Employee stepped out of the cab of the truck to clean the mirrors when EE noticed the truck was rolling. While re-entering the cab, EE's left hand was pinched between the cab handrail and the truck bed in front of EE.

Employee reported eye irritation several hours after leaving the job site on 11/7/2022. Sought medical treatment the next morning, was prescribed prescription antibiotics.

While assisting the drill operator in removing a piece of starter steel on a DM30, employee was struck by an object and knocked off the drill and onto the ground.
The employee was helping install a center pin on a 988 H. EE was using a sledge hammer to check for movement in the pin. On the last swing EE's hand caught a sharp edge on the side of the machine. The injury resulted in a broken tip of the left index finger and a laceration that required nine stitches.

Trackhoe op. called supervisor to exchange trackhoe. Supv walked new trackhoe to the cut. Supv parked new trackhoe beside trackhoe that was in production. Supv was attempting to walk trackhoe off cut & moved tracks in wrong position. Machine walked off 8' cut & overturned trackhoe as a result. Supv slammed rt shoulder in cab w/seatbelt on, breaking shoulder socket.

EE was in process of changing out troughing idler on 30"" conv. belt. While performing this task with a co-worker the idler tilted down and came in contact with the conv. frame. EE's right middle finger was mashed between the idler and the conv. frame. EE's right middle finger,last joint recieved approx. 11 stitches.

EMPLOYEE WAS LOADING SHOT ROCK WITH A 992 JOHN DEERE TRACKHOE. HE DROPPED A ROCK ONTO ANOTHER ROCK; WHEN THE ROCK COLLIDED, A FRAGMENT BROKE OFF, ENTERED THE CAB AND STRUCK HIS FINGER CAUSING A GASH ON HIS RIGHT RING FINGER.
