Employee was washing out the sand screw when EE lost control of the hose. The hose struck EE in the left eye.

Fall to the walkway or working surface

The employee was walking through the scrap yard when EE tripped on a wire. EE extended arms to break the fall causing EE to land on their hands. EE reported it to the manager, but did not seek medical attention at that time. On 8/26/19, EE came to work stating was still sore and was taken to the Dr by management where EE was put on restricted duty making this incident reportable.

WHILE EE ATTEMPTED TO REMOVE PIECE OF METAL CHUTE THAT WAS BEING REPLACED, THE METAL PIECE TILTED TO THE RIGHT PINNING EE'S RIGHT THUMB AGAINST A METAL BRACE. EE'S THUMB REQUIRED STITCHES.

INJURED EE WAS TAKING CRANE TO SHOP. AS HE WAS GOING DOWN RAMP, THE CRANE CAME OUT OF GEAR. THE EE JUMPED FROM CRANE INJURING LEFT ANKLE AND RIGHT KNEE.
