Injured employee and others were removing belt clips using a chisel. As he struck the chisel, a small piece broke off and struck his arm, causing a laceration.

Fall to the walkway or working surface

Injury was walking from his car to the timeclock inside the electric warehouse when he tripped and fell to the ground. He reported that at that time his right knee began hurting.

Employee was helping change screen cloth. In the process of removing the screen, he cut his finger on the sharp edge of the screen cloth.

Injured employee was attempting to place a wheel chock under a rail car wheel. As he did this, the wheel rolled against the chock, forcing the handle down against the ground. His finger was pinched between the chock handle and the ground.

EMPLOYEE WAS USING HAND WRENCH TO REMOVE BOLTS FROM RIDE STRUT ON HAUL TRUCK. COULD NOT BREAK LOOSE & HAD ANOTHER EMPLOYEE USE HYDRAULIC CRANE BOOM TO PUSH ON WRENCH, THE WRENCH CAME OFF THE BOLT HEAD & STRUCK EE IN BOTH HANDS & CUT OFF PART OF (1) FINGER & CUT/SMASHED (2)OTHER FINGERS.

EE WAS HELPING REPLACE CONCAVES IN 42"" GYRATORY CRUSHER, AND WAS PUTTING 30 POUND BLOCK OF ZINC INTO MELTING POT WHEN HIS GLOVE CAUGHT ON THE SIDE OF THE POT. HIS HAND WAS CAUGHT BETWEEN THE POT AND THE ZINC, CAUSING A LACERATION TO HIS FINGER.

EE WAS APPROACHING A PARKED CUSTOMER TRUCK TO WAKE THE DRIVER, WHEN HE SLIPPED AND FELL, HIS LEGS WENT UNDER THE RUNNING BOARDS OF THE TRUCK. HIS SHINS STRUCK THE TRUCK, CAUSING THE INJURY.

EMPLOYEE STATED HE WAS WORKING ON CONVEYER #16 REPLACING ROLLERS, WHEN HE STEPPED FROM CONVEYER TO WALKWAY, HE TWISTED HIS LEFT KNEE AND FELT PAIN, BUT KEPT WORKING. PAIN CONTINUED, AND HE RE PORTED IT THE NEXT DAY.

INJURED WAS HELPING ANOTHER EE REMOVE A SECTION OF DRILL STEEL FROM THE SERVICE TRUCK. THE STEEL ROLLED UNEXPECTEDLY, & PINCHED INJURED'S FINGER AGAINST THE TRUCK BED, CAUSING THE INJURY.

EE WAS STANDING ON A ROCK HELPING LOAD EXPLOSIVES IN SHOT. HE TURNED TO STEP OFF THE ROCK AND SLIPPED AND FELL. HIS LEFT HAND STRUCK A SHARP ROCK, CUTTING HIS HAND.

EE WAS INSTALLING CONDUIT BRACKET FOR WIRING ON A NEW SHUTTLE CONVEYOR. HE WAS HOLDING THE BRACKET WITH HIS HAND WHILE DRILLING A HOLE. THE DRILL HUNG UP IN THE HOLE, CAUSING THE BRACKET TO TW IST, CUTTING HIS FINGER.

CONTRACTOR WORKING ON RAILROAD TRACK. EE WAS REMOVING BOLTS FROM FROG WITH A PUNCH AND CO-WORKERWAS HELPING BY KNOCKING OLD BOLTS OUT WITH A HAMMER. A PIECE OF METAL FROM HAMMER STRUCK EE ON LEFT ARM JUST ABOVE ELBOW INSIDE ARM. EE WAS TAKEN TO HOSPITAL AND GIVEN TREATMENT. DR. AT E.R. CUT SMALL INCISION TO REMOVE BUT FAILED & REQUIRED 5 STITCHES FROM THIS.

EE WAS ASSISTING OTHERS TRYING TO PUT A TRACK BACK ON A SMALL EXCAVATOR. A CHAIN HAD BEEN ATTACHED TO THE TRACK TO APPLY TENSION AS THE TRACK TURNED, IN AN ATTEMPT TO GET IT OVER THE SPROCKET. THE CHAIN CAME LOOSE AND WHIPPED BACK, STRIKING THE EMPLOYEE IN THE LEG.

EMPLOYEE WAS USING A CHIPPING HAMMER TO REMOVE J BOLTS FROM SCREEN CLOTH. HE STRUCK THE CHIPPINGHAMMER WITH ANOTHER HAMMER, CAUSING A METAL CHIP FROM THE CHIPPING HAMMER TO FLY OFF AND ENTER HIS HAND.

Struck by flying object

EE WAS HELPING OTHERS REPLACE BELTING ON CONVEYOR. WHILE WORKING, THE WIND CAUGHT THE BELTING AND LIFTED IT UP. THE BELTING STRUCK THE INJUURED IN THE FOREHEAD, CAUSING LACERATION. A BELT SPLI CE WAS LOCATED WHERE THE BELT STRUCK THE EE, AND A CLIP CAUSED THE LACERATION.
