Truck Driver attempted to dump trailer on uneven surface. Trailer overtuned.

Kenworth tractor trailer truck was in the shop being repaired. The mechanic was letting the hood down, she did not know the driver was trying to get a shop light from the top of the front tire. The mechanic eased the hood over striking the driver causing bruising of the upper body area.

Foreman was cleaning up the cut with a Cat D6M dozer. He went over a small hump of dirt, his cell phone engaged the parking brake on the left arm rest causing the dozer the stop suddenly. The foreman had his seatbelt on, the stop caused his neck to snap forward. He was stunned for a few minutes, then drove himself to the doctor.

While lowering drill pipe to connect tool joint, helper put left hand under drill pipe being lowered to remove dirt. Middle finger was jammed between drill pipe causing laceration of finger. Helper was wearing gloves.

EE WAS REPLACING THE HOIST FRICTION BAND ON LARGE DRAGLINE #17. THE TASK REUIRES THAT THIS DEVICE BE HOISTED OVERHEAD AND MOVED TO OUTSIDE AREA. DURING THIS PROCESS, THE DEVICE FELL DUE TO INADEQUATE RIGGING. IT DEFLECTED OFF THE MAIN GEAR BOX AND STRUCK EE CAUSING A SPRAINED WRIST AND BRUISED RIBS

EE WAS ATTENDING TO HIS NORMAL OILER ROUTINE WHICH CONSISTS OF RUNNING A DOZER & DRAGLINE SERVICE & MAINTENANCE, WHEN HE DISCOVERED THAT HIS EYE WAS IRRITATED. HE THOUGHT IT WAS NORMAL EYE IRR ITATION & DIDN'T REPORT IT TO HIS SUPERVISOR. HE WENT HOME AT END OF SHIFT & THE EYE BECAME PAINFUL & HE HAD TO GET MEDICAL ATTENTION. THE WEATHER WAS VERY WINDY & HIS SAFETY GLASSES DIDN'T P

EE, #17 DRAGLINE OILER WAS WALKING DOWN THE DRAGLINE LEVER - STAND STEPS WHEN HE TWISTED HIS LEFT ANKLE ON THE LAST STEP. HE STATED THAT ONLY HIS LEFT HEEL MADE CONTACT WITH THE LAST STEP, CAU SING HIM TO TWIST AND SPRAIN HIS LEFT ANKLE.
