Fall from ladders

While climbing out of a rail pit employee brushed up against exposed live lead heater wires connected to hoist motor #1 running underneath platform. Wires were exposed and should have been disconnected or locked out. No damage or impairment was sustained to mining operation.

Employee was struck by a rock on the back of his neck.

employee was using arc gauge hook up to welder cutting off rebar & all thread from form reinforcement. He went to throw away the used rod & part of it dropped off into his boot & burned his leg before he could get his boot off.

The employee was moving a junction box for the page 740 dragline. He was walking along side of the junction box as the 966 loader was carrying it with a chain. The chain slipped off hitting the employee in the upper lip.

Employee bent over to retrieve his lunch box when he felt a sharp pain in the middle of his back.

Employee was in haul truck and the loader operator put a load of material in the back of the haul truck. The load had a big boulder that bounced the employee while he was in the haul truck hurting his back.

Employee was trying to fix the fan blade of the air conditioner on a loader when he was cut by the blade causing a laceration on his left middle finger.

EE WAS PERFORMING THE TASK OF GREASING THE HEAD PULLEY OF A CONVEYOR. HE HAD FINISHED GREASING AND WAS ATTEMPTING TO VISUALLY CHECK THE BEARING. HE CLIMBED OUT OF THE GUARDED AREA ON TO THE CONVEYOR BELT WITHOUT FALL PROTECTION. HE SLIPPED AND FELL 30 FEET TO THE STOCKPILE BELOW.

THE EMPLOYEE WAS CLIMBING BACKWARDS OFF THE GRADER. THE STEPS BENT UNDER THE EQUIPMENT. THE EMPLOYEE MISSED THE STEP AND FELL BACKWARDS ONTO THEGROUND ON HIS BACK AND RIGHT WRIST.
