A skid steer bucket fell on employees fingers.

Employee was changing screens in plant when left knee bumped j-bolt and twisted slightly, causing discomfort. Did not visit dr initially. Worked with no issues until treated for knee swelling/pain on 3/13/2018.

Employee was washing rollers, attempted to climb onto a belt and slipped and fell approximately 6 feet. Employee did not follow fall protection procedures.
The employee struck a lace breaker with a 4 lb. hammer and the hammer bounced off the point he was hitting, striking him on the center of his right kneecap. Deep bone bruise - modified duty.
Employee strained back while using sledgehammer. After persistent pain, the next day (Mar 18) employee decided to seek medical treatment.
Employee states that while climbing the ladder on a haul truck he missed stepping down on the top step and fell to the ground. Nobbody witnessed the fall, and the ladder was not broken or bent.

Employee was climbing down the steps of a haul truck and on bottom step slipped and fell to the ground. The distance of fall was 23 inches.
Employee was standing on the bottom rung of the ladder for a loader trying to find an oil leak, slipped and fell landing on his left elbow resulting in a contusion to his left elbow.

EE WAS HELPING TO CARRY AN OXYGEN BOTTLE, AFTER SETTING IT DOWN HIS LOWER BACK MUSCLE CRAMPED AND STARTED TO HURT.

EMPLOYEE WAS ATTEMPTING TO LIFT A 35 LB BELT CLAMP TO START LACING A BELT. HE ATTEMPTED TO LIFT THE CLAMP IMPROPERLY AND FELT SOMETHING PULL IN HIS BACK.
