EE was walking down the steps and slipped and fell. Xray of the left foot/ankle, there is a displaced, unstable fracture of the lateral malleolus with soft tissue swelling. Further care of the fracture. Oral pain medication, and a Sugar-tong ankle splint placed on the left lower leg. No weight bearing on Lt leg

Employee was cleaning up some build up in between the 403 pumps, Employee slipped and fell due to the slimy conditions landing on EE's left shoulder causing loss of mobility of EE's arm. Restrictions are in place light duty no lifting/using lt arm
EE tripped on a cable hitting L shoulder, L wrist, R knee. EE did not request med attention at this time. 6/12 EE complained L shoulder still sore. 6/13/24 EE was seen and diagnosed L should sprain, contusion L wrist, R knee. Follow up 6/17. EE restriction No use of L arm.

EE turn on pressure hose causing EE to lose footing. EE stepped on to slab base of staircase felt as EE was going to fall. EE stepped back awkwardly with LT foot causing toes to bend backward EE heard a ""pop"" coming from LT foot. EE was taken for X-rays on 11/6/23 EE has an ""acute"" fracture of the first metatarsal on left foot. EE was placed on sedentary duty.
EE was kneeling underneath ladder of the dredge removing bolts on the lower flange and placed EE's fingers through the grating above EE's head to balance self. Another EE working removing the U bolt dropped bolt to the grating striking the EE hand. EE was wearing gloves at the time of the incident. X-ray done on 10/18/2023 showed nondisplaced fracture. No restrictions

EE states was working on repair day at Wingate hanging a box and using tools. EE states that later that evening EE felt pain in their right elbow.

Section of handrail traveling down the stairway towards group of EEs. The ee nearest the landing heard the noise and saw the handrail coming down the stairwell at ee. In an attempt to protect self and the other ee, ee reached out to stop the handrail and left middle finger was pinched between the handrail and the outside wall of the screen.

EE was discarding used cutter teeth into the scrap bin when EE felt a pain in EE's left shoulder. A short time later while changing teeth on the Liberator dredge EE felt a throbbing pain and numbness. EE then contacted supervision to report the incident. EE referred to PT on 3/19/2021

Employee tripped on uneven ground and fell forward. Employee attempted to brace for the fall using right hand. The right hand then made contact with a truck door causing the pinky finger to be jammed into the palm.
Employee was struck by cable reel machine when cable became tight due to not clearing the starboard winch during unspooling. On 11/19/19 employee was placed on restrictions after the MRI results revealed multiple tears in the right shoulder.

On 6/24/18 EE was Cleaning up clay when the shovel came in contact with grating and jolted shoulder. EE was still having pain and was sent for testing on 7/24/18 as a result of tests EE was placed on restricted duty on 8/21/18.
On May 24 EE stepped forward to inspect a water box and stepped into a hole in the grating caused by the bearing unit coupling they were working on. EE was still having pain in right shoulder. On June 4, EE had MRI showing right shoulder rotator cuff tears, bursitis, joint effusion with synovitis and hypertrophic changes of the AC joint. EE was put on restrictions at that time.

During an ERT Drill conducted with Myakka City Fire Department, a responding ERT member cut hand on a breakaway tamper evident tag used to secure the zippers on the medical bag. The employee was transported to the Four Corners clinic for treatment. Surgical glue used for wound closure and placed on restricted duty.
Employee was leaving the cab of the dredge when employee realized employee had forgotten a paper when employee went inside employee held the door open with employee's left hand when employee's hand slipped going in between the door and the door frame. Door caught Left middle finger

EE was in the process of turning pump shaft with chain wrench to tighten impeller when the chain wrench failed to release pinching EE thumb. EE was wearing gloves.
Due to the cumulative nature of carpal tunnel EE found it to be an 80% work related and 20% personal as far as a major contributing cause is concerned.
Employee was working on a valve flange when wrench slipped off of the bolt and struck upper lip.

EE was moving a water cannon during clean up. EE was backing up and turning the water cannon at the same time with their right foot on concrete and left on grating. EE felt pop in their left knee while turning. *Accident was not recordable until 12/8/15*
Employee was using a powered hammer drill with a hole saw attachment. Employee reached down with his left hand to steady the piece being drilled on. The hole saw bit attachment became bound up and jumped out of the drill hole and struck the employee's left pinky finger resulting in a laceration. Employee received 7 sutures.
While at home on 07-13-15 at approx. 8:00pm, a Mosaic Plant Maintenance Employee began to feel discomfort in his eyes accompanied by sensitivity to light. Employee notified his Supervisor at home to report this condition despite not having experienced any discomfort during the work day.
While removing a blind out of a discharge chute on the log washer employee struck his head on an adjacent overhead structure. This resulted in a 1.2 cm laceration to the left anterior parietal area of the scalp which required sutures.
Employee notified supervisor of a ""slight"" irritation in left eye. EE suspected it was debris from gusty winds while working on the 2nd floor of flotation plant replacing floor grating. After work employee sought medical attention from personal eye doctor who removed a small piece of debris from left eye around cornea, and prescription medication.
Employee stepped from a pipeline pontoon onto a tugboat and slipped. His left shoulder struck the push knee of the tugboat and became dislocated.

