The employee's index finger was injured when EE was changing the wear shims on the drill head. EE was prescribed a brace for EE's finger making this a reportable injury.

The employee slipped while descending a loader and pulled shoulder muscle. EE did not receive medical treatment at the time. The employee later went through shoulder surgery, which made the injury reportable. We are meeting the 10 day MSHA requirement.

EMPLOYEE WAS ATTEMPTING TO ADJUST THE SHELL ON THE CRUSHER - IT WAS SET IN PLACE BY THE CRANE AND WAS NOT LEVEL. WHEN HE MOVED THE CYLINDER, THE SHELL FELL INTO PLACE, CATCHING HIS HAND BETWEE N SHELL & ADJUSTMENT CAP.
