EE was unrolling torch leads, and was walking backwards. EE slipped on a loose rock and fell. As EE was falling, EE put right hand out to catch self and in doing so dislocated shoulder.

Employee was unhooking a rock knocker when it came loose and knocked the ladder out from under employee causing employee to fall 34 inches to the ground landing on employee's side.

EE was climbing up a ladder and slipped causing them to hit their left elbow.

EE performed work in chute, climbed to top of screen deck, slipped and fell (slid) head first 25' through the screen and down the chute.

Employee was operating a crane when it tipped to its side. The operator jumped out of the crane landing face first striking their forehead causing laceration needing stitches.

Employee was removing a brass plate when it slipped and contacted EE's left middle and ring finger.

Employee was attempting to free a rock at the back plate of a conveyor and the belt pulled the employee's hand into the back plate of the conveyor.

Employee was getting a roller out of the frame and lost control of the frame causing the roller to come out smashing EE's pinky finger.

Employee lacerated right pointer finger on an Impact Pad.

Injured worker was in process of jacking the shuttle conveyor back in place when the conveyor structure unexpectedly moved catching IW's right hand in between the conveyor structure and a stop block.

While tramming the drill, the door slammed on the driller's finger causing a fracture.

Employee was washing out the primary crusher from underneath in the feed box. As he was standing in the box 30' away a rock fell and hit the crusher saddle and ricochet striking the employee in the right hand causing a laceration to his right thumb resulting in three stitches.

While throwing a rock from the crusher the employee struck his finger on a piece of metal.

Employee suffered fatal heart attack.

Employee was cleaning the shop because the plant was down for electrical problems. As he was cleaning he attempted to lift a wooden cribbing block when he strained his shoulder.

Employee was installing a 10"" pipe to FRP pump when the strap holding the pipe came loose causing the pipe to fall pinching employees finger between the pipe and pump flange.

EE was attempting to remove screen cloth from the screen tower. As he attempted to throw the screen cloth off the tower it caught the back of his head, causing a laceration.

Employee was stepping onto the fuel truck which was parked at fueling station. He grabbed the piping used as a hand rail when it broke loose causing him to lose his balance and fall causing a laceration to his left foot and spraining his left ankle.

While cleaning out gate of #9 screen and reversing the gate flow, gate swung forcing bar forward pinching the employee's finger between the bar and I-Beam injuring the tip of right little finger.

WHILE UNLOADING A RETURN ROLLER GUARD FROM WELDING TRUCK, GUARD BECAME ENTANGLED IN TORCH HOSES CAUSING IT TO BE JERKED FROM EE'S HANDS. EE LOST BALANCE AND FELL TO THE GROUND. GUARD FELL ON T OP OF EE CUTTING HIS HEAD.

WHILE DRIVING MASTER PIN INTO TRACK ASSEMBLY, A PIECE OF MASTER PIN CHIPPED OFF AND HIT EMPLOYEE IN CHEST, EMPLOYEE WAS TAKEN TO A CLINIC TO HAVE PIECE REMOVED. EMPLOYEE THEN RETURNED TO WORK.

EE WAS PLACING LEFT FRONT TIRE AND RIM ASSEMBLY ONTO HUB OF GMC WELDING TRUCK. WHEN TIRE WAS SET ON HUB HIS RIGHT MIDDLE FINGER WAS PINCHED BETWEEN THE RIM AND HUB.

EE STEP OFF BACK OF GREASE TRUCK PULLED MUSCLE BEHIND LEFT KNEE.

EMPLOYEE WAS USING A COM-A-LONG TO RESET THE TRUCKS OF A RAIL CAR WHEN THE TRUCKS MOVED RELEASING THE TENSION ON THE COM-A-LONG CAUSING THE COM-A-LONG TO SLIP AND SNAP DOWN ON EMPLOYEES FOREAR M.

EMPLOYEE WAS ATTEMPTING TO OPEN COMPARTMENT DOOR TO CHECK OIL WHEN HE FELT A ""POP"" IN LOWER BACK RESULTING IN A MUSCLE STRAIN.

EE WAS CLEANING OFF TOWER #4 & GOT LIGHT-HEADED.HE WAS DIAGNOSED WITH HEAT EXHAUSTION.

EE WAS SECURING CAP AND CABLE TO DRILL STEEL INSIDE DERRICK OF T4 DRILL. APPARENTLY THE DRILL STEEL MOVED AND FELL ON OR AGAINST HIS LEFT INDEX FINGER, AMPUTATING THE TIP.

EE FELL FROM LADDER WHEN HIS FOOT SLIPPED FROM THE SECOND TO THE BOTTOM RUNG OF THE LADDER. HE WAS DISMOUNTING THE 980 CAT FRONT END LOADER AT THE END OF HIS SHIFT.

EMPLOYEE WAS REPLACING 1 1/4"" SCREEN CLOTH. WHILE LIFTING EMPLOYEE OVEREXTENDED POSITION PULLING LOWER BACK.

DRILLER WENT TO REMOVE A NEW DRILL BIT FROM THE STORAGE AREA ON THE DRILL, THE BIT STARTED TO SLIP OUT OF HIS HANDS AND WHEN HE CAUGHT THE BIT HE HUNG HIS LITTLE FINGER, LEFT HAND ON THE SIDE OF THE DRILL. THE WEIGHT AND FORCE OF THE FALLING BIT CAUSED HIS HANDS TO DROP SOME RESULTING IN HIS FINGER GETTING BENT BACKWARDS.

EE WAS UNSTOPPING THE 8-10 MATERIAL BIN. HE TURNED AND STARTED WALKING BACK TO THE FRONT OF THE TRUCK AND STRUCK HIS FOREHEAD ON AN I-BEAM.
