Employee alleged that EE slipped and struck their side on a handrail fracturing two ribs.

Employee was changing the lip guards on loader when a bolt on the underside of the bucket spun and cut through his gloves.

Employee reached around alternator guard on a Haul Truck & lacerated his finger on the alternator pulley.

EE WAS PLACING A PIECE OF SCREEN CLOTH(WIRE) ONTO A CHUTE ABOVE THE 4 1/4 CRUSHER, THE SCREEN CLOTH SLIPPED AND HIT TOP OF FEED PLATE KISKED BACK AND HIT EE IN THE NOSE

The ee was operating our water truck and the vent cover on the air condition unit fell off. The ee proceeded to replace the vent cover while the air condition unit was in operation and the fan blade struck his left middle finger resulting in a laceration requiring three stitches.

EE WAS USING A 5' IRON BAR TO CLEAN OUT CHUTE. ROCK CAME DOWN CHUTE AND HIT BAR CAUSING BAR TO STRIKE EE IN HEAD.

EMPLOYEES WERE CHANGING A 50HP MOTOR AT THE WASH PLANT SCREEN WHEN THE ACCIDENT OCCURRED. THEY WERE LIFTING MANUALLY. WEIGHT SHIFTED & CAUGHT EE'S THUMB BETWEEN MOTOR & HANDRAIL.

EMPLOYEE WAS CUTTING A PIECE OF RUBBER WITH A BOX KNIFE-PULLING TOWARD HIS BODY- ATTENTION CALLED AWAY FROM TASK AT HAND AND CUT RIGHT THUMB W/KNIFE.

EE WAS USING A SHOVEL TO CLEAN DIRT FROM UNDERNEATH THE CONVEYOR WHEN A DUST PARTICLE FELL INTO HIS RIGHT EYE. PARTICLE WENT BEHIND SAFETY GLASSES HE WAS WEARING AND CAUSED SCRATCH AND SLIGHT IRRITATION TO HIS EYE.
