Employee was operating a haul truck transporting material from the wash bins to a stockpile located Northwest of two settling ponds. On the return trip to the wash bins, the haul truck traveled through a berm on the left side of the haul road, went over an embankment and overturned on its left side in the settling pond.
Employee was trimming rubber for a back skirt on a conveyor belt. While cutting the rubber the razor knife slipped and cut the outer right thigh.
Employee was removing a conveyor gearbox when the pipe wrench EE was using slipped causing EE to strike EE's left knee and smash EE's right thumb.
EE was lifting a roller during maintenance on a conveyor when EE strained EE's left shoulder.
Employee passed away to unknown circumstances.
Employee lacerated finger while tighten bolts on motor base on screens.
Employee was lifting a tool box and felt a something in arm. Employee didn't require medical treatment. On February 29th the employee was recommended to seek further treatment. Restrictions were given at this doctor visit.
A shovel leaning against the front of an oil cooler fell and when he reached to pick it up his right middle finger struck the fan guard resulting in a laceration.
EE went to remove rock at primary feeder and noticed a rock was hung between the rollers. He used a bar to remove the rock, when the bar kicked out and threw his hand into frame of the roller, cutting his little finger on his left hand.
Injured EE was attempting to reposition chains to crusher saddle, when another EE bumped the saddle and it fell 3-4 inches, smashing his ring finger on left hand.
Feet got tangled in Plastic Loop, fell on knees
WHILE REMOVING AN 8"" STEEL PIPE, EE TRIPPED & FELL BACKWARDS. RESULTING IN THE 8"" X 10' PIPE LANDING ON EE CAUSING INJURY TO GROIN AREA.
