The senior blaster received fatal injuries when he was conducting a post-blast examination. The accident occurred when the ground on which he was walking caved in. The circumstances surrounding the accident and the cause of the accident are still under investigation.

Wet plant crusher wheel spun once struck hammer was free catching EE's foot as EE got out of way cutting EE's achilles tendon area

Cement silo was not flowing properly, ee shot air into silo to increase flow. EE put hand inside silo and paddle wheel caught fingers on left hand. Index finger was removed by doctor and middle and ring finger were broken but were saved.

Struck by falling object

Went to bathroom and was walking back. He walked under the belt of the crusher and a rock hit him on the right shoulder. It scraped and bruised it.

Fall from scaffolds, walkways, platforms

Cleaning the 770 dragline, crossed over catwalk where he was not supposed to be slipped & fell several feet to the ground.

Contact with hot objects or substances

Crusher was running & employee checked radiator as the machine was running hot. The radiator cap blew off resulting in a burn on the right side of his upper torso, chest & neck.

Over-exertion in lifting objects

Picked up a 5 gallon jug of oil and immediately felt pain in his hand.

Flash burns (electric)

Checking electrical problem. something occurred inside the electrical panel. which created an electrical explosion, resulting in burns to left hand and left side of face.

EE WAS IN OFF ROAD TRUCK WHEN BACKHOE OPERATOR HIT OFF ROAD TRUCK WITH BUCKET JARRING AND SHAKING UP EE

Drag line cable flexed out, hit his right hand & threw his right hand into a metal plate.

CLIMBING OFF OF LOADER, HE SLIPPED ON BOTTOM STEP AND HIT ELBOW ON STEP ABOVE.

Contact with hot objects or substances

EMPLOYEE WAS USING IMPROPER CRIBBING WHILE WELDING. CRIBBING CAUGHT FIRE AND EMPLOYEE UNKOWNINGLY LAYED HIS FOOT ON IT WHILE HE WAS WELDING.
