Mild chest pains began and intensified over the course of approximately thirty minutes.

Employee was climbing down 2 step Ladder/platform with handrails and foot slipped. Employee stated they were holding on to handrails when this happened and sudden drop of body weight must have hurt their left shoulder. The rung on ladder was made out of Stainless-steel pipe and may have been wet at time.
Employee said they felt discomfort in shoulder after clearing a jam on pallet feeder. Employee did not seek medical attention until 28 Aug when prescription medications were given.

The crew had just finished drilling the hole & had begun to grout the surface. After mixing the 2nd batch of grout a co-worker turned on the pump. As soon as it was turned on, the hose whipped out of the EE's hand & struck the EE in the face/mouth resulting in missing teeth, several cracked teeth, & a laceration to upper lip.
Employee opening manual valve when hand slipped, and arm struck adjacent object. Employee sought medical/first-aid on 8/29 and 9/8. Prescribed medication on 9/26.

Employee was adjacent to dock while equipment was being unloaded from a commercial truck with a forklift. The pallet that the equipment was on failed causing it to fall cutting the employee's hand and fracturing their wrist.

Employee was on site at BASF Mid Ga facility when EE seemed to pass out and hit the ground. EE was not performing any task at the time. EE received stitches at the hospital. EE was never in a life threating issue.

Employee was walking on flat, dry, clean surface at bagging operation when they tripped over their own feet and fell on same level. Operator extended both arms while falling in an attempt to stop fall. The right hand struck a metal frame causing a laceration that required 7 stitches. Operator states they were wearing gloves at the time of the fall. Restricted duty, no lost days.

Operator was walking in packaging area with the flow of forklift traffic. Forklift driver was carrying a pallet load of packaged product. Forklift driver struck operator on left side knocking operator to the ground. Operator suffered bruising, skin abrasions and pain in left shoulder.

Employee was walking to his vehicle in the parking lot around 7:00 PM when he stumbled and fell. The employee landed face down on the asphalt parking lot hurting his knee in the process. He was placed on restrictive duty as per doctor's orders.
Mill process operator received a laceration to his right lower leg when one of the concrete support corners under a U-drain cover collapsed. The operator's leg came in contact with the U-drain wall causing the laceration that required several sutures to close.

Mechanic walked around the Roving Crew truck to get something out of the tool box when he struck the bumper-mounted vise with his right knee. EE sought medical treatment on 10/8/07.
EE sustained laceration to head due to contact with grating as a result of a fall of unknown cause, which was recently deemed by treating MD to be the result of EE having a ""TIA.""
Employee was working in a tank, he turned and began to slip on some sludge in the tank. Employee turned to catch himself but his feet stuck causing him to fall and twist breaking his left leg.

While working a shut-down providing construction management, a leaking hose was spraying slurry on asphalt on a slight incline creating a slick surface. While trying to get the attention of the production workers to report the problem, she shifted wait from one leg to another and lost balance. The fall caused her fibula to break.

EE was removing a pressure washer from an upper level floor & it was being lowered down the steps to bottom floor for repairs. The EE used the hoist to raise the front of the unit to get it off the handrail when the hook connected to the handle & slid over pinching his rt. little finger causing an avulsion to the distal end.

A PIECE OF CARDBOARD WAS COVERING A HOLE AND WHEN HE STEPPED ON IT, THREW HIM OFF BALANCE.
AN APPRENTICE, MECHANIC BELOW STATED EE RECEIVED A LACERATION TO HIS RIGHT FOREARM WHEN IT CAME INTO CONTACT WITH A PIECE OF METAL. HE WAS SENT TO COMPANY PHYSICAL WHERE HE RECEIVED 6 STITCHES , THEN RTW WITHOUT RESTRICTIONS.
