EE was struck by belt from behind causing EE to come in contact with belt rolling frame.

Employee was leaning across the drag line cable electrical box to verify there was no energy prior to performing work The door closed, hitting the employee. Incident date: 1/25/25. Became reportable when employee received restrictions on 2/7/25

Struck by flying object

EE was attempting to straighten 12"" U Bolt using a frame press with bottle jack attachment. EE turned U bolt on side to compress/reform bolt with a 4x4x2 piece of wood as a brace. When EE released bottle jack the stored energy caused the 4x4x2 piece wood to eject from the press, striking the EE in the face.EE was sent to emergency room for stitches.

Caught in, under or between a moving and a stationary object

Employee was attempting to adjust airbrakes using slack adjuster on a Mack truck. Another employee was instructed to engage airbrakes to check adjustment. Employee finger was pinched between slack adjuster and frame. Employee was sent to emergency room received 20 stitches to right ring finger around nail area.

Employee was using a Dewalt 6"" grinder to cut/fabricate a piece of metal for support for a welding machine. Grinder kicked back and struck left hand. Injury was to the left hand middle finger, ring finger, and forefinger, just above the palm. Employee was sent to Lee Memorial Convenient Care where EE received 13 stitches to the fingers mentioned above. Restricted Duty prescribed.

EE was splicing a de-energized 5KV electrical cord using Green Lee cable cutters. EE's hand slipped while trying to steady the cable and EE cut EE's finger. EE sheared the nail and some skin off of EE's left thumb.

Struck against a moving object

EE was in truck filling out pre operation inspection report, dropped pen on floor, bent over to pick up pen and strained his back. Supervisor witnessed incident. Sent to Doctor for evaluation.

Struck by flying object

Employee was grinding and welding at the 1260 Dragline. Employee was wearing face shield and safety glasses. Between the time he was welding and the time he returned to the shop had a pin size piece of debris enter eye. Employee washed out eye and thought all was removed.

Fall from machine

Dismounting Volvo Loader 220e #8 to clean windshield at wash station. While dismounting slipped, missed middle step and strained lower back catching himself. Employee was using proper technique (3 points of contact) when dismounting. Sent to doctor, light duty prescribed

Struck by falling object

EE was assisting in the removal of a pillow block on conveyor C0, at the primary crusher. After removing the block his foreman rested the block on a support frame to readjust hand position. The block fell and hit ee hand which was resting on another support. EE was sent to the Urgent care where he received six stiches to his right ring finger.

Struck by flying object

Employee was operating a push mower and struck a rock in the grass. The rock came out the back of the mower, through the guard and made contact with his shin. He was sent to Urgent Care as a precaution and medical evaluation. He received three stitches to his shin.

Struck against stationary object

EE dismounted equipment and stepped on a rock. Injury was to L foot, arch area. Minor contusion/bruise. EE was asked if he wanted to go to urgent care, he declined. 6/29/15 EE called me at approx 8:35 pm. I informed him to go to the hospital if he was in ""that much pain"". EE was sent to Podiatrist on 6/30/15 to follow up. MRI and blood work 7/1. EE admitted 7/4 w/infection L foot.

Fall from machine

EE got off of his 360 Volvo excavator to check for a coolant leak. When EE was attempting to board excavator he grabbed the windshield wiper when he started to fall. Windshield wiper gave way and he fell off of his machine. EE was sent to Lee Memorial hospital as a precaution. He received a head to toe CT scan and x-rays. No issue found.

Employee was working with a 4 1/2 grinding wheel adjusting safety chains on 4600 dragline. While cutting chain grinding wheel kicked back striking his left hand. Employee sustained a cut on the the knuckle side of his left hand, approximately 2 1/2 inch long. The injury required 3 staples. No loss of work, no light duty prescribed.

EE was cleaning/removing debris and rock that had become lodged between the frame and belt for the return for conveyor belt C8. The belt was not running, and was locked/tagged out. While removing the debris a rock became dislodged and smashed his right hand against the frame, fracturing his middle finger. No other injuries were present.

EE was using a utility knife to cut a conveyor belt. The knife slipped and he cut the top side of his thigh and needed four stitches, they will be taken out on Thursday and there is no loss off work or restrictions.

EE was changing screen decks that are held in place by J bolts. He was in the process of putting a nut on the J bolt when he cut his finger we took him to urgent care were he received stitches and was sent back to work. The doctor said there was no need for lost work.

Fall from machine

Employee was fueling the 800 excavator and slipped on the muddy tracks as he was getting down. The mud caused him to slip and fall about 4 feet to the ground. Employee tried to break his fall using his arm and immediately felt pain in his left wrist.

Clearing dense bush with chainsaw over head, chainsaw became tangled in dense weeds. Instead of turning it off, EE jerked chainsaw freeing it from debris causing the force to push the chainsaw downwards hitting his left knee.

Fall to the walkway or working surface

EE was burning brush when he lost his footing falling over a stacked pile of Pine Trees and Pepper Bushes falling face first putting hands out to brace his fall. Urgent Care 09/02/09 9:50 AM X-ray results showed Fracture to left distal ulna and radius. Referred to Orthopedist released light duty.

EE was drilling fence post holes when the auger he was using hit a rock and jerked him forward pulling a muscle in his left leg.

Employee was tightening bolts on stacker with a wrench. Wrench slipped free from his hand and struck his left cheek which resulted in a laceration that required 4 sutures. He is back at work today with a follow up appointment on February 16th to have sutures removed.

Flash burns (welding)

Employee was welding and cutting metal near our dragline without wearing his safety shield. He had been trained to do so in our weekly safety meeting. He received flash burns and the ER doctor gave him a Rx and to come back if he had pain. Employee was off on Sunday 12/2 but didn't report to work on Monday. He missed one day of work.

Employee was cutting a piece of belt and the knife slipped and cut the top of his left thumb. He had 2 or 3 sutures and came right back to work.

Fall from machine

Employee slipped while exiting the dragline in an improper manor; instead of exiting at one of the four proper exits, EE made a shortcut to what is called the shop (not the correct way to exit). The result was that he slipped & landed 3'to the ground & broke his wrist.

EE was walking down the steps on the Crusher when he stepped on a large rock. When the rock rolled out from under his foot he had rolled his ankle wich in turn resulted in a light fracture to his ankle.

Worker was using hammer to loosen shaft coupling being disconnected from motor drive shaft of hydraulic pump on drilling rig platform. When swinging hammer, blow glanced off coupling and worker's hand hit shaft where he was holding the handle of hammer. Laceration of right hand index finger requiring five stitches. Limit use of right hand, no lifting over 10 pounds.

ON 2/25/05 EE ALLEGES THAT WHILE ATTEMPTING TO LOAD A BLAST HOLE WITH A BULK MATERIAL, EE FEET DROPPED INTO A VOID THAT WAS CAUSED BY SOLUTION MINING OF OVERBURDEN MATERAIL. EE BODY ALLEGEDLY TWISTED IN A MANNER WHICH THE DOCTOR FEELS STRAINED SCIATIC NERVE. EE WAS PLACED ON PAIN AND NERVE MEDICATION AND DUTIES WERE MODIFIED
