159E; Muck & pad 159; trammed to 186 chute; felt sick, had headache, headed to 5200 #3 station, rode cage out at end of shift; stopped @ 2400 station to pick up miners, passed out in cage.
5200-206 glue back hole with dwyidag, PVC pipe slipped out of hands of miner; striking upper lip, laceration.
Cleaning out gob pile, move old rail with cable off bucket of mucker; guide mucker operator in to pull rail, slid down pinched foot under bucket.

No. 3 Hoist computer screen locked up, per instructions on screen - breaker was reset. Breaker would not reset - electrician turned main breaker off; everything reset.
4900-310 Load-out barring back down after blast, stepped back to look for better angle to bar; material from behind wire fell out hit miner on the shoulder.
Miner wash down raise, rock rolled off bulkhead striking miner on nose and left cheek
Hoist man moved conveyance without a release bell from cagers working on 4600 #3 Station. One cager was inside cage when it was moved and other cager was on station.
#3 Hoist Galena Mine - Pulling muck on #1 side, power dropped out, night shift crew on surface. Avista power line coming on site insulator broke and dropped power out back to Osburn Sub-station.
4300 Diamond Drill station - move gopher drill forward when base slid out, drill rocked ahead, miner tried to catch it and caught right arm on cable guard. Laceration to inner right elbow.
Pull lead on #1 side going up; hoist shut down. Computer would not let hoistman to re-set
A cam that releases the hoist brake did not fully engage causing the brakes to remain in prime. Safety's shut hoist down. Reset cams.
Pull lead off 5200 pocket, computer fault came on, no oil flow to inboard bearing. Maintenance on site - adjusted oil flow.
Hoistman performing pre-shift on #3 Hoist, heard ""pop"" then noticed the monitor screen in hoist was blank, unplugged and plugged back in and monitor came back on, but had tripped out a breaker in the mezzanine. Breaker reset and hoist was up and running again.
3400-049E prep and scale for rock bolting in raise stope, miner bolting back, material came out of crown of hanging wall hit left shoulder.
Hoist man heard a loud snap; ee tried to reset the brake wear fault. After failing to get a reset ee called maintenance, electricians and safety.
5200 Upper Pocket, spot cage at pocket landing step out, slipped on cap of landing.
4900-195 Incline, drilling with jackleg for roof bolting, machine came back striking miner with handle in right cheek, fracture right sinus
Wetting down muck pile in 5200-206 ramp, rock came out of rib striking left forearm, laceration to left forearm
3200 Chippy hoist person stopped conveyance on 3700 station. Short time later alarm activated and hoist could not be restarted. Code indicated brake was not set, the fact was that the brake set switch was not fully engaged.
3400-49RS install raise gate 20' down from top; nail lagging to side for gate to stop on when you close it. Had to hold lagging with left hand to nail in place, hit back of hand with poll of axe. Fracture middle left finger.
No personnel underground at time of incident. AC Line Contactor on No. 3 Hoist computer system failed, took electricians time to diagnose problem - spare contactor installed and hoist back up and running.
Abandoned 4600 ""G"" entry had a unconfirmed panel collapse - pushing the water out of the old workings; water went down No. 3 Shaft and Galena Shaft - concern over damage to No. 3 Shaft. MSHA on site March 9th to look at entry
Miner using snow rake to clear snow off Morning Shop open warehouse roof, snow rake came out of EE's hands falling back striking EE above right eye - laceration above right eye
4900-121 Shop, move full 55 gallon drum of oil off timber truck, barrel slid causing left little finger to be fractured between two barrels.
Miner claims carpal tunnel syndrome on January 2, 2018. Went on lost time as of March 1st 2018 for scheduled carpal tunnel surgery (both wrists).

Trying to break pipe loose, trip backwards over top of tails box in mill, caught foot on grating or discharge line, hit back of head against grating
52-164 bolting rib in ramp felt left knee sting a little bit looked at knee and was bleeding, laceration to left lateral knee.
No. 3 Hoist - hoisting muck when hoistman saw flash by 1750 HP motor, Hoistman shut hoist down; bar holding armatures came loose, tightened, run checks up and running at 1405 hrs.
3400-057RS Moving track hoe out of ditch, back hoe slid off 5 x 10 putting miner in rib, miner to clinic deep bruise to left knee physical therapy start on Nov 27.
3200-019 get personnel gear off locomotive, slipped on mud landing on rail, catching rail between fingers of right hand. Went to ER, negative x-ray, was released but Dr wanted PT to be seen at clinic, waited two days for appointment - at that time released to full duty.
Pipe line road from Galena to Coeur Mine - snow load dropped tree limbs on power line knocking power out to Galena Mine. Power off at 5:30 AM back on at 7:45 AM.
On Alimak deck up 5200-345 RS, rock bolting when face popped out hitting miner on right wrist. Miner wearing gloves and arm guards at time of accident.
5200 Ramp exhaust fan on mainline - miner on motor was in exhaust of the 100 HP fan - got material in left eye
Going through open air doors as swamper on ore train. Miner ducked head down to clear door, they didn't duck far enough - hard hat ear muffs caught on air door arms pulling face of miner into the pull arms.
On August 14th, 2017 miner was lifting 4' x 8' sheets of plywood, strained abdomen. Came in Tuesday, December 10, 2019 - wanted to see doctor concerning the injury in 2017, medical have taken miner off work till ultra sound had been completed.
Employee lifted 4' x 8' sheet of plywood resulting in hernia on August 14 2017. Went to surgeon in January 2020 - insurance accepted diagnosis and surgical repair as of February 6, 2020
Drilling pin hole with jackleg drill, pulled drill steel out of hole and smashed finger on rib - fractured middle right finger between drill and rib. ER did reduction on July 28th. Surgery on August 1st.
Hoist tripped out on main bearing temperature, cooling water line was plugged , electricians flushed the line reset and hoist was up and running by 0045 hrs June 22nd
No. 3 skip stalled above 5200 level, ""E"" brake set itself, when hoist man reset the limit switch would not reset. Electricians corrected.
