An employee was removing a lever hoist used to align a shaft during a bearing replacement. During the lever hoist removal, an employee's nose was lacerated when struck by the lever hoist. The initial X-ray results were negative for any fractures or broken bones. A fracture in the nose bone was revealed during a follow-up CT/MRI scan on 3/3/2026.

On 1/25/25 at approx. 8:30am a drill operator was injured when a hook from the drill winch cable fell from the top of the drill mast. The hook struck them over the left eye causing a laceration over their left eye. EE was taken to the ER, receiving multiple stitches. The operator had a follow-up appointment on 1/27/25 was released back to work 1/28/25 without Restrictions.

EE was placing a track pad when EE felt a slight strain in their neck.

EE was walking to the parts room to return tools and slipped and fell on ankle.

Operator was climbing down from machine and was using 3 point contact and missed the last step and fell to the ground and fractured their right ankle.

While doing maintenance on a crusher, employee was struck on the chin by a bolt that fell causing a laceration.

Employee struck pin on primary rock breaker with hammer. Upon impact a piece of metal came off the pin and struck the employee in the eyebrow.

Employee was repositioning a battery when it fell unto their foot causing a fracture behind toes

Employee was closing a Southway rail car door when a finger got caught between the door and latch.

While changing a return roller, the employee struck left thumb with a hammer causing a laceration and fracture.

While replacing guard after changing belts on screen the guard slipped and caught his hand between the guard and motor sheave resulting in a laceration to the hand.

A piece off a hammer broke off and entered leg while removing bucket teeth on loader.

While clearing down trees from drainage ditch, a tree entered the front windshield of cab injuring employee.

Employee was cutting the tips off of the used screen panels to remove them when the utility knife slipped and cut his left leg.

Employee was moving jack stands with hand trucks when he pulled/strained muscle in groin area.

Employee was attempting to remove rubber from a blown tire that was wedged between the rim of the truck. As he attempted to cut the rubber with his pocket knife, the knife slipped cutting two fingers on his left hand.

EE's were lifting the base off the to of machine at the scales. After the base was lifted, the swivel on the clam shell moved causing the base to travel back 12"", catching the EE's left leg between the base and electrical bax attached to the midrail. This caused a lacertion to his leg.

EE was pulling on a water hose when he tripped on the catwalk, striking a rock with his leg, requiring 10 stitches just below his knee.

Pulled lower back while opening lower gate.

While walking to a loader on #8 pile, twisted knee on uneven ground, where floor of #8 pile was being dug up.

Employee was installing a chute on the load out bin, utlizing a come a long. Come a long slipped & the chute twisted, which caused the ee's hand to be caught between the chute & the come along. Employees left index finger was severly cut and smashed. Injury resulted in stitches to the finger. Employee was wearing personal protective equipment, when the accident occured.

He mechanic was working on a brake cylinder which was mounted on truck. Started to tighten bolts and and bolts stripped from studs causing chamber to separate.

After going under a conveyor frame, the employee rose up into a structural I beam resulting in a broken nose, cut upper lip having 5 stitches, chipped front tooth and cut on forehead.

EE WAS USING A SLEDGEHAMMER TO BREAK A BEARING HOUSING. REPEATED BLOWS WERE NECESSARY TO BREAK HOUSING & THE RESULTING IMPACT/JARRING MOTION CAUSED SORENESS & PAIN IN THE RIGHT WRIST/HAND AREA. EE HAD NO PAIN IN WRIST/HAND ON THE FOLLOWING DAY (FRIDAY), HE NOTICED PAIN & TIGHTNESS ON SATURDAY 3/20/04.

EMPLOYEE WAS STRIPPING THE WIRES DOWN ON THE SMA SCREEN MOTOR, WHEN THE KNIFE ROLLED OFF THE WIRE AND STUCK INTO HIS LEFT HAND CAUSING A 1 1/2"" LONG LACERATION TO THE INSIDE PALM OF HIS LEFT H AND REQUIRING FOUR STITCHES.

EMPLOYEE DEVELOPED BACK PAIN WHILE OPERATING AN OFF-ROAD TRUCK. HE DID NOT REPORT UNTIL AFTER HIS TERMINATION ON 3/9/04. HE REPORTED ON 3/23/04 HE WAS UNABLE TO PROVIDE SPECIFIC DETAILS THAT LED TO HIS COMPLAINT.

WHILE LEAVING THE PRIMARY CONTROL HOUSE GOING TO PRIME DUST PUMP, EE STRAINED HIS LEFT KNEE GOING TO THE TRUCK.

EE WAS LOADING MATERIAL ON BACK OF TRUCK & SLICED HIS LEFT INDEX FINGER ON A SHARP PIECE OF METAL THAT WAS LYING ON BACK OF TRUCK. LEFT INDEX FINGER WAS SLICED BETWEEN PALM & FIRST KNUCKLE. TH IS CUT REQUIRED 8 STITCHES TO CLOSE.

EE WAS WASHING OUT A RAISED TRUCK BED AND A ROCK ROLLED OUT OF BED STRIKING HIM IN THE LOWER RIGHT LEG, CAUSING A CUT TO THE RIGHT SHIN REQUIRING FOUR STITCHES.

WHILE ATTEMPTING TO MOUNT A MOVING RAILCAR, THE VICTIM FELL AND WAS STRUCK BY RAILCAR, CAUSING LACERATIONS AND CONTUSIONS TO LEFT HIP AND PARTIAL AMPUTATION TO LEFT FOOT.

OPERATOR WAS DRIVING LOADED HAUL TRUCK TO CRUSHER WHEN HE FELL ASLEEP AND STRUCK BERM ON SIDE OFHAUL ROAD CAUSING TRUCK TO TURN OVER.

EE WAS USING A CLECO HAND HELD GRINDER W/6"" GRINDING WHEEL TO SHARPEN A BIT. THE GRINDING WHEEL CAME APART AND A PIECE OF IT WENT BETWEEN THE TOP OF HIS SAFETY GLASSES AND THE LOWER RIM OF HIS HARD HAT STRIKING HIM JUST ABOVE LEFT EYE. HE HAD A LACERATION & WAS TAKEN BY PRIVATE VEHICLE TO MED-CENTER NORTH MACON. FURTHER EXAM REVEALED A FRACTURE OF SKULL. TRANSPORTED FOR SURGERY.

LACK OF UNDERSTANDING THE TASK BEING PERFORMED AND IMPROPER POSITIONING.
