Maintenance Employee was installing sheet metal roofing on a structure and caught corner of a piece of roofing with EE's right forearm resulting in a 3 Inch laceration that required 9 stitches to close the wound. No lost time or work restrictions.
Was operating Excavator. Got out and walked over to talk to dozer operator. As EE was walking over, EE stepped in a hole and dislocated their ankle.

Softrock Haul Truck Operator was operating a CAT 777G Haul Unit too close to berm on left side of road and ran into berm knocking off left wheel of Haul Unit. Truck rolled over onto its left side. Operator jumped out of truck injuring left arm, elbow and left foot.

On 6/18 employee was involved in a vehicular collision which resulted in minor back pain to lower back. Employee did not want to seek medical attention on that date. On 6/24 employee was still experiencing back pain and was taken to the doctor where EE was placed on restricted duty.
Maintenance Mechanic was walking to the work station and rolled ankle resulting in a sprain.
Employee was driving company truck (Ford F150) and veered off a paved road and hit a concrete barrier causing airbags to deploy striking face and chest. Employee sustained a cracked sternum and broken nose.

On 6/8/20 Employee was moving a tow cable and strained EE's right shoulder. Reported Incident on 6/9/20 and did not ask for medical attention on that date. Taken to doctor on 6/11/20 and given light duty work restrictions by physician.
Employee was moving a tow cable and a wire from the cable punctured EE's left forearm resulting in a small puncture wound. Employee went to doctor following day and received a tetanus shot. Back to work with no days off.

Employee was swinging sledge hammer and strained right shoulder. Taken to doctor for examination and EE was placed on light duty.
Operator dismounting dragline. Guardrail safety chain across ladder access had been unlatched and hanging down in close proximity to access ladder. While dismounting, operator's finger became snagged on the latch on safety chain, lacerating the finger.
Employee was stepping down from truck, missed a step and fell on the ground.

Employee was operating a McCloskey ST110 Portable Stacker via remote control and ran over ee left foot with the left track of the machine, breaking 2nd and 3rd metatarsal bones in foot.
Employee was removing hydraulic line on gate actuator and nut on hydraulic hose hit employee in the mouth and chipped the Left Front Tooth. No other injury sustained, No Restrictions. Came back to work that day. Needs to get tooth repaired on 5/15/2018.

Haul Truck Operator was bending over to get into the cab of EE's CAT 777 Haul Unit and felt a sharp pain in EE's lower back. The pain persisted and EE notified EE's supervisor and was sent home. The next day EE went to the clinic and was diagnosed with Sciatica. EE was placed on light duty for 1 week. EE returned to work on 8/26.
EE was removing the belly pan off of a 615 Cat water wagon. When EE was removing the last bolt, the pan came down and landed on hand and wrench. This caused a small cut on the palm of hand below the index finger. The cut required 4 stitches. EE returned back to work and hour and a half later.
On 1/14/2016 Mechanic sustained a puncture wound from a fractured piece of metal after striking a crusher hammer with a sledge hammer. Employee taken to doctor and released to Full Duty, No Restrictions. On 1/29/2016, the employee was taken to doctor for follow up and placed on restricted duty, No heavy lifting. On 1/29/2016 this injury became a reportable injury

Employee was lifting the lid of a metal container and the lid hit his forehead and caused a laceration on the left side of his head that needed 7 staples. He was released to full duty.
On 8/21 employee sustained a puncture wound while using a hammer and punch. On 8/25 he was complaining that there was a piece of metal in his left hand and taken to the doctor. The hand was x-rayed and a piece of metal was. He was sent back to work with NO Restrictions. On 9/2 he had the piece of metal surgically removed and placed on restrictions with limited hand use.

Fixed Plant Mechanic was replacing a conveyor belt on a Power Screen and sustained a small laceration to the top of his left ring finger while he was trimming the belt. He was taken to the ER and the laceration was cleaned and glued together. No restrictions were given. He was given a tetanus shot and prescribed antibiotics.
Fixed Plant Mechanic was helping Mechanic remove a 200 pound bearing from a pallet. During the removal process, they dropped the bearing and it landed on top of Plant Mechanic's left foot, which resulted in the fracture of 3 metatarsal bones.

