The miner was loading cribbing material, 30-40lbs per, into the bed of service truck when the rope handle attached to the cribbing wrapped around the tip of right middle finger. The miners right middle finger from first joint to tip was broken and degloved.

The employee was attempting to remove a pin from the loader bucket, when striking the pin, a piece of metal came off striking them in the neck causing a laceration.

Employee was working on the 990 loader door safety latch. When he reached toward the door to remove a nut which was directly in front of him he felt a pop in his right shoulder.

Employee was installing a table bushing. Bushing became stuck on the second piece of drill steel. As the employee was aligning the piece of drill steel to lower, the bushing fell approximately six inches pinning his left pinky and ring finger between the table and bushing, resulting in fourteen stitches.

Employee was changing out belts on a conveyor drive motor, when the metal railing he had the come-along attached to broke, causing his finger to be pinched between the belt and pulley. The injury required stitiches.

EE WAS ABOUT TO HOOK A CRANE TO THE TOGGLE PLATE FROM THE JAW CRUSHER. THE TOGGLE PLATE ROLLED IN ITS SEAT AND FELL ON BOTH OF HIS FEET PINNING HIM UNDER IT. THE PLATE WAS NOT BLOCKED UP. THE PLATE WAS LIFTED OFF OF HIM AND HE WAS TRANSPORTED TO THE E.R. VIA AMBULANCE.

EE WAS BEGINNING TO HOIST AN I-BEAM USING A CHAIN HOIST WHEN THE LOAD SHIFTED AND THE I-BEAM ROLLED COMPLETELY OVER FALLING ON THE TOP OF HIS FOOT.
