Caught in, under or between collapsing material or buildings

Wash plant operator was operating a front-end loader during the overnight shift as normal. EE exited the loader and entered the hopper filled with sand the EE had just dumped. EE was fatally injured when they were engulfed in the sand, as it vibrated to the bottom of the hopper. EE may have entered the hopper to try to retrieve two pieces of metal inadvertently dumped into hopper.

Fall to the walkway or working surface

The miners were trying to switch out a 10"" x 50' poly pipe that had ruptured with a Caterpillar excavator. EE had put the chain choker on the pipe so it could be removed from the ditch and was trying to exit the ditch at the steepest end and in doing so lost footing and fell down twisting left knee.

Over-exertion (Not Elsewhere Classified)

The operator went out to the manual water cannon to reposition it and slipped. EE was trying to stop the fall and grabbed a beam to hold self up and in doing so twisted around fracturing the humerus near the shoulder. We have installed a no slip surface in this area and ordered a remote controlled nozzle.

Caught in, under or between a moving and a stationary object

Miner was changing the bag on the rotary airlock at the base of the dust collector. EE was trying to clear a bridge and removed the guard and put EE's hand in the airlock to clear the bridge without ensuring that EE locked out the disconnect. While doing so, the airlock actuated and EE's hand became caught between the blade and the side wall.

Struck against stationary object

The miner was walking beside a pick-up truck and slipped in mud and when EE grabbed the side of the bed on the truck EE's hand was cut.

Miner was attempting to remove a bolt from an actuator when the bolt broke. His momentum from the force applied made him step backwards to balance himself. When he stepped back his knee twisted causing the injury.

A 10"" water pipe chained to an excavator bucket whipped while workers were trying to clear a clog. The miner was walking back to the head of the pipe and was struck in the leg. The miner suffered a proximal tibular fracture where the tibia connects to the fibula.

Miner was feeding wire over a beam to wire in new lights at the dry plant. He felt no effects until the next day. He has pain in in his shoulder when he rotates his arm and has occasional numbness in his fingers.

Struck against a moving object

Haulage road was not properly constructed and the seat on the pan was not properly set to the right amount of pressure resulting in the miner receiving a back strain from the strong bouncing of the equipment.

Miner was going down onto the beach and slipped on the steps and twisted his ankle.

Miner was trying to get onto the railcar to apply the brake and slammed his knee into the car.

Contact with hot objects or substances

Miner allowed a sand line to become clogged. In the course of unblocking it the water close to the pump became heated and when the blockage broke loose the hot water shot out on him.

Miner was grinding on steel and a tiny piece of the metal bounced behind his safety glasses and hit his eye.

Miner stood up from working on loader and hit head on the boom.

Fall from scaffolds, walkways, platforms

Platform on the wheel loader gave way while he was standing on it. The loader was being repaired and a supporting piece had been removed. He tried to balance himself on the platform and fell hard on his feet when the platform slipped. He suffered a jarring injury to his left knee. The mechanic has knee problem history prior to coming to the company.

Employee was standing beside Pump face trying to pry the seal while the backhoe lifted with a chain. The lifting ring broke causing the chain and ring to hit the miner in the right wrist and forearm causing a 1 inch laceration approx 1/4"" deep and blunt force trauma resulting in contusions and a slight abruption to the forearm.

Miner struck finger on 2 x 8 board.

Contact with hot objects or substances

Miner was using torch and pushed a bucket back with his leg. When he started the torch, his pants leg ignited from gasoline. A miner left an unmarked, unapproved container of gasoline in the area and the miner doing the cutting failed to do a work area inspection before he began his work.

Miner hurt knee last year and did not seek medical attention. He is now seeking help because the knee is giving him trouble.

Contact with hot objects or substances

Miner was futting a piece of pipe with a torch and the pipe broke loose burining his arm. Over the weekend the burn wound became infected resulting in fever and general malaise.

Fall from machine

EE was exiting the loader and slipped coming down the ladder. He fell forward and used his hands to push off the loader and avoid hitting his head, but landed hard on the ground in the process.

Employee was pulling up roots in the pit and the chaulk ledge he was standing on broke away and he twisted his knee.

Employee was cutting a conveyor belt with a utility knife and it slipped, cutting his thigh.

EE WAS RUNNING BULLDOZER ON AN OPEN PIT MINE ROAD, MOVING FROM ONE SIDE OF THE JOB SITE TO ANOTHER ON THE ROAD. THE DOZER BLADE WAS TOO LOW. THEDOZER DROVE INTO A LITTLE MUD HOLE. HIS BLADE CA UGHT ON THE ROAD.
