Tip of employee's right pinky finger was partially amputated after being caught between a v-belt and motor sheave connected to the jaw crusher flywheel. The flywheel counterweight was not resting in the down position after lockout. When the employee checked the tension on the v-belt, it caused the flywheel and belt to move and caught EE's finger in the sheave.

Employee was removing the gear box from a head pulley. Employee was using a 3 ft 6 lb soft face hammer. EE struck the bearing in an up to down motion. When ee hit bearing it shattered in multiple pieces. One of the pieces struck the employee in left upper arm, approximately 4 inches in length.

Employee was positioning hydraulic ram on 4 1/4 cone crusher to tighten bowl down. As ram retracted employee did not realize his finger was in the way of the moving cylinder. As the cylinder closed the tip of his right index finger was crushed.

While cleaning tracks on excavator part of the debris came over safety glasses into eye. This was due to material being frozen more force was used to get it to break loose. Attempt was made previous day but again it was frozen. Employee sustained a scleral abrasion to the right eye.

WAS TIGHTENING THE TENSION ROD AT THE PRIMARY CRUSHER WHEN THE ROD BROKE, AND THE TORQUE EXTENSION HANDLE END RAKED ACROSS HIS HAND CAUSING A DEEP LACERATION REQUIRING STITCHES TO CLOSE.
