EE was unrolling torch leads, and was walking backwards. EE slipped on a loose rock and fell. As EE was falling, EE put right hand out to catch self and in doing so dislocated shoulder.

Employee was unhooking a rock knocker when it came loose and knocked the ladder out from under employee causing employee to fall 34 inches to the ground landing on employee's side.
EE was climbing up a ladder and slipped causing them to hit their left elbow.

Employee was installing a 10"" pipe to FRP pump when the strap holding the pipe came loose causing the pipe to fall pinching employees finger between the pipe and pump flange.

EE WAS SECURING CAP AND CABLE TO DRILL STEEL INSIDE DERRICK OF T4 DRILL. APPARENTLY THE DRILL STEEL MOVED AND FELL ON OR AGAINST HIS LEFT INDEX FINGER, AMPUTATING THE TIP.

DRILLER WENT TO REMOVE A NEW DRILL BIT FROM THE STORAGE AREA ON THE DRILL, THE BIT STARTED TO SLIP OUT OF HIS HANDS AND WHEN HE CAUGHT THE BIT HE HUNG HIS LITTLE FINGER, LEFT HAND ON THE SIDE OF THE DRILL. THE WEIGHT AND FORCE OF THE FALLING BIT CAUSED HIS HANDS TO DROP SOME RESULTING IN HIS FINGER GETTING BENT BACKWARDS.
EE WAS UNSTOPPING THE 8-10 MATERIAL BIN. HE TURNED AND STARTED WALKING BACK TO THE FRONT OF THE TRUCK AND STRUCK HIS FOREHEAD ON AN I-BEAM.
