Employee was cutting expansion metal with a diamond wheel cutting disc on EE's grinder. While cutting the metal the grinder kicked back cutting EE's right hand on the thumb.

The employee was attempting to break loose a motor base bolt on slurry pump #3 with a wrench. When employee heard the muscle in right foreman pop.

Struck against stationary object

Employee's hard hat fell off and they bent over to pick it up when they raised back up they hit their head on the passenger side platform of a 950 loader.

Caught in, under or between a moving and a stationary object

The injury occurred when adjusting the impeller clearance on a slurry pump. While checking the rotation of the slurry pump shaft, the left thumb of the EE was caught between the v-belt and motor sheave.

Contact with heat

Employee was working outside on bench during a hot day. Employee was loading a shot and drinking fluids during the work day and taking scheduled breaks. However, employee felt overheated, started cramping and got sick near the end of the shift. The employee stopped work to cool off and drink fluids, but needed to be taken to the ER for IV fluids.

Struck by falling object

EE was performing maintenance on a set of screens. While inside the screen deck EE looked up and a foreign object fell between EE's safety glasses and EE's eye. EE washed it out and reported to supervisor. The next Monday (1/13/2020) it was bothering EE. At this time supervisor took EE to clinic.

Employee alleges that while operating the motor grader on either 3/21 or 3/22, its seat was too close to the controls, making ee sit ""too straight"" and causing back soreness. EE could not point to a specific time/date or incident that caused the soreness.

Struck by falling object

While employees were clearing out crusher, an employee was reaching over the rubber ring surrounding the crusher with a rod in their hand, and their head was looking over the rubber. A couple of rocks fell from somewhere above, hit the hard hat, knocked it off, and struck the employee in the back of the head.

The EE was replacing drive belts on the sand screw. He was standing at the end of the screw when he reached down to unscrew the bolt on a guard. He felt a pop on the left side of his rib cage. He reported it, but did not feel the need to go to the doctor until 8/13. He received a shot for pain on 8/13, and another for inflammation on 8/17. Released to full duty on 8/20.

Employees were replacing top stand rollers on C1A. As the two employees were pulling one half of the stand out, the stand twisted and caught his hand between the idler and the conveyor frame. We were notified on 6/1/2015 and the employee was taken to the doctor. He was diagnosed with a contusion to his right wrist and was placed in a soft brace.

Fall from machine

Crane operator was climbing into the cab of the crane and the metal hand grab broke causing the crane operator to lose his balance and fall approximately six feet to the ground. The operator sustained a minor fracture to his left arm.

Struck against a moving object

EE was trying to pry a large boulder loose with the loader in the pit. The bucket slipped on the boulder and jarred the loader. The right control arm would not lock in place and started moving back and forth while the EE tried to guide the loader. EE suffered a strain injury which was treated conservatively, but after further complaints medical treatment was prescribed on 3/25/11.

Fall from machine

While the employee was climbing the ladder on his haul truck, the employee said that his hand slipped and he started falling, landed briefly on his feet, and then fell to the ground on his back, rolling over onto his left shoulder.

EE & co-worker were in the process of removing an impact idler from conveyor. They slid the idler back to make room for removal. When they flipped the idler over, EE's finger was pinched between the idler and conveyor.

In process of changing cutting edges on D8 dozer, one man was on opposite side using impact wrench, while ee was on the other side putting pressure on bolts. Instead of using something to apply pressure on bolts, he placed his hand on bolts, cutting his left hand.

Employee bent over to p/u a cap and booster and felt sharp pain in back that radiated to right leg. EE tried heat/ice until 9/5 when his back was too sore and stiff. EE did not go to doctor and receive medical treatment until 9/5/06.

Maintenance crew was changing the cable on conveyor #27 at the load out plant. The cable was being removed in order to lower counter weight to the ground. The counter weight was being held by a front end loader. The counter weight shifted causing the conveyor to move and pinching the employees left foot.

Employee was climbing onto rock breaker, when he pulled himself up on the tracks he twisted his upper back. Three point contact was used.

Chute work under screen #7 had stopped up. Injured and other ee were washing out the chute. Other was on top washing under the screen and injured was below on the catwalk washing out the lower part of the chute.

Transformer weight was 8000#. Two employees were digging underneath the transformer to remove conduit and in doin so undermined the transformer causing it to fall. It hit one employee on his left side.

EE WAS HOOKING UP BUZZER AT PRIMARY CRUSHER. HE SAW THAT A HAUL TRUCK HAD ALMOST RUN OVER A SHARP, POINTED ROCK. EE WENT TO MOVE THE ROCK. WHEN HE CARRIED THE ROCK OVER AND PITCHED IT OVER THE BERM, HE FELT PAIN IN HIS LOWER BACK.

IN AN EFFORT TO CLEAN THE RIGHT CAB MOUNTED MIRROR OF A CATERPILLAR 775B TRUCK, THE OPERATOR EXITED THE CAB AND ATTEMPTED TO CROSS THE HOOD IN ORDER TO GAIN ACCESS. WHILE CROSSING HE SLIPPED AND FELL TO THE GROUND CAUSING THE INJURY.

WHILE HOLDING A 2"" X 4' BOLT WITH HIS LEFT HAND, EE WAS TRYING TO UNSCREW THE NUT WITH HIS RIGHT HAND. THE BOLT SLIPPED AND SMASHED HIS MIDDLE FINGER.

EE WAS TRYING TO CLEAR 3 BIG ROCKS FROM THE 7' STD WITH A PRYBAR. WHEN THE BAR CAME IN CONTACT WITH THE ROCK AND THE CRUSHER, THE BAR JERKED INTO THE CHUTE WORK. EE'S HAND (RIGHT HAND) WAS CAU GHT BETWEEN THE CHUTE AND THE BAR.

THE OBJECT WAS SUSPENDED FROM THE OVERHEAD HOISTAS EE WAS SLIDING THE OBJECT THE VIBRATION ON THE FLOOR OF THE JAWBOX CAUSED A ROCK TO SLIDE BETWEEN THE GRIZZLY BARS AND DEFFECT OFF THE SLIDE PLATE UNDER THE GRIZZLY HIPS. AS IT FELL INTO THE JAW BOX, IT CLIPPED EE RIGHT KNEE.

DRILLED 45' DEEP HOLE; CAME OUT OF HOLE WITH STEEL; BROKE FINISHING ROD LOOSE FROM STARTER ROD. PICKED UP FINISHING ROD TO PUT INTO CARASEL. I WAS HOLDING THE STEEL TO LINE IT UP WITH THE CUP IN THE CARASEL. AS I WENT TO TURN THE CARASEL TO LINE IT UP THE PIECE OF STEEL SLAMMED MY FINGERS AGAINST THE TOWER.

EE DROPPED AN INK PEN INTO A HOLE ON THE FLOOR OF HIS MACHINE. HE ATTEMPTED TO RETRIEVE THE PEN. ON THE WAY OUT OF THE HOLE WITH THE PEN, HIS HAND CAME INTO CONTACT WITH A BLADE ON THE AIR CON DITIONER FAN. THE BLADE STRUCK THE FINGERNAIL.

EE WAS STANDING ON CONVEYOR #5 TRYING TO REMOVE THE ROCK THAT WAS WEDGED IN THE BACK FEEDER OF THE SECONDARY TUNNEL. HE WAS REMOVEING THE ROCK THAT WAS WEDGED IN THE BACK FEEDER WITH A JACK HA MMER. WHEN HE TURNED, HE SLIPPED ON LOOSE ROCKS, CAUSING HIS KNEE TO TWIST.
