Driver attempting to dislodge lump of crude by hand smashed 5th digit left hand in tailgate.

Upon completing removal of a stick from the boom of a CAT 336 excavator, technician was gathering tools when a hydraulic pipe (which was bound by removal) sprung loose smacking right thumb against hard line mount bracket

Fall from machine

Miner was climbing onto an excavator and slipped and fell to the ground injuring right hip and lower back. The ground and track of the excavator was wet from an overnight rainfall.

Employee has been experiencing knee pain for over a week and has related the place that they believes the pain started was at Dixie Mine on 8/19/2020. Employee states that they kneeled down on right knee and felt like they had kneeled on a rock. EE then states that they had pain on the outside of the knee. Reported 8/27/2020.

Employee was exiting dozer, feet slipped out from under employee and fell on tracks. Employee went to ER complaining of back pain, x-rays are negative but employee is now scheduled for an MRI and currently waiting for MRI.

Mechanic was mounting a Cat 349 Excavator to repair a hydraulic leak behind the boom. Mechanic's right foot slipped and caused him to twist his left leg. Mechanic experience pain in his left knee which result has resulted as a sprain knee.

EE was operating the water truck on the haul road. He started up the ramp entrance to the pit area, as he got to the top of the hill, his truck started spinning on the wet road. The truck slip ped backwards, down the ramp and hit the berm, which caused the truck to turn over. There was cab damage to the truck.

Employee was driving a 10 wheel dump truck on haul road at Dixie Mine. Road surface was wet clay. As vehicle was negoiating a curve, the vehicle started slidding. Vehicle rolled over.

OPERATOR WAS ATTEMPTING TO SHUT LEFT DOOR ON MOTORGRADER, WITH HIS RIGHT HAND HOLDING CAB POST HE LEANED OUT AND PULLED DOOR SHUT. THE DOOR CLOSED SHUT ON HIS RIGHT THUMB, BREAKING THE BONE AN D CAUSING A LACERATION REQUIRING SIX STITCHES.

EE WAS STARTING UP A CLASSIFIER WITH JOA SWITCH, LOCATED ABOVE THE CLASSIFIER, WHEN A JUNCTION BOX FELL SUDDENLY AND MAD CONTACT WITH THE EE'S RIGHT LEG ABOVE HIS RIGHT KNEE.

EE SUSTAINED A SEVERE LACERATION TO HIS RIGHT THUMB WHILE WORKING IN A BLUNGER REMOVING A BLADE. WHEN HE KNOCKED THE BLADE OFF, IT STRUCK AND CUT HIS THUMB.

AN APPRENTICE MECHANIC WAS ASSISTING A 1ST CLASSMECHANIN REMOVING A SET OF BLUNGER BLADE FROM #1 BLUNGER. THE INJ EE APPRENTICE MECHANIC LIFTED THE SET OF BLADES UP ON 1 SIDE TO ATTACH A STR AP TO THE BLADES WHEN THE BLADE SLIPPED FROM HISHAND & LACERATED IT. HE WAS WEARING GLOVES