On 8/7/14 while moving a hyd. pump, the pump slipped out of ee's hands. The other 2 ee continued to load the pump. At the time the affected ee felt no discomfort and did not report anything to supervision. On Sat 8/16/14 the affected employee went to Urgent Care complaining of back pain related to this incident. EE was placed on restricted duty by Mosaic Co PA on 8/18/2014.
Laborer on float crew was stepping over south matrix line with his left foot. As he applied weight to bring his right leg over, his left foot slipped in the sandy material and he felt his knee pop. He had immediate soreness in his left knee.
On Monday, May 5, 2014, a heavy equipment operator visited the Physicians Assistant at Four Corners' clinic with complaints of tingling in both hands and fingers. He was diagnosed with carpal tunnel syndrome as a result of repetitive motion related to 30 plus years of heavy equipment operation. Recordable due to surgery 6/12/14.
Results of 2014 hearing evaluation by independent audiologist indicates that employee has a Standard Threshold Shift (STS) in both ears.
Employee was removing a wire cable sling from a crane and placing it on a pallet when taking the sling off the hook it sprung back striking him in the mouth, causing a contusion to his lower lip and chipping a tooth.

(Previously reported as a FA on 12/05/13) Employee was re-routing a water jack discharge. While installing a 20"", 90 degree fitting, the employee twisted his knee. Injury became recordable due to surgery on 3/19/2014.

Worker was using 3' pry bar to remove face plate off north washer clean up pump, when he felt a pop in his right shoulder, followed by a sharp pain.
Employee stepped up on walkway to floating dock with right foot. Upon stepping on walkway with right foot, left leg went up to approach walkway and left quad muscle struck the left handrail post causing him to stumble. Employee scratched right ankle, left hand and left forearm and a contusion on the left knee.
Injured pulled guard of G1 Tails pump, turned and placed guard on ground. The Employee felt pain in lower back. After 1st visit, the nurse practitioner released employee to return to work with no restrictions. After the 2nd follow-up visit with the nurse, the employee was prescribed prescription medicine and put on restricted duty, due to condition not improving.
Employee stated that while exiting the cab of the dozer he lost his grip falling.

In an attempt to drive pin out of spreader bar employee was in the process of swinging an 8lb sledge hammer when he felt pop in the upper right shoulder blade.
Employee was walking along the curb of the reagent yard containment area when she slipped down off the curb and rolled an ankle.
Injured first complained of pain on 1/28/09 after several hours using a three pound sledge hammer to install poly deck screens. Pain subsided after shift but returned worsened three weeks later. Employee treated in-house on 2/23/09 for pain in right arm believed to be due to repetitive motion.
During movement of 10' Drisco pipe, the employee was struck in the legs by pipe, throwing him approximately 10 feet in the air. Employee landed on his right shoulder.

Employee was moving a canopy, it got caught in a bind, he went to move it and a piece of angle iron jumped up and his just above his right eye lid. Went to hospital, just got it glued and was released to go back to work.
Employee was changing out a gasket in pipeline. Employee had loader to pick up the pipe. Employee put the head of a bolt in between the flanges to wedge the pipe open. Employee was still holding bolt in hand when pipe was lowered; pipe twisted causing bolt to fly out from between flanges and cut employee's right hand on inside just under the thumb.
While walking down stairway, employee reached bottom step and turned, then felt pain in left knee.
Employee was bitten on the left forearm by an unknown insect while servicing Gulf #1 machine.
EE was handing bolts to another employee, who was at a higher elevation than EE, when a piece of debris fell into right eye. *The only reason this is being reported is due to a perscription that EE received at the doctors office. There was no lost time/restricted days and EE returned to Full Duty on 6-28-07.
Employee was performing PM routine tasks on plant equipment and became nauseated and said he felt faint. Employee was moved to air conditioned office area and first aid procedures for heat exhaustion were administered.
Employee was dismounting tractor after per-shift inspection; during the turning process employee felt right knee pop. Employee was give ibuprofen by Co Nurse and had knee XRayed at hospital. Employee was then put on restricted duty.
When descending stairs of the Maintenance conference room employee's left foot slipped. His fall was broken by another employee. Employee sustained a strained knee.
Employee put on his work gloves and was changing out ladder wire on the Gulf II dredge. Employee took his glove off & laid it to the side. When the employee put the glove back on he felt a sting on his left hand. Employee removed glove, inspected it and found nothing. Later that same day he noticed his hand swelling. While grinding on elevated steel beam in high winds, a foreign body blew into employee's eye & became embedded. Employee returned to regular duty after treatment.

Employee was cutting loose sand pit for move. He caught his foot on corner push post on sled. This caused him to slip and bang his shin on post cap plate. He was placed on restricted duty 5/19/06.
Employee received an insect bite while working in the warehouse on 5/2/06. He was treated conservatively until 5/8 when site was lanced and packed. He was given prescription medication and placed on restricted duty 5/8/06.

While holding up a man hole cover, cover slipped and fell on right hand across middle and index fingers. X-ray revealed no fractures and employee returned to work with no restrictions. Pain medication was issued after OTC medication would not relieve pain.
Employee was reaching above his head to grab valve handle while attempting to close a valve. He felt a sharp pain in his rib cage. X-rays revealed no broken bones. Employee was returned to work on restricted duty.
As ee walked near deoiling cyclones, he walked up a step and bumped his head on cross brace that obstructs walkway. He was looking down at his feet and did not see cross brace. He was treated as first aid until 3-17 when pain medication was prescribed and x-rays ordered.
EE WAS STUNG BY INSECT ON 2/10. NEXT MORNING, FINGER WAS SWOLLEN AND SORE AND EE WAS TREATED AS FIRST AID. EE WAS TREATED WITH ANTIBIOTIC THERAPY ON 2/14
While oiling and greasing dredge, felt burning sensation to back of neck; no contact w/chemicals; using gloves; unknown if from sweat, collar rubbing, touching neck w/gloves; sunburn, etc. Inflamed area approx. size of 1/2 dollar on back of neck along hairline - blister.
