The employee was attempting to remove a pin from the loader bucket, when striking the pin, a piece of metal came off striking them in the neck causing a laceration.

As the employee was aligning the piece of drill steel to lower, the bushing fell approximately six inches pinning his left pinky and ring finger between the table and bushing, resulting in fourteen stitches.

EE WAS ABOUT TO HOOK A CRANE TO THE TOGGLE PLATE FROM THE JAW CRUSHER. THE TOGGLE PLATE ROLLED IN ITS SEAT AND FELL ON BOTH OF HIS FEET PINNING HIM UNDER IT. THE PLATE WAS NOT BLOCKED UP. THE PLATE WAS LIFTED OFF OF HIM AND HE WAS TRANSPORTED TO THE E.R. VIA AMBULANCE.
EE WAS BEGINNING TO HOIST AN I-BEAM USING A CHAIN HOIST WHEN THE LOAD SHIFTED AND THE I-BEAM ROLLED COMPLETELY OVER FALLING ON THE TOP OF HIS FOOT.
