Employee was using an open blade box cutter to cut the top of a cardboard bore hole saver tube when the knife slipped off the tube cutting EE's right thigh. This resulted in a laceration of approximately one inch long and one quarter inch deep requiring four stitches.

FINALLY IT WAS PUT OUT WITH DIRT AND MUD. BEFORE DOING SO, EE BREATHED SMOKE AND TOXIC FUMES IN HER LUNGS. DUE TO SMOKE INHALATION, THROAT AND LUNGS WERE BURNT. SHE WAS HOSPITALIZED.
