Operator was doing pre-shift on a D8N Dozer while closing engine inspection door they slipped and fell, they broke their wrist as a result of the fall.

Employee was called out to install a hose for 10 silo. The clamp failed and EE had to come back out and install it again. During the second installation the hose was found in the car, full of clay. Another Employee helped EE pull the hose out of the car and this was when the employee felt a pain in the lower stomach.

EE was crossing the grass going to the crude shed and struck right knee on a jersey barrier.

Mechanic was welding in dust collector, due to reflection of the welding arc, mechanic received flash burns to the eyes.

The employee's pants leg caught a piece if grating causing them to fall. The employee denied medical attention until 2/5/2018 at which time they were placed on restricted duty.

DISCHARGE SHOOT WAS CLOGGED AND EMPLOYEE PICKED UP THE HOSE ATTACHED TO SHOOT. HOSE BROKE AWAY FALLING ON EMPLOYEE. HIT EMPLOYEE AND CAUSED FRACTOR TO FOOT.

Driver was trying to hurry to get back to truck, slipped on wet clay and struck knee on the ground.

The employee stumbled at the Rough Room Door. Extended his right arm which was hit by the door as it was closing. Causing pressure into right shoulder possibly causing injury.

Employee was washing out a rail tanker car when he slipped and cut his foot right below the ankle. Employee was wearing brand new metatarsal boots and all his other required PPE. He got up and finished cleaning the car, but when he exited the top he realized his foot was injured.

Changing hammers in hammer mill when left hand index finger was hit with pry bar

Employee was exiting dozer had a lunch bag in hand. He started to step down from dozer and his foot slipped on the tracks. He fell to the ground.

Cleaning inside railcars with a pressure washer and the pressure washer contacted the side of right foot causing a hole in boot and a laceration to right foot.

Employee was mounting front end loader when he lost grip with his right hand and fell to the ground

Was walking out of rotary dryer room when the operator hollered at employee to come back and restart the dryer. When he turned to go back into the rotary dryer building, he twisted his left knee. Surgery done on 3-7-12.

Hammer mill rotor was locked down with clay. Used vacuum truck to remove clay and then tried to free up rotor by rotating it. Employee pushed on rotor and when it broke free it caught his left hand middle finger between wall and a hammer causing a laceration.

WHILE HYDROBLASTING INTERIOR OF TANK RAILCAR DIRECT WATER PRESSURE FROM WAND EXTENSIVELY LACERATED LEFT FOOT; DUE TO EMPLOYEE CARELESSNESS IN OPERATION OF WAND & FAILURE TO RE-POSITION AS NEEDED FOOT SHIELD OF LEG/FOOT PPE TO ENSURE PROPER COVERAGE WHERE MOVEMENT WITHIN TANK CAR CAUSED SHIELD ""FLIP UP"" TO MORE VERTICAL POSITION & LEFT TOP OF FOOT/RUBBER BOOT IMPROPERLY EXPOSED.

Employee was going down 5 steps behind the bagger, tripped and fell to the floor, broke his left leg.

MINER ON ROOF WEARING SAFETY HARNESS BUT NOT TIED OFF TO EXISTING CABLE WHERE EXISTING RETRACTABLE WAS AVAILABLE. MINER WAS STEPPING BACKWARD WHILE PULLING HOSE IN PREPARATION FOR COATING ROOF WHEN HE STEPPED ONTO & THROUGH SKYLIGHT SHEETING. MINER FELL APPROX 17 FT ONTO CONCRETE FLOOR BELOW & SUFFERED BROKEN FEMUR.

Employee struck his head on a light fixture in a storage area of the office.

Emplyee was inside a tank car, cleaning the car. Local railroad came on site, accidently releasing rail cars above the car the employee was inside. The released cars struck the car being cleaned, knocking the employee down, bruising his lower back. No medical treatment was deemed necessary by attending physicians.

Employee was climbing off a backhoe, missed a step, fell to the ground.

Employee was washing out a rail tank car, he slipped, the pressure washer cut across his boot, cutting the top of his foot.

Employee was standing in front of the slicer on floor level. A large lump of clay fell from the top of the slicer, striking the employee on the left shoulder bruising it. The blow knocked the employee to the floor, cutting his left thumb on a tool he was holding.

EE WAS ATTEMPTING TO BURN PALLETS. WHEN THE PALLETS IGNITED, THE FIRE FLASHED BACK ON HIM.

EMPLOYEE WAS WORKING ON REJECT LINE ON TOP OF REJECT SILO, STRUCK RIGHT HAND ON REJECT LINE COUPLING.

EE WAS INSIDE OF BLUNGER, BUMPED INTO BLUNGER BLADE WITH HIS RT. SHOULDER CAUSING LACERATION.

EE SLIPPED ON WET CLAY & STRAINED HIS LEFT KNEE

EMPLOYEE FELL OUT OF CHAIR & STRUCK HAND ON TOOL BOX.

ANOTHER EE STRUCK EE IN THE FACE WITH HIS FIST.

EE WAS WORKING AT THE HARRIS MINE, USED THE BATHROOM PORTA JOHN AND A SPIDER BIT HIM.

EE WAS SAMPLING A BULK TRUCK WITH A SAMPLE STICK.HE CLOSED THE SAMPLE STICK AND CAUGHT HIS HAND IN THE OPENING OF THE STICK. (GRAIN THIEF)

EMPLOYEE WAS REMOVING SHAFT FROM MILL WITH A 20 LB HAMMER. THE HAMMER SLIPPED AND HIS FINGERS STRUCK THE FAN BLADE.
