While attempting to dismount Pit Loader 07-0014 (992K) from the rear access ladder, EE twisted EE's right knee as EE made contact with the ground.
While the employee was cleaning M-14 Crusher, a rock impacted EE's middle-left finger.
While pressure washing a company vehicle, EE attempted to crouch down to clean the lower portion of the vehicle when EE felt a sharp pain on EE's left knee.

EE was welding dragline bucket chains. At some point, a small metal fragment entered EE's right eye. In the initial consultation, a doctor removed the foreign object. At a follow-up visit, antibiotic drops were prescribed for an infection that had developed.
While working on the Concrete Metso Hammers, a small piece of metal shaving impacted their left forearm causing a very small laceration.
EE's cutting torch on the service truck wouldn't light. Upon inspecting the truck, EE found the acetylene quick connect had separated. When EE reached into the compartment of the truck, the acetylene gas that had escaped ignited, causing first degree burns to EE's left hand, abdomen, and face.

Employee was walking when EE stepped into a small hole/impression in the ground. EE's right hip shift unexpectedly causing discomfort. On Monday, while on shift, EE notified the supervisor, for the first time, that EE was in pain, then EE was taken to Concentra MC. Diagnosis Sprain of Right Hip. released with restrictions. We will provide work under those restrictions.
Employee was removing a shaft bearing when EE struck EE's left little finger with a 4-pound hammer, sustaining a laceration requiring stitches.
When placing the hose on the connection, the end of the hose that connects to the water hose hit employee.
When the employee attempted to mount a dozer, EE slipped and fell.

As EE was dismounting a 992 pit loader EE missed the last rung of the ladder, causing EE to put all EE's body weight on EE's arm, causing a sprain to EE's left wrist.
While EE was cleaning around the weight scales EE was bitten by an insect in EE's right buttock area.
EE has never reported any related Accidents, injuries or issues involving EE's shoulder. EE has been under the care of EE's primary physician for approximately 6 months. EE approached us on 11/06/23 stating that EE thought that this issue with EE's shoulder could had happened from working throughout the years.
While EE was cleaning and preparing the Bohringer Crusher for maintenance, EE slipped and fell (same level) onto buttock causing some pain to EE's lower back area. EE was taken to Medical Center, for medical attention.
While employee was resetting the hammers on M48 Crusher, the tool that was been used slipped out of their hands impacting EE in the lip.

Picking up for the company in the rain and tripped and fell from the stairs
EE was working / cleaning around M216 conveyor area with a water hose, when suddenly and unexpectedly EE felt a pain on EE's left knee.
Working on loader assembling the center hitch, was setting rear frame on to jack stands 25 ton with 3' tubes, was adjusting the tube that sets the height, as they pulled pin that adjusts this left hand ring finger was caught in the hole of the jack stand, as tube slid down it slid onto the finger caught in the jack stand hole.
While working on M20 conveyer, a rock bounced of the empty conveyer impacting the employees hard hat.
Employee threw welding connector, looks like a clamp, it bounced back at their nose.
While EE was walking to M215 Conveyor, EE stepped on a rock which caused EE to shift weight to prevent twisting ankle. This maneuver caused a strain to EE's right knee.

EE was working on M33 crusher when a single rock that was stuck on M26 conveyor head pulley, fell striking EE on EE's hard hat.
Employee was cleaning with a water hose, the hose became tangled causing them to twist left arm.
When was working on M26 Conveyor, a rock bounced off the belt impacting the left leg (calf). EE suffered a minor contusion from the impact of the rock.
While working on the DL the EE had water drip onto forehead. The water ran under EE's safety glasses and into eye. When the EE woke up the next morning the eye was irritated. The EE was taken to the DR. EE's eye was flushed out. DR prescribed antibiotic eye drops. EE returned to work.
EE tripped over a very small sand pile, causing EE to fall and impact the ground. The impact caused a contusion to EE's forehead, above the right eye. EE complained of neck pain, but refused medical attention, was given first aid. EE told supervisor EE would notify supervisor in the morning if the pain persisted.

