Fall to the walkway or working surface

At 9am driller observed EE's knees buckling and going to the ground. EE insisted being fine just tired from lack of sleep. He finally agreed to be driven to hospital. He was admitted for heat exhaustion and severe dehydration. Later diagnosed with knee contusion and hairline ankle fracture from fall. He was released 8/16. His return to work is pending medical release.

Struck by falling object

Empolyee was performing cleanup in the plant near the secondary crusher when the crusher backed up causing material to overflow. The overflowing material struck the employee on the left side of his back above the belt line.

*Please see the attached document.* Customer truck left on property overnight was found completely burned the next morning. The truck had suffered mechanical problems.

Struck by falling object

Employee was replacing bolts in the grizzley bars. He was removing rocks and layed his hand on the side of feeder when a rock fell onto his hand, cutting his left index finger on the inside.

Struck against stationary object

Employee was exiting the 66 crusher platform and stepped on a golf ball size rock and turned his rt. ankle. Employee sought medical treatment and became recordable on 03/24/2006.

Fall to the walkway or working surface

Employee tripped and fell while holding a 50-60 drill bit. Injury result was lower lumbar strain.

Contact with heat

Employee was on yard, working inside a screen and began experiencing dizziness. Employee was transported to hospital and given IV fluids.

Struck by flying object

Employee was painting guards when a piece of metal flew into his eye. Employee was wearing safety glasses. THe metal was removed, but the employee recieved a prescription.

EE WAS ATTEMPTING TO REPAIR A HOLE IN A ROCK CHUTE. WHILE CONDUCTING THIS TASK, WELDING SPARKS CONTACTED THE EE'S LEFT HAND CAUSING HIM TO DROP THE PLATE, & PULL HIS LEFT ARM DOWN. THIS CAUSED THE EE TO STRAIN HIS LEFT SHOULDER.

EE WAS USING A SLEDGE HAMMER TO ADJUST BOLTS ON THE PRIMARY JAW CRUSHER. WHILE SWINGING THE HAMMER THE EE FELT A SHARP PAIN IN HIS LOWER BACK.

EMPLOYEE WAS MOVING STOCKPILE WITH A LOADER WHEN A CONSUMER TRUCK PULLED IN BEHIND HIM. THE OPERATOR TURNED HIS HEAD SUDDENLY AND STRAINED HIS NECK.

EE WAS ATTEMPTING TO CLIMB UP ONTO THE JAW PITMAN TO PERF9RM MAINTENANCE ON THE SHAFT OF THE JAW PITMAN. IN DOING SO, WHILE THE EE WAS REACHING FOR A HAND HOLD HE ACQUIRED A SHARP PAIN IN HIS LOWER BACK.
