Struck against a moving object

While dumping on stockpile - dump and truck tipped on left side

Caught in, under or between a moving and a stationary object

Employee was lifting a piece of drill rod off of a rack and smashed pinky finger between rod and breakout table resulting in 5 stitches. Employee was wearing gloves.

Over-exertion in lifting objects

Employee was hooking up line to trash pump and felt tug/tightness in lower back. Employee did not report injury until 5th of July and did not seek medical attention until 12th of July.

Miner was injured while attaching metal pull cable to haul truck stuck on the dump site. A frayed wire on the cable struck and lacerated EE's right leg. This resulted in 7 staples to close the wound.

Struck against stationary object

The operator was exiting the cab of an excavator, when EE's left arm came in contact with a broken rear view mirror. The task being performed was clearing land when the mirror was broken. A piece of mirror glass cut EE's arm about 1.5 inches long around the elbow area. This cut required 7 stitches.

Employee was on a callout working with contractors to repair process water line feeding the mill. Employee dismounted a front end loader and felt his knee pop when he stepped down.

Over-exertion in pulling or pushing objects

While attempting to climb on machine employee stepped on bottom step and reached up and grabbed hand rail while pulling up felt a pop in left shoulder experiencing pain in L. arm. Diagnosis no use left arm/must wear sling, muscle in left shoulder damaged pending Arthrogram.

Struck against stationary object

Mechanic was cleaning out a six-inch drain pipe in a concrete pit. The gate valve in the pit is designed to leak minimally as it opens and closes. The mechanic was cleaning the pipe with a water hose by running it up the line. As he was pulling the hose back out of the drain pipe, his elbow ran across the edge of a piece of expanded metal being used to guard the nearby gate valve.

Employee (Mechanic) got his middle finger (right hand) caught between the cable and load block pulley on a mobile crane. Emp. was attempting to push cable back into pulley groove when injury occurred. Emp. was not wearing gloves.

A tractor operator got stuck in the mud & called for assistance from another operator/the injured operator. The injured operator was attempting to connect the cable to his tractor when he states having sustained an injury to the back of his right leg. EE reported accident on 4/4/05 & diagnosed with a large herniated disc by a specialist on 4/13/05 & was placed on disability.

MECHANIC FELL INTO A 15"" DEEP DRAIN IN THE PUMP HOSE FLOOR WHEN A PIECE OF GRATING COLLAPSED. HE SUSTAINED BRUISES TO HIS RIGHT HIP AREA. DATE OF FIRST MEDICAL TREATMENT 3-24-04.

Flash burns (welding)

Employee was welding in blunger. After his shift, both eyes reddened, became teary and painfull and vision blurred. Employee saw eye doctor and was diagnosed with kerato conjunctivitis secondary to welding arc. He was given prescription eye drops and anti-inflammatory.

HEAVY EQUIPMENT OPERATOR COMPLAINED OF FEELING SICK ALL NIGHT AS HE DESCENDED HIS PIECE OF EQUIPMENT, HE BECAME DIZZY AND FEEL AGAINST THE SIDE OF THE EQUIPMENT BEFORE COMPLETELY DESCENDING IN JURING HIS L SHOULDER. HE WAS TREATED IN ER FOR PERSONAL ILLNESS AND DIAGNOSIS OF L SHOULDER STRAIN. PANEL PHYSICIAN PUT HIM ON RESTRICTED DUTY.

EE WAS MOUNTING THE REAR OF A MOTORGRADER TO CHECK THE ANTIFREEZE LEVEL HIS LEFT FOOT SLIPPED OFF THE REAR STEP CAUSING HIS LOWER LEFT LEG TO COME IN CONTACT WITH A VALVE THIS FORCED HIS LEG TO BEND BACK FURTHER THAN IT NORMALLY COULD CAUSING PAIN & SWELLING IN HIS LEFT KNEE BY 5/4/00 AT WHICH TIME 1ST AID WAS GIVEN. MED EVAUATION 5/19/00 MRI 5/31/00 DIAGNOSED TORN LIGAMENT
