Struck by flying object

Removing liners from a ball mill a liner was stuck, he struck the liner with a sledge hammer to dislodge it causing it to shatter and a piece of metal hit him in the lower right leg shin area causing a laceration and a piece of metal in right leg.

EE was inflating a tire on a 863 bobcat utilizing a ratcheting strap to seat the tire to the wheel. When the strap was released it re-coiled upward striking the employee's forearm requiring 8 stitches to close.

While operating a walk-behind concrete saw, a rock jammed the saw wheel. Employee reached to remove rock and his sleeve was caught on saw arbor causing forearm to contact blade.

Struck by falling object

EEs were replacing supporting pipes for head protection inside the ""kin riser"". Debris came loose, struck EE on the head and shoulder, knocking his hard hat off and causing a contusion on his shoulder. Another piece then fell and struck his head, he was wearing a Tyvek suit hood and a head sock. He was treated and released from the hospital.

Employee was operating a vacuum truck suction hose. While in the process of cleaning he was holding the 8"" hose with his left hand and maneuvering the hose with his right hand. The hose turned and engulfed his left arm in the hose causing a negative suction to his arm.

In the process of adding grinding balls to the coal mill, the drum containing the grinding balls tipped over striking right arm against metal chute.

Employee was cleaning out a plug in the nextor hopper (massary additive) by standing on a platform and digging into it with a shovel. When ee returned to the lab, she compalin of back pain when she breathed. The dr prescribed pankillers and muscle relaxers for her muscle sprain and gave her a release to came to work.

Struck against stationary object

Employee was closing a port door on a preheater tower. The hinged section of the port door was stuck, requiring more force to be applied when closing. As the hatch closed, his hand slipped causing a laceration to the finger requiring 4 stitches.

Employee was installing a chute on a clay conveyor. While maneuvering the chute into place, his hand was caught between two pieces of steel. This caused three fingers on his left hand to be smashed, resulting in contusions to fingers.

Employee tripped on a 1/2 inch raised portion of walkway grating. The fall resulted in a small laceration to her knee.

Employee was using a pry bar to close the bottom door of a cement railacar. His hand slipped from the pry bar, causing him to lose his balance, and twist his ankle. This resulted in a sprained ankle.

Employee was kneeling down on top of a metal tank performing electrical repairs to a bindicator. When he stood to leave the area, he felt a pain in his right knee. He requested medical attention the next day ad was seen by the emergency room physician who referred him to our regular WC physician who placed him on restricted duites.

Employee was manually pulling welding cables from the ground floor to an elevated platform when he felt a pain in his lower back. He requested to see a physician on 2/13/2007. He was returned to work with some restrictions until further evaluation. Restricted duties began on 2/14/2007.

Employee was cleaning the discharge screen of a finish mill. The screen was plugged with hardened cement. As the particles broke loose from the screen, material blew into the employee's eyes. One eye sustained a minor corneal abrasion, requiring prescription of an antibiotic ointment and restricted duties for one day.

Employees were working in the roll mill, the wind blew some cement dust and it got into EE's right eye. He rinsed it at an eyewash station, but still felt something in his eye, so he was taken to get it looked at by a physician.

Employee was standing beside a stack of metal plates when the stack tipped, trapping his left foot under one of the plates.

Employee was removing a pipe cap with a large pipe wrench when his hand slipped from the wrench. This caused him to stumble and fall, subsequently twising his ankle. The fracture to the ankle required surgery.

EE was operating a cement bag packing machine. As a bag was filling, the bag ruptured, blowing cement dust onto the ee. The ee was wearing safety glasses and a face shield at the time. Some cement dust entered the ee left eye, causing irritation.

EE injured knee while working behind maintenance shop. The employee tripped over a pallet and twisted his knee. After evaluation, he returned to work. He had surgery while off work from a non-work related medical condition. Upon release by his dr, he RTW with no restrictions. EE continued to feel discomfort & was sent for further medical evaluation. Light duty began 4/10/2006.

A NEARBY EMPLOYEE WAS USING AN 8 LB SLEDGEHAMMER TO REMOVE A SHAFT. THE INJURED EMPLOYEE WALKED BEHIND THE EMPLOYEE USING THE SLEDGEHAMMER AT THE MOMENT HE PUT THE SLEDGEHAMMER TO REST ON HIS SHOULDER. THE INJURED EMPLOYEE WAS HIT ON TOP OF SHOULDER WITH THE HAMMER. THE BLOW WAS WITH MINIMAL IMPACT, BUT CAUSED SLIGHT BRUISING.

EE WAS INSTALLING A DRIVE BELT ON A SHEAVE. FINGER WAS CAUGHT BETWEEN THE DRIVE BELT AND THE SHEAVE. SHEAVE GROOVES WERE WORN, CAUSING THE EDGES TO BE SHARP. FINGER WAS SEVERELY LACERATED WHEN PRESSED INTO THE GROOVE OF THE SHEAVE.

Employee was welding, torching & grinding on 3/13. The next day he thought he might have burned his right eye welding. Employee returned to TN from FL on 3/17, right eye had gotten worse, went to doctor on 3/19 and it is believed that he got debris in right eye while grinding, which caused infection.

EE was changing out a damaged screw auger. the old screw was lying on the floor adjacent to the immediate work area. the ee stepped back onto the screw auger and tripped, falling onto the auger. the screw auger scraped his lower back slightly. the ee felt o.k. at the time but began experiencing lower back pain by the 25th of march.
