The employee was using a sledge hammer to break up a rock. When employee hit the rock, a sliver flew off and struck another employee in the lip causing a laceration.

The employee was cutting a piece of wear metal away from the bed of a large quarry haul truck, when it came loose from the truck bed and sprung past the pry bars placed below the metal plate for foot protection. The metal plate cut through the employees uniform pants and the top of work boot causing a laceration to the top of right foot.

After the mechanic hooked the jumper cables to the batteries on the excavator he told the operator to start the machine. When the operator turned the ignition to start the machine the mechanic noticed a puff of smoke come from a filler cap. Upon noticing the smoke the mechanic turned his face away from the area. The battery then exploded causing loss of hearing in right ear.

While cutting a weld with a cutting disc on a 4"" grinder the disc caught an edge breaking the disc and caused the grinder to kick back. EE lost grip and the grinder landed on his left forearm causing abrasions and lacerations.

While stringing out an extension cord the employee stepped from the chute deck to the motor deck where he stepped on a rock and twisted his left ankle.

Employee was assisting the Orica blasting crew in getting a shot prepared. He was cutting a blasting tube to hold the shock tube. He was cutting downward and it only cut a couple of inches. He turned the knife around and cut upward on the tube. As it came through the tube the knife stuck in his left wrist.

Flash burns (electric)

Phillips Electrical Technologies EE was drilling a hole to relocate a ground bar in an electrical panel. EE received an arc flash on his face, neck and arms. He was transported to Grady Hospital in Atlanta, GA for treatment of second degree burns and is expected to be released on 9/29/06.

Struck by falling object

An employee was cutting out a liner in a crusher. The safest way to remove the liner was to cut it up into 12"" to 18"" sections while positioned out of the way and allow them to drop to the belt. The first section was cut, and apparently hit another section along the way down to the belt causing it to take an unusual flip landing on the employee's right foot.

Operating crane (50 ton grove) rigged to 5700 lbs truss, moving 100' downhill in reverse, load in uphill position. Started to descend air pressure 120 lbs, crane in low gear reverse, traveled 30' to 40' down incline boom full retracted. Lost pressure due to mechanical failure. Brake max ruptured.

Fall from machine

The employee was standing on a 980 G loader talking to the operator and then attempted to egress off the machine, lost his grip and fell approximately 6 feet to the ground. The employee broke his right hip.

Employee began experiencing back pain while installing wear liners in chutes. Initially, employee did not report or seek medical treatment until Saturday the 25th. The employee has a low back muscle sprain and is on modified duty and has multiple perscriptions.

The employee was using a hammer to remove a wedge bolt on a crusher. The hammer struck the side of the crusher moving the direction of travel hitting his left wrist. The employee did not seek medical treatment until 8/3/04. The diagnosis was a contusion to the wrist, no modified duty, but he recieved a perscription for swelling.

Employee was repairing a belt, lost his footing and fell. He had an electric impact in his hands, and when he fell he dropped it and it bounced striking him in the mouth. He recieved seven stitches.

EMPLOYEE WAS REPAIRING SKIRT RUBBER AND A SMALL ROCK ROLLED DOWN FROM A CONVEYOR OVERHEAD AND STRUCK HIS ANKLE.

EMPLOYEE WAS USING A RAZOR TO CUT A PIECE OF HOSE WHEN THE KNIFE SLIPPED CUTTING THE EMPLOYEE. HE RECIEVED STITCHES FOR THE CUT.

OP WAS PUSHING ON BIG ROCK WITH DOZER AND DOZER BLADE SLIPPED OFF ROCK. BACK OF DOZER SLAMMED DOWN CAUSING BACK INJURY. EE WENT TO EMERGENCY ROOM, DIAGNOSED WITH BACK STRAIN, GIVEN MEDICATI ON AND SENT HOME.

EE WAS USING A PICK TO CLEAN OFF A CATWALK. WHEN THE EE SWUNG, HE STRUCK A HARD ROCK & CAUSED HIM TO STRAIN HIS ELBOW.

THERE WERE MULTIPLE EES WELDING IN A CLOSE QUARTER AREA FROM DIFFERENT ANGLES. AS THEY WELDED, THE LIGHT FROM THE WELDERS REFLECTED BACK AND CAUSED THEM TO GET FLASH BURN.

OPERATOR WAS LOADING IN A MUCK PILE, AS HE WAS BACKING UP FROM THE PILE A ROCK WENT UNDER THE BUCKET. WHEN THE LOADER CAME OFF THE ROCK IT CAUSED THE MACHINE TO SHIFT FROM SIDE TO SIDE. THIS CAUSED THE OPERATOR TO STRIKE HIS HEAD AGAINST AMETAL BOX INSIDE THE CAB OF THE LOADER.

WHILE WELDING, WIND BLEW DEBRIS IN LEFT EYE UNDER WELDING HELMET. USED EYEWASH ON SEVERAL DIFFERENT OCCASIONS; EYE IRRITATED AND DEBRIS STILL IN EYE. AT THIS POINT, SOUGHT MEDICAL ATTENTION.

THE MAIN ROCK CHUTE, UNDER THE MAIN ROCKER SHAKER IN THE PLANT WAS STOPPED UP. TWO EES LOCKED OU THE NECESSARY EQUIPMENT & WHILE OBSERVING THE SITUATION, EE PLACED HIS HAND ON THE ROCK CHUTE. IN DOING SO, THIS CAUSED THE ROCK TO SHIFT. ALSOA LOOSE STONE CAME DOWN & STRUCK EE ON HIS RIGHT INDEX FINGER & PINKY FINGER.

EMPLOYEE WAS STANDING IN A DISCHARGE BOX, MEASURING A HEAD PULLEY FOR A GUARD. HE SLIPPED AND OTHER EMPLOYEES GRABBED HIM TO KEEP HIM FROM FALLING. IN THE PROCESS HE INJURED HIS RIGHT SHOULDER.

EE WAS ATTEMPTING TO SET A CONCRTERWEIGHT ON A CONTRACTORS BACKHOE. WHILE IN THE PROCESS THE CRANE TIPPED. THE EE HAD THE CRANE IN THE TRAVEL POSITION WHICH OVERRIDES THE SAFETY DEVICES ON TH E MACHINE. ALSO THE CRANE WAS SET UP ON UNLEVEL GROUND AT AN UNLEVEL POSITION. THIS ACCIDENT CAUSED THE EE TO EE HIS WRIST.
