EE was performing a drag resocket. EE experienced a laceration to the distal phalanx of the right-hand little finger when a small chip of metal that was embedded in the sledgehammer's handle cut the glove and finger. EE received stitches.

EE was in the process of assisting the adjustment of the travel chain. A hydraulic ram was used to tension the chain. During the process, the spacer was ejected, striking the employee and causing a laceration requiring stitches.
As employee was pushing the drag drum dirt pan into place, EE felt a pop in EE's left elbow. First aid was administered. However, injury required surgery on 09/21/2022.

While disconnecting the D-ring from the cable tractor, the D-ring rolled forward onto the EE's foot. A bone in the EE's foot was fractured.

Employee had his rt. foot caught in/between a winch drum and the floor plate, resulting in the steel toe on his boot being smashed. There were exposed bolts on the winch drum that pinched his foot and resulting in a cut/laceration of the rt. big toe.

EE was fueling his loader. As he twisted to replace the fuel nozzle he heard something in his back pop. He was in a great deal of pain and was taken to the hospital. He was prescribed medicine and placed on restricted duty by the physician. The physician states the injury may be a herniated disk.
Employee caught his wrist between a roll and a door while turning the roll on a portable crusher. His wrist was hyperextended backwards and he suffered a sprain.
