Operator was getting down from machine and lost footing on the next to top step and fell to the ground on their side and it tore a muscle in their leg and cracked a rib.

Employee was changing a head pulley bearing. After removing the turnbuckle EE was caught between the motor and frame.
Employee was repairing the weatherhead on CR4 motor and while removing the bolts with a drill the adapter caught in the bolt and twisted the drill quickly forcing the employee's hand into a piece of expanded metal motor shaft guarding causing a laceration on employee's hand.

Employee alleged strained shoulder while descending a ladder.
On January 26, 2021 employee was moving steel from an overhead rack on the drill when EE felt shoulder pain. EE sought medical attention on March 1, 2021. An MRI was performed on March 10, 2021 and EE was diagnosed with a rotator cuff tear. EE will work full duty until EE's surgery date, March 19, 2021, which will then become a lost time incident.
Employee was cleaning a chute when EE was struck by a bar causing a laceration.

The employee sustained a finger injury while replacing feeder bottoms. The chain hoist wasn't secured when positioning the plate and came loose striking the employee's finger. The employee was taken to the hospital for medical treatment resulting in a broken finger/laceration.

On 8/3, EE was installing a guard for a grid coupling. As the guard was lifted, the lid fell back & the corner of the lid struck thumb causing a laceration. EE did not seek medical treatment at that time, but later advised the thumb was swollen, sore & EE was unable to bend it. EE was taken to medical facility on 8/6 where EE was treated for infection.
While the employee was changing the skirt on the transfer point between C-1 & C-2 conveyor, left hand made contact with the metal causing employee to receive a laceration to left index finger. The employee was wearing gloves at the time of the incident.

At some time around 11:45 am - smoke was seen on our berm surrounding the property. The fire department was notified, the local MSHA field office was called as well as the 1-800-immediatly reporting line shortly when we realized the fire would not be extinguished within 30 minutes.

Employee was performing clean-up activities at #3 belt-lower surge tunnel. Abandoned water line-valve he was trying to load onto boom truck. The pipe swung around & smashed his little finger on right hand.
THE INJURED EMPLOYEE WAS STANDING AT THE RIGHT REAR OF A BOOM TRUCK WHEN THE OUTRIGGER WAS LOWERED DOWN ON HIS LEFT FOOT.
EE WAS WELDING BED LINER IN BED OF HAUL TRUCK. CLIP BROKE, CAUSING STEEL PLATE TO FALL ON EE'S FOOT.

The turbo failed on the pit loader resulting in a fire. The ee turned off the ignition, exited the cab and dropped from the midrail on the loader to the ground, approx 5 ft. a fire suppression system was on the loader and a fire extinguisher was located at the door also.

THE BELT CABLE GOT HUNG ON THE BELT SCRAPER SO THE INJ EE WENT UNDER THE DRIVE BELT GUARD & BEGAN PULLING ON THE VBELTS-ONCE HE PULLED ON THE BELTS THE CABLE THRU THE BELT SCRAPER & THE SHEAVE MOVED. THE EE'S HANDS WERE PULLED THRU THE SHEAVE.
EE WAS TRYING TO CONTROL THE FALL OF A WOODEN BLOCK & IT MASHED HIS RIGHT RING FINGERTIP. HE WAS BLOCKING THE FRAME OF THE LOADER WITH 8"" X 8"" X24"" BLOCKS. FINGER WAS CUT & NEEDED 6 SUTURES.

EMPLOYEE WAS REMOVING BOLTS FROM ECCENTRIC DRIVE ON CRUSHER. THE EMPLOYEE WAS USING A 1"" AIR WRENCH, HE WAS HOLDING THE SOCKET W/LEFT HAND; IT SLIPPED OFF RESULTING IN A CUT TO LEFT HAND BETWE EN THUMB AND INDEX FINGER.(3 STICHES TO HAND).
EMPLOYEE WAS OPERATING DRILL, HYDRAULIC HOSE TO COMPRESSOR CAME LOOSE FROM FITTING SPRAYING HYDRAULIC FLUID ON DRILL. THE HOSE CONTACTED THE COOLER FAN, GENERATING SPARKS WHICH CAUSED THE HYD FLUID TO BURN. FLAMES CAME INSIDE CAB AND BURNED DRILLER'S LEFT FOREARM AS HE WAS EXITING DRILL CAB.
EE WAS DESCENDING STAIRWAY OF CRUSHER RUN CONTROL TOWER; STATED HIS LEFT HAND SPASMED AND CAUSEDHIM TO FALL OVER THE HANDRAIL.
