Due to personal medical illness, employee fell out of pickup truck parked at electrical room in the secondary plant. They suffered a laceration to the forehead and received stitches.

The employee had just dumped a load in the jaw crusher when he pulled the Komatsu HD605 in front of the pit breakroom to get a bottle of water. He had a plastic cup and trash in his hand and as he was stepping down the ladder from step 2 to 3, his hand slipped and he fell to the ground. He landed on his feet, but broke his ankle in three places, requiring surgery to repair.
Employee was performing maintenance when hot metal contacted the torch hose causing it to ignite. While trying to crimp off the hose the employees right hand was burned when it came in contact with the flame. The burn was treated and the employee will regain full use of his hand.

Employee was working in a chute on station #3 when a piece of plate he was holding in place to weld, fell, striking his left hand.
Injured employee was using a sledge hammer while repairing a crusher, and he felt pain in his shoulder. After several weeks, the pain lessened, but did not stop. A second medical exam and MRI revealed 'impingement' of the rotator cuff, and surgery was recommended. The surgery was completed on May 20, 2011, and he returned to work the next scheduled work day.

The injured employee and another were preparing a product sample. As they lifted the sample (100+ lbs) from the floor to the scale (8 inches), the injured said he felt a pain in his back. Initially diagnosed with a muscle strain, he returned to his normal work duties. He returned to the doctor on October 2, 2009, and was again returned to work, but with restrictions.
Employee's finger was pinched between a bin gate and a clevis pin as the employee and another were attempting to replace the connecting pin.

Driller mistakenly rotated the head while a 36"" pipe wrench was attached to the casing. The wrench swung around and hit the helper, knocking him to the floor, then hit the driller.

The employee was changing cutting edges on the dozer blade. As he was cleaning the area, his hand contacted the sharp edge of the blade, causing a cut on his palm.
Employee was driving a wedge to secure the door on the sand box. As he swung the hammer, his hand contacted the door frame, breaking his finger.
Employee was attempting to remove a gear from a housing, striking it with a hammer. As he hit the gear, a piece of metal broke off the gear and struck him in the leg.

OPERATOR WAS TRYING TO GET UP ON A DOZER, HIS RIGHT FOOT SLIPPED. HE TRIED TO CATCH HIS FALL CAUSING HIS RIGHT LEG TO HIT THE PUSH ARM, HIT SHIN BONE AND SKINNED IT WHICH LATER GOT INFECTION,CELLULITIS REQUIRING MEDICAL ATTENTION. INVESTIGATOR SAID DOZER PUSHARM NEED TO BE FIXED AND HAD BEEN REPORTED. IT WAS LEANING AND OP HAD TO STEP ON IT FIRST
Employee was attempting to clean screen deck. In the process, his hand struck a bolt on the deck and the sharp edge cut his finger.

EE WAS USING TORCH TO CU T BOLTS LOOSE. GAS LEAKED FROM A HOLE IN THE HOSE AND IGNITED. EE RECEIVED FIRST AND SECOND DEGREE BURNS ON THE RIGHT ARM.
EE WAS REMOVING SHACKLE CONNECTING ROCK CHUTE TO CRANE CABLE. CHUTE SHIFTED UNEXPECTEDLY, TIGHTENING CRANE CABLE. EE S FINGER WAS PINCHED BETWEEN CABLE AND SHACKLE.
