Employee felt pain in knee when they stepped off the ladder of the drill platform on October 24, 2025. Employee was diagnosed with a ligament tear on November 13, 2025. Employee is currently working full duty while waiting on an orthopedic appointment.

Fall from machine

Employee was exiting the bed of an F350 service truck when they missed a step, lost footing, and fractured their left ankle.

Employee was attempting to remove a steel plate from the metal rack when the plate slipped out of their hands landing on their right foot.

Employee alleges that while operating the water truck on 8/25/23, employee hit a bump causing a back pain. Employee did not report the incident until 09/05/23 and sought medical treatment on 09/06/23. While this incident is still under investigation, this 7000-1 is being reported within the 10 business days of receiving medical treatment.

Employee was setting the water jacket in a 7 foot crusher. The water jacket shifted and pinned the employee's leg between the water jacket and socket causing a fracture.

While torqueing wheel fasteners on a 785 truck the technician placed hand in the path of the reaction arm causing the hand to be crushed between the arm and the wheel.

The employee was injured while removing a impact feed plate on a screen. When the employee was removing the plate, the plate slipped above gloved hand and lacerated forearm. Employee was taken to the ER where employee received nine stitches.

Employee was using a hand grinder with a brush attachment to remove rubber material from a screen support tube being resurfaced. As material was being removed, the attachment caught on the rubber causing the grinder to kick back on EE. The results from this action caused a laceration to the upper left arm area requiring seven stitches to close the wound area.

A driver was driving haul truck from pit to primary crusher. Another employee was driving empty haul truck from primary crusher to pit. Due to inattentiveness of the driver, the empty truck slid into loaded truck.