EE was cutting a wear plate from a 4'x8' steel sheet plate with a track torch. As EE reached the end of the final cut the plate became unstable causing it to fall impacting the floor first and then EE's right foot.
Employee was crouched down behind MCC-1 and Approximately 13' in front of welding truck; preparing a skirt rubber for M-22 Conveyor. Driver got into the truck and began driving forward not seeing employee, impacting them and running over legs near the ankle, causing a fracture to the left leg just above the ankle.
EE was working on M-11 using the pressure washing truck, as EE stepped down from the truck EE's fingers got stuck between the handrail and taillight causing a laceration of the small finger in the right hand.
The utility Knife employee was using to cut a section of the conveyor belt slipped causing a laceration of right leg.

The day following shoveling fine material, employee said they felt soreness in their back but refused a medical exam or treatment. A medical checkup was not requested until 6/27/2017 at which time they received an appointment on the 28th.
EE slipped into a gator hole which was covered by vegetation while walking during the performance survey duties, causing knee to impact the ground and/or twist.
Walking backwards while using a hose to clean a discharge ditch employee lost balance falling backwards (on the same level). Employee sprained left hand/arm trying to break fall.

EE slipped on the M-14 deck while walking towards the stairs. EE grabbed the handrail with right hand which prevented ee from falling but caused a strain to right shoulder as ee was successful in keeping body from landing on the deck.
Employee's right thumb was crushed and nail bed damaged (nail avulsion) when EE placed their hand in the door jamb that closed unexpectedly. Diagnosis: Avulsion fracture of rt thumb; laceration of rt thumb.
A piece of anvil broke loose impacting EE leg; requiring 4 stitches; when EE struck the anvil to be removed with an 8 pound sledge hammer.

EE hit his left knee against the dozer track while he was dismounting the Cat D6 after he had fueled the dozer. The accident occurred on 10/8/2015 but ee did not request medical attention until 10/29/2015.
EE was moving a plate liner to work with it when his finger was caught between the liner an another piece of metal injuring his finger.
As employee was unscrewing the hydraulic hose on the tramping cylinder the residual pressure in the hose caused it to come off quickly, striking his finger. NOTE: Employee initially refused medical treatment until 3/13/2015 at which time he was treated and returned to work able to his fulfill his job responsibilities.

Employee dropped a tool inside a chute. He cut his forearm on a piece of flat bar while reaching to the tool.
EE attempted to lift the engine compartment door to gain access to the radiator when he strained his back.
EE was holding the bladder with a pair of vice grips while a co-worker was removing the steering accumulator. The vice grips opened striking EE on his lower lip.
While lubricating heavy equipment using latex gloves employee developed dry hands which lead to lacerations on his hands.
Employee was climbing on to the Cat 988H front end loader when he slipped and fell. His left elbow and knee struck the loader during the fall. The clinic gave first aid to his contusions. MSHA definable treatment did not begin until the MRI performed on 1/19/2012.

EE was exiting the cab of his loader, missed the step, fell forward and grabbed the railing applying force to his elbow.
Delivery Driver opened the door of his tri-axle delivery truck and stepped out to get his delivery ticket. His truck rolled back pinning his left thumb between the truck's door and the frame around the delivery ticket printer.
EE first came to us on 3/16/2011 saying he was experiencing back pain. He did state that the pain was caused at work. On May 2, 2011 he saw a doctor who indicated that his operating of a bulldozer was partially responsible for his back pain. The doctor prescribed medication.

EE was exiting the shaker screen unit when he misjudged the foot hold he expected, which made him to slip causing his left arm to support all his weight and dislocating his shoulder.

Injury jumped about one and one half feet from the concrete plant foundation to the ground. When he landed he felt a pain in his right knee. He did not think it was a reportable injury, but the next day the pain continued and got worse.

Employee was hammering a tooth on a loader bucket to loosen it for removal. A metal chip came off the bucket tooth and lodged in his left arm.
Climbing up the ladder to enter the cab of the Caterpillar 785 Haul Truck, employee slipped and fell approximately 5 to 8 feet to the ground. The area in which the truck was parked was muddy.
When employee was setting up a pull cable, the cable fell hitting and cutting his right leg.
Miner was clearing material from tail pulley area, when he hurt his back.
Employee said that his shoulder began hurting on 3/13/2007. He could not identify an incident or specific event that tiggered the pain. We had him go to an orthopedic doctor who indicated that in his opinion the shoulder pain was a work-related condition.

