Struck against stationary object

Operators machine was down. Mechanic was working on it. Operator rested right hand on stick while waiting. EE slid hand down and caught hand on a metal burr, cutting EE's palm.

Caught in, under or between a moving and a stationary object

Repairing crusher and got hand caught between spider and mainframe. This cause miner to have a crushing action on their right hand.

Struck by falling object

Loading a fuel tank on a trailer. Ladder fell down injuring right index finger.

Struck by flying object

Surface drill operator assisting mechanic removing internals of a 4-inch down hole hammer (off the drill). Inertia movement of internal piston caused it to knock the internal sleeve out of casing, striking driller in forehead. Driller received 3 sutures and returned to work immediately.

At approximately 7:15 am Monday Morning, employee came and sat down in the plant manager's office. Employee stated he did not feel good. He stated he was having some chest pains and sweating real bad. Plant manager asked him if he needed to go to the hospital. Employee stated ""yes"". The Plant Manager drove him to the hospital.

Caught in, under or between a moving and a stationary object

Employee was helping to remove a bolt from the 4265 gyratory crusher when his right middle finger got caught between the large bolt and the crusher frame. The employee was taken for medical treatment and received stitches to close up the cut. He was released to full duty.

Employee was removing the chute from the HP 500 crusher. He and a co-worker were cranking the chain hoist when the eye it was attached to broke loose. When the lifting eye came off, the chain hook hit the employee in the face causing the cut above his lip.

EE WAS LOOSENING A BOLT THAT HOLDS THE CUTTING EDGE ON THE UNILOADER. HE WAS USING AN ELECTRICAL WRENCH ON THE NUT ON TOP. THE BOLT IS COUNTERSUNK SO YOU DON'T NEED A WRENCH TO HOLD THE BOLT WHEN YOU ARE LOOSENING THE NUT. HOWEVER, EE WHENEE LOOSENED THE NUT THE BOLT CAME OFF OF THE COUNTERSUNK HOLE. EE HAD GLOVES ON SO HE PUSHED THE BOLT AND THE NUT TURNED AND CUT HIS FINGER.

MY FOOT SLIPPED INTO A HOLE AND MY RIGHT FOOT REMAINED OUTSIDE OF THE HOLE, BENDING MY BACK INTOAN AWKWARD POSITION. MY BACK POPPED AND STARTED TO HURT.

Struck by flying object

EMPLOYEES WERE CHANGING SHANK PROTECTORS ON PIT LOADER BUCKET. THE LEFT SIDE WAS STUCK SO THEY HAD TO HIT IT WITH A 8 LB SLEDGE HAMMER. A PIECE OF METAL BROKE OFF THE SHANK PROTECTOR AND STRUC K EE IN THE LEFT LEG CAUSING A PUNCTURE WOUND IN THE FRONT OF HIS THIGH AND THE METAL LODGED UNDERNEATH THE SKIN ON THE BACK OF HIS LEG.

LOADER OPERATOR WAS OPERATING A 980G CAT LOADER ON THE SHIPPING YARD. HE RAN LOADER INTO A SMALL HOLE (POTHOLE) AND IT SHOOK HIM AROUND IN HIS SEAT. THIS CAUSED HIM TO STRAIN HIS BACK.
