Employee was stepping over cutting torch hoses at the crusher when employee tripped causing knee to pop. Employee stated employee didn't pick foot up high enough to clear the hoses. Employee was seen by a doctor and received light restricted duty.

Employee had just started stacking #50 pound bags from the industrial line when EE felt a sharp pain in left bicep area. EE was seen by a doctor and was diagnosed with strained arm muscles. EE has a follow-up with the doctor on June 29th and should be released back to work with full duty.

Employee bent over to pick up a busted product bag (weight approximately 20 lbs.) from the walkway and strained their lower back. Employee was swinging the bag while bent at the waist. Employee was seen in the ER and wrote out of work 48 hours then returned to full duty.

Stepped into the bucket of a skid steer loader to enter the operators cab. Right foot slipped when left foot caught the edge of the bucket causing employee to trip, twisting right foot resulting in a fracture to the right foot.
A temporary employee had emptied a portable dump hopper and was latching it back down when EE's finger got pinched between the lever and the wall of the hopper cutting the tip of EE's finger off. The employee was not wearing gloves at the time of the incident. EE was released back to the temp. service when EE admitted that EE could not pass a drug test.

EE had dumped a portable hopper with a forklift and when EE was latching the dump lever the forklift rolled forward onto EE's foot and ankle. No fractures, EE was written out of work until seen by an orthopedic. Orthopedic released EE to full duty on 11/12/2019 with a diagnosis of a sprain. Park brake on lift held while dumping then failed.

Employee suffered laceration to left, middle and ring fingers, as a pry bar pinched the fingers between the pry bar and housing.
EE was getting a pallet from a stack when the back corner of the pallet got hung on the pallet below, the EE jerked on the pallet and it came loose striking the rib cage causing a fracture.

Employee had been at work for an hour and was installing lid clamps. He stated he started feeling dizzy and was diagnosed with heat exhaustion.
Employee was attempting to open a jammed clam shell gate with the lever attached to the gate. The gate broke free and he mashed his finger between the lever and structure. He received stitches to his finger.
EE was working at the crusher attempting to help free a rock that was hung in the jaw. EE moved a rock which caught another lighter rock causing a smaller rock to strike EE's knuckle causing a laceration to the back of his left hand. EE received stitches.

Employee went up and over the rear fender of the truck from the concrete silo supports instead of correctly mounting from the rear where the safe access is provided. The fenders have flat slick surfaces.

Employee was climbing the ladder to the loader cab and slipped on the second step. He caught himself with his right arm on the handrail, regained his footing and climbed up to the cab. He later went to the ER where it was determined he broke his right forearm. He was talking with another employee while climbing and was distracted.

Another employee got irate with IE for not doing his job and physicaly assaulted him. The employee doing the assaulting was terminated and taken to jail and IE was taken to the ER by the EHS Manager where he received 3 stitches on the left eye brow. He returned to work and didn't miss any time.

DRIVER CLIMBED ON TOP OF HIS TRAILER AND WAS GOING UNDER A BEAM WHEN HIS MEASURING STICK STRUCK THE BEAM KNOCKING HIM OFF BALANCE. HE FELL ON HIS STOMACH AND THEN SLID UNDER THE FALL PROTECTION HAND RAILS. HE GRABBED THE RAILING BUT WAS UNABLE TO HOLD ON AND FELL ABOUT 8 FEET TO THE GROUND. HE STATED THAT HE WAS OK JUST A LITTLE SORE & DIDN'T WANT TO GO TO THE DOCTOR.
Injured was tapping a shaft out of a bearing. When it came out if smashed his finger between the shaft and the table. The basic cause was having his fingers under the shaft and the immediate cause was inattention to job hazard and improper hazard assessment of task. He was taken to the ER and received three stitches to his left index finger.

CLEANING WET BUILD UP OUT OF BUCKET ELEVATOR WITH THE ELEVATOR RUNNING. WHEN THE BUILD UP CAME LOOSE THE BAR WENT INTO THE BUCKETS CAUSING HIS FINGER TO GET CAUGHT BETWEEN THE BAR AND THE ELEV ATOR FRAME WORK.