Employee was dismounting a dozer when he hit his knee against the dozer track.
Drill helper put his hand between kelly bar and the steel tube cap which is used to hold the tube down while the outer kelly bar is being pulled out of the ground. His arm was caught and pulled down iunto the borehole. The drill bar slipped on top of driller's hand. EE had surgery and lost right index finger, other broken fingers and lacerations to forearm.
He burned his forearm when he dropped the cutting torch he was using to cut plates in the plant.
Fell from Cat 740 Haul Truck after completeing refueling the truck and attempting to dismount.
Employee struck his L-hand middle finger with a sledge hammer while hammering piece into place resulting in a closed fracture to middle phalanx.
Operator of Cat777C Haul Truck fell asleep while driving empty truck to the pit. H/T ran off the road over the berm into rough terrain. Damages to H/T frame and minor injuries to operator.

The welder was grinding metal with disk grinder. He was wearing proper PPE consisting of safety glasses & welding hood. A piece of metal entered his right eye. The metal was imbedded in the cornea.
IE, who is a haul truck driver felt a sharp pain to his neck every time his truck hit a bump. Cont. cause: Unusual postures or repet. motions.
IW received lacerations to left ear and face & head contusion after he was struck by a 20-Ton bottle jack that slipped off while it was being used to raise a screen shaker. Failure to restrain the jack with chains to prevent ejection in case of slip off was the main cause of accident.
Open fracture of L-hand middle finger distal phalanx (tip) resulted after screen shaker pulley being replaced by IE slipped and fell over IE's injured body part. IE is being treated by physician. He is working under restricted duty (no use of L-hand). Contributing cause: IE failed to use proper mechanical devices (come-a-long) to hold pulley in place while being replaced.
IW was on ground ~ 4 feet from basket of manlift. Operator placed manlift in reverse motion, wheels turned right. Boom/basket abruptly swerved left and struck IW on right side of body. Left hand of IW got caught between midrail of basket and conveyor column causing L-hand laceration, left index finger fracture and partially severed L-thumb. Cause: untrained operator for equipment.
EE was on a conveyor belt that slide down, which caused him to twist and he injuring his ankle.
EE WAS CUTTING ROUND BAR ON THE HAMMERS WHEN A PIECE OF HOT SLAG FELL ON THE THE TONGUE OF BOOT. THE SLAG BURN THROUGH THE BOOT AND BURNED THE TOP OF EE FOOT
EE WAS HELPING TO INSTALL THE 3RD RAIL ON THE RIGHT HAND SIDE OF THE MACHINE. CO-WORKER ASKED IF WE NEED TO TIGHTEN ALL THE BOLTS ON THE 3RD RAIL. EE SQUATTED DOWN AND CALLED TO ASK ABOUT IT. HE LOST HIS BALANCE AND FELL ACROSS THE RAIL ONTO HIS BACK.

EE was replacing the hammers on M33 Crusher. During the installation the new hammers slipped. His right hand was breifly caught by the hammer and he jerked his hand back. The result was a sprained wrist and crush injury to the 4th finger.

A WATER HOSE WHICH HAD JUST BEEN TURNED ON SLIPPED FROM THE HANDS OF AN EMPLOYEE AND THE WATER STRUCK VICTIM IN THE EYE.
EE WAS ATTEMPTING TO REPOSITION THE TRAILING CABLE WITH AN INSULATED CABLE HOOK. THE CABLE WAS DAMP AND AS HE STARTED TO PULL ON THE HANDLE, THE HOOK SLID ALONG THE CABLE. HE LOST HIS BALANCE AND FELL BACKWARDS STRIKING THE BACK OF HIS HEAD ON THE D-RING OF THE CABLE TRACTOR PARKED NEARBY. THE BLOW CAUSED A LACERATION WHICH REQUIRED 9 STITCHES TO CLOSE.
EE WAS DISMOUNTING THE PAYSTAR FUEL TRUCK WHEN HIS FOOT MISSED THE STEP. HE FELL TO THE GROUND INJURING HIS HIP/PELVIC AREA.
EMPLOYEE WAS TURNING THE ROTOR SHAFT IN ORDER TO CHANGE THE HAMMERS. WHEN THE ROTOR SHAFT MOVED FURTHER THAN INTENDED HE TRIED TO GET OUT OF THE WAY, BRUISING HIS RIGHT LEG AND DISLOCATING HIS LEFT SHOULDER. NOTE: SHOULDER HAD BEEN DISLOCATED PREVIOUSLY. DOCTOR SAID SHOULDER WAS UNSTABLE PRIOR TO ACCIDENT.
