EE clocked in and was walking to EE's vehicle from the scale house. EE cut the wedge between the sidewalk and parking lot through the grass to EE's car. EE slipped on the wet grass injuring ankle.

Employee was performing maintenance on the secondary crusher and received a laceration to the left thumb. Employee received 2 stitches as a result of the laceration.

Fall down stairs

Ee was walking down the stairs from a conveyor when EE stepped on the edge of the second to last step, lost balance, and fell back on left wrist. EE originally did not want to go to the dr, but continued to have pain and visited dr on 4/18. EE was diagnosed with a fractured left wrist.

In process of replacing blown hydraulic hose, he fell off tower deck from approximately 4 feet striking a wrench laying on ground and injured his left arm.

EMPLOYEE WAS CARRYING A COME-ALONG UP THE CATWALK TO CHANGE OUT THE DISCHARGE LIP ON THE 5 ?' CRUSHER AND ROLLED HIS FOOT ON SOME LOOSE MATERIAL.

Employee was attempting to cut the band off a pallet of screens with tin snips. The band was very tight. When the cut went through, the screens released with force and struck him in the left arm requiring six stitches.

GETTING OFF A PIECE OF MACHINERY - EE JUMPED, LOST HIS BALANCE AND A ROCK CAUSED A CUT ON HIS HIP.

EE WAS REMOVING ROCK WHICH HAD SPILLED ONTO THE WALKWAY FROM THE SCREEN AREA. TO PERFORM THIS TASK, HE WAS IN A ""THROWING"" MOTION. HE DID NOT NOTICE WHERE HIS HANDS WER IN RELATION TO THE HAND RAIL. HE WAS WEARING GLOVES AT THE TIME OF THE INJURY.

EE WAS CONDUCTING A PRE-SHIFT INSPECTION ON THE WATER TRUCK. HE LIFTED THE HOOD ON THE TRUCK ANDADDED HYDRAULIC OIL TO THE TRUCK. HE HAD TO LEANOVER THE AIR FILTER TO DO THIS TASK. HE SAID HIS BACK BEGAN TO HURT.

EMPLOYEE WAS PUNCHING A HOLE IN A CONVEYER BELT WITH A BELT KNIFE. HE WAS PULLING THE KNIFE TOWARD HIMSELF. THE EMPLOYEE WAS KNEALING AT THE TIME, THE KNIFE CUT HIS LEFT LEG A FEW INCHES ABOVE THE KNEE. HE SHOULD HAVE BEEN USING A BELT PUNCH FOR THIS TASK AND CUTTING AWAY FROM HIMSELF.
