Employee was working to remove broken studs out of a steel ring using a 4 foot pipe wrench. While performing the work, the pipe wrench handle broke striking the employee in the face around the right eye.

While attempting to clean the windshield of a front end loader, the employee slipped and fell to the ground.
Employee reported having discomfort in their back after operating a skid steer. Refused medical treatment. Later sought treatment with personal physician, then requested medical attention. Incident investigated for questionable work-relatedness. Decision made internally to accept. On restricted duty.

Employee was adjusting the set screw that controls the RPM's on the grinder in an attempt to lower RPM's. While the grinder was being adjusted the disk broke apart striking employee on left hand between middle finger and ring finger.

At an area on the property but not at the location of the mining operations, an abandoned, burned car was discovered by the Plant Foreman and a Contractor. Upon further investigation of the vehicle, remains of a deceased victim were discovered in the trunk. Police investigators concluded the victim and automobile had been disposed of at the site.
EE was dismounting from his 773 haul truck and stepped backwards away from his ladder and stepped on a 4"" x 4"" block. EE then lost his balance and contacted the maintenance shop wall with his hand and cut his hand. EE immediately called management over the Motorola radio.

Employee was removing bolts from a wear plate segment on a loader bucket. One bolt dropped down out of its seat causing it to spin when the employee applied the impact wrench. The employee used his left hand to lift the bolt back into its seat and applied the impact wrench. The bolt then spun causing a laceration to the left middle finger that required stitches.

Employee was cleaning up under a transfer box when he slipped on some rocks causing him to fall forward into a support beam. This incident resulted in the employee suffering a cracked rib.
While attempting to place crusher main shaft, employee's gloved hand was snagged by the slag left from cutting off the pad eye, pulling him slowly in the direction the crusher main shaft was turning causing him to grab with his other hand to keep from losing his balance. This caused the jerk on his left arm and the resulting injury.

GIVEN THE EE'S BODY POSITION A ND THE REQUIRED RESISTANCE AND FORCE, THE EE SUFFERED A STRAIN TO HIS GROIN. EE IS PROJECTED TO RETURN TO FULL DUTY ON 12-23-03.
LOADER OPERATOR WAS GREASING HIS LOADER AT THE SHOP WHEN HE WAS STUNG BY A BEE OR WASP IN THE LEFT SIDE OF HIS ABDOMEN. EE DEVELOPED A SUSPECTED ALLERGIC REACTION CAUSING SWELLING AND DISCOMFO RT, WHICH REQUIRED EMERGENCY MEDICAL ATTENTION AND SUBSEQUENT LOST TIME.
EMPLOYEE WAS CUTTING BOLTS OFF OF THE CYLINDERS OF A CRUSHER MANTLE WITH AN OXY-ACETYLENE TORCH. HOT SLAG BURNED A HOLE THROUGH THE ACETYLENE GAS HOSE AND CAUSED A FIRE THAT BURNED THE EMPLOYEE'S RIGHT KNEE. HIS PANT LEG HAD SOME OIL ON IT WHICH CAUSED THE FIRE TO BURN THROUGH HIS CLOTHING.

WHILE DESENDING LADDER OF HAUL TRUCK, EMPLOYEE'S FOOT SLIPPED OFF OF RUNG, SPRAINING HIS KNEE. EMPLOYEE CONTINUED TO WORK REGULAR SHIFT UNTIL A FOLLOWUP APPOINTMENT WITH DOCTOR DISCOVERED TORN LIGAMENT, REQUIRING SURGERY - SCHEDULED FOR 1/28/03.
WHILE REPLACING A BROKEN WIRE CABLE, EE SLIPPED ON THE TRACKS OF THE DROPBALL, HITTING HIS NECK AND RIGHT SHOULDER ON A PAD. EE WAS WORKING ON RESTRICTED DUTIES UNTIL 12-30-02, WHEN HE WAS OR DERED BY NEUROSURGEON TO PERFORM NO MORE WORK UNTIL MRI EVALUATION IS COMPLETE.
HE WAS HELPING MOVE A GEAR BOX, WHEN THE SHAFT HE WAS HOLDING BROKE, SNATCHING & SPRAINING HIS RIGHT HAND. AFTER REST DID NOT HELP THE HAND TO HEAL, SURGERY WAS PERFORMED ON 4/18/02 TO REATTAC H A LOOSE LIGAMENT IN THE HAND.
EMPLOYEE WAS IMPROPERLY REMOVING A DRILL BIT ON A FACE HOLE. THE BIT WAS SITTIG ON THE GROUND AND HE REACHED UNDER THE TABLE TO LINE THE BIT UP WITH THE HAMMER AS IT CAME DOWN. HIS THUMB WAS T OP OF THE BIT WHEN THE HAMMER CAME DOWN, AMPUTATING HIS LEFT THUMB. GLOVES WERE ON.

HAND ON A RUNG ABOUT HEAD HEIGHT. WHEN HE TRANSFERRED HIS WEIGHT ON RT. FOOT ON 2ND RUNG HIS FOOT SLIPPED CAUSING HIM TO FALL ABOUT TWO FEET TO THE GROUND CAUSING A SPRAINED ANKLE & CRACKED
EE WAS LOADING TRUCKS WITH A 980F WHEEL LOADER. THIS LOADER HAS FINGERTIP CONTROL AS TO SPEED AND BUCKET MANEUVERABILITY. EE NOTICED A STINGING SENSATION IN HIS RIGHT SHOULDER. AFTER SEVERAL H OURS THE PAIN DID NOT SUBSIDE SO MEDICAL ATTENTION WAS SOUGHT.
EMPLOYEE WAS PLACING SCREENS IN #2 SCREEN. HE WAS LYING ON HIS BACK BETWEEN DECKS 1 & @. HE WAS TURNING TO GO IN A BETTER BOYD POSITION WHEN HE STRUCK A SHARP PORTION OF THE UPPER DECK FRAME . WITH HIS RIGHT CHECK RESULTING IN A SMALL CUT IN WHICH THREE STITCHES WERE TAKEN.
EE WAS ATTEMPTING TO CLEAR A ROCK FROM THE TAKE UP PULLEY. EE WAS STANDING ON THE GROUND WITH LEFT HAND HOLDING THE PIPE THE TAKE UP PULLEY WEIGHT OPERATES IN. HE SIGNALED FOR THE BOLT TO BE B UMPED AROUND WHICH SHOULD CLEAR THE ROCK. WHEN THE BELT (BOLT?) WAS BUMPED THE COUNTERWEIGHT SWUNG SIDEWAYS PINCHING EMPLOYEE'S LEFT INDEX FINGER.
