Drowning

Employee was operating a haul truck transporting material from the wash bins to a stockpile located Northwest of two settling ponds. On the return trip to the wash bins, the haul truck traveled through a berm on the left side of the haul road, went over an embankment and overturned on its left side in the settling pond.

Employee was trimming rubber for a back skirt on a conveyor belt. While cutting the rubber the razor knife slipped and cut the outer right thigh.

A shovel leaning against the front of an oil cooler fell and when he reached to pick it up his right middle finger struck the fan guard resulting in a laceration.

EE went to remove rock at primary feeder and noticed a rock was hung between the rollers. He used a bar to remove the rock, when the bar kicked out and threw his hand into frame of the roller, cutting his little finger on his left hand.
