Fall to the walkway or working surface

Employee was walking to her personnel car to retrieve an item. As she was walking down the ramp she stepped off the matted area onto an area that had frost on it When she stepped on the frosted surface she lost her footing and ended up falling to the ground. During the fall she broke her leg in two places above ankle.

Employee was assisting in removing bearings from a screen. He took a 4 lb. hammer and struck the bearing race so the bearings would break loose, when a piece of metal entered his right eye. The employee was not wearing safety glasses.

Struck by falling object

While cleaning out underneath the primary crusher with a water hose, a small rock was thrown from the crusher striking the employee on the right side back of his head.

Employee was checking rock shaker to see if it was locked down. The counterweight was stopped on the topside, and when the employee moved the sheave, the counterweight rolled to the bottom, catching his hand between the belt and the sheave. This caused lacerations to the right index and second fingers.

Absorption of radiations, caustics, toxic and noxious substances

Employee notified us of accident on 2/21/05. Employee was standing in scale house; claims spider bite on the right inside leg.

EE WAS WALKING FROM PRIMARY CRUSHER HOUSE TO THE CONCRETE PAD BELOW THE PRIMARY CRUSHER. AS HE WAS WALKING, HE SLIPPED ON LOOSE GRAVEL FALLING ON HIS HIP.
