Struck by powered moving object

Plant was shutdown to address problem on #2 screen feed belt. The tail pulley was removed and loaded into the skid steer bucket to be moved out of the way. While backing and turning to the right an employee walked into a blind spot causing the skid steer bucket to make contact with EE's left knee.

Over-exertion (Not Elsewhere Classified)

Employee reported intermittent left knee soreness at the end of shift, but couldn't think of any incident or event that could have caused it and didn't think that it was work related. The employee sought medical care when the issue didn't resolve, and was advised that it could be related to repetitive pushing of the clutch pedal on the site's front end loader.

Employee was tightening the head nut on a Telsmith cone crusher with a large hammer. The employee placed their left hand in the line of fire between the hammer and the head nut. Their left pinky was struck when the hammer deflected, breaking the finger.

Struck by flying object

In process of removing cheek plate from jaw that was binded (jammed) up for removal, attached crane to cheek plate and applied pressure and cheek plate popped out and swung 5 feet and caught welder fabricator in face and knocked EE backwards. Worker got bloody nose and 5 stitches. Went to doctors and told to take day off.

Fall from machine

Employee was exiting a 980H front end loader and preparing to descend the ladder. As the operator turned to begin descent, lost grip on the handrails, missed the first step and fell to the ground. Treated and released until further medical evaluation. Further medical evaluation the following week recommended restrictions until next evaluation
