Fall onto or against objects

Operator entering a loader. While ascending the ladder,foot slipped off rung and hand then slipped off the Grab bar causing him to fall into the loader hitting his nose and forehead on the seat panel. The employee nose bleed and became dizzy. drove the employee to the hospital. His nose was swollen and possibly broken.Prescription medication was given once.

Employee was performing a preshift inspection on the excavator. As he was walking back to cab on the catwalk, he grabbed the cab door. The door came unlocked, swung out and knocked him off the catwalk. We have begun to add handholds to the top of the cab.

Contract employees were at JM Huber - Wrens 17C mulching the dam with hay blower and tractor. It was windy and some of the hay blew under the tractor and came in contact with the exhaust, causing the tractor to catch on fire.

BACKHOE OPERATOR WAS LOADING CRUDE CLAY TRUCKS OFF OF THE TOP SHELF (APPROX. 8 FEET) WHEN HE GOT THE BACKHOE TOO CLOSE TO THE EDGE AND THE MACHINE SLIPPED OFF ON THE RIGHT SIDE. WHEN ATTEMPTING TO RELOCATE THE BUCKET TO BETTER SUPPORT THE MACHINE, IT SLIPPED DOWN VERY SLOWLY APPROX. 1 FOOT AND THEN SLOWLY ROLLED OVER ONTO ITS RIGHT SIDE.

STEPPED DOWN FROM THE DOCK OF A SMALL FUEL TRAILER. STEPPED ONTO THE TONGUE OF THE TRAILER. THERE WAS DEW ON THE TONGUE OF THE TRAILER, HIS FOOT SLIPPED & HE FELL ACROSS THE TONGUE.

EMPLOYEE WAS EXITING TRUCK AND FOOT SLIPPED OFF OF CAB STEP RESULTING IN FALL OF 4' TO GROUND. SURFACE WAS RED CLAY EARTH. POSSIBLE CLAY ON SHOES MAY HAVE RESULTED TO FALL

EE WAS PERFORMING AN EQUIPMENT SERVICE ON AN EXCAVATOR. HIS FOOT SLIPPED OFF STEP WHILE STEPPING DOWN FROM EXCAVATOR. (GREASE ON BOTTOM OF EE'S SHOES.) EE FELL ON GROUND. EE SUSTAINED A SPRAINED HIP, PER ORTHOPEDIC SPECIALIST. EE'S WORK AREA SHOULD HAVE BEEN MAINTAINED IN A CLEANER MANNER TO PREVENT GREASE FROM BEING ON SAFETY BOOTS.

EE WAS OPERATING DUMP TRUCK, ATTEMPTING TO DUMP LOAD IN AN UNLEVEL AREA. WHEN BODY OF TRUCK WAS RAISED THE TRUCK LEANED OVER ON ITS SIDE.

EE WENT TO TELL ANOTHER EE SOMETHING AS HE WAS CHANGING THE TIRE ON THE MOTOR GRADER. WHEN EE TURNED TO WALK OFF, THE LOCK RING ON THE TIRE CAME LOOSE AND HIT HIM.

EE STRAINED LEFT SHOULDER WHILE USING A ""POKE POLE TO DISLODGE KAOLIN CLAY WHICH HAD BRIDGED OVER IN A SHREDDER HOPPER. THE ""POKE POLE"" IS A PIECE OF 3/4"" IRON PIPE ABOUT 10' LONG. IN ORDER TO DISLODGE THE KAOLIN, THE POLE MUST BE LIFTED UP AND DOWN. EE INJURED HIS SHOULDER WHILE DOING THIS.
