Employee was trying to get into the bucket truck when employee's right leg slipped and folded employee's left knee into a squatted position. Employee did not go to the doctor until the following day.

Repositioned a piece of steel to be cut. Steel then slid and landed on top of hand.

Employee was changing drill bit on DM30 drill. The driller dropped the drill bit onto his finger.

Went to catch the chute on the bulk truck & struck left index finger. On 6/3/05 lost feeling with the middle/index finger. Reported claim to the employer on 6/3/05.

Fall from machine

Employee was raising gyratory crusher door with electric wench. The cable broke; ee jumped to make sure he was clear of the cable and fell onto his back.

The metal access door for the gyratory crusher came loose and fell into the primary feeder. The door was cut into two pieces. One piece was successfully removed without incident. The second section of the door shifted as it was being lifted and smashed the employee's thumb.
