EE was cutting wedge bolt from stationary jaw dye. While cutting the wedge bolt, the bolt sheared and made contact with EE's welding hood that they were wearing. The frame that holds the lens in the welding hood broke loose and lacerated EE above their left eye. EE received 12 stitches above left eye.

Reinstalling tail pulley guard with another miner when guard slipped and cut finger thru gloves.

On 10/21/2020 employee was found lying on the ground near the water truck. EE stated was unsure as to what happened and conveyed to EMS that EE had previously experience a TIA several years ago.
Employee was operating yard loader. As EE traveled across the yard the employee hit a bump, which resulted in pain to the lower back.

Employee was team lifting a nitrogen bottle. The employee slipped and fell. Employee put a hand out to catch fall. The nitrogen bottle landed on thumb. All correct ppe was being used including impact gloves.

The employee and a contractor were welding tread plates on the scale. The employee awoke during the night with flash burns, but he did not report having any issues with his eyes immediately after welding. It appears that welding flash entered welding shield while performing back to back welding.

EE was enlarging a hole in metal beam to install stairway with a cutting torch. As EE was removing torch from between stair treads, the torch tip got caught in the cuff of his welding gloves, causing 1st and 2nd degree burns to the wrist.
At approximately 9:30 pm the EE was repairing laces in the #7 belt using a die grinder to cut the laces. The trigger on the die grinder stuck in the on position and wouldn't release. Upon reaching around to unplug the grinder, it dropped down, struck, and lacerated his left knee.
At approximately 10:30 AM this morning, the employee was attempting to assist another employee roll a drop chute that was being painted for installation in the new Newton County Plant. A contractor was also assisting in this effort. The chute they were attempting to roll was setting on two 4 x 4 wood posts.

The Employee was in the process of removing a conveyor belt tail pulley guard. All of the guard fasteners had been removed and most of the bolts had been removed but two bolts were in a bind and would not move, he attempted to move the guard and the bolts finally broke loose the guard fell and struck the employee. He recieved a tetnus shot and 1 day modified duty.
3/14/07 the employee was in the process of shimming the jaw in order to close the setting. While in the process of shimming the jaw the employee struck his left knee on the two inch shim. Employee did not notice any pain until the next morning. Employee told his supervisor the next day, and went the following Monday to a company physician on 3/19/07.

EMPLOYEE WAS REMOVING A HANG UP FROM THE PRIMARY JAW CRUSHER USING A ROCK WEDGE. THE WEDGE KICKED BACK DRIVING THE WEDGE HANDLE INTO THE OPERATOR'S CHIN CAUSING A LACERATION REQUIRING MEDICAL ATTENTION.