Electrician was walking down the North 42 Inch conveyor catwalk, slipped and fell backwards. He landed on his left elbow and felt pain in his left shoulder. He was taken to the doctor the following day (11/08/2013) and placed on light duty (no overhead work) and given pain medication.

Employee was carrying a block of wood while walking behind his service truck. His right hand collided with a step on the back bumper, causing his finger to be smashed between the block of wood and the bumper. The employee sustained a laceration to his right ring finger requiring 10 stitches.
Employee was climbing down the ladder of a CAT 777D Haul unit and lost his footing. He fell backwards and hit his head on the ground resulting in a 1 inch laceration to the back of his head. The injury required 3 stitches.

Employee was using a utility knife to cut a piece of skirt rubber. The knife slipped and cut the employee's right thigh. Employee sustained a three inch laceration that required 6 stitches. Employee was returned to full duty with no restrictions.
Employee was turning handle to a winch and let go of the winch handle. The handle to the winch spun backwards, hitting the employee on top of the left hand, breaking two bones in the hand.
Employee was removing a pin from a pneumatic actuator utilizing a sledge hammer and a punch. Upon striking the punch with the hammer, a small piece of metal from the hammer punctured the employee's left hand. The metal had to be removed by a physician.

Employee was lifting the hood on the Mack Blasting Truck and felt a sensation in his L lower abdomen. Later that day he began to experience pain in the area. He was in pain the next day and sought medical treatment. He was diagnosed with a hernia in his L lower abdomen.
On 8/11/07 employee was welding a discharge lip to a shaker screen. He was supporting the metal lip with his right arm. He turned his body and felt a pop in his shoulder. He felt minor pain in the shoulder for several months and on 1/2/08 he asked to see a doctor. Employee was released to full duty on 1/2/08 and given a prescription steroid for inflammation.
Employee was washing off scale with water hose. She lost her footing and fell backwards landing on her left hand and buttocks. She suffered a sprain to her left wrist.
Employee was lifting an 80 pound wear liner into crusher. During the lift he felt a pop in his left shoulder and experienced pain. Employee was taken to the doctor the next day and treated for muscle strain.

Employee was climbing up the ladder of a Cat 777 Haul unit and her hand slipped on the railing, but she did not fall. Later that evening she complained of back pain. The back pain persisted for several days. On May 24 she asked to see a physician because the back pain had not improved. Doctor reveiw revealed muscle spasm and she was treated with medication and placed on light duty.
Employee and two other mechanics were lifting a metal skirtboard in place. During the lift the employee felt a pop in abdomen. A small lump in lower abdomen was noticed the next day. Employee visited physician for diagnosis. Employee was diagnosed with small hernia in abdomen. Employee placed on light duty and scheduled for surgery.

Employee was tightening bolts on screen deck with a 3/4 inch impact wrench. Socket and wrench separated during tightening and wrench traveled up employees arm inflicting a 1 inch laceration on his wrist requiring six stitches.
Employee was washing spilled material underneath plant with hose. She turned valve off on the hose she was using and another hose that was running underneath plant came loose from its location due to the pressure change and hit her elbow. Her elbow remained sore for a few weeks and she was taken to the doctor on 9/26/2005 for evaluation.
Employee was chipping slag from a weld with a hammer. A piece of the slag penetrated his left wrist. He visited the doctor and the incident was initially treated as a first aid. Upon returning to the doctor (8/17/05) for a follow up visit, the doctor recommended the piece of metal be removed. The metal was removed on 8/19/05 and a stitch was placed over the wound.
Employee was stepping off of a 920 Loader. When his right foot hit the ground, he felt a pain in his right knee. His knee continued to hurt throughout the day and he was taken to the doctor. He was placed on light duty and scheduled for further diagnostics of the knee.

EE WAS DESCENDING THE SOUTH LOWER STAIRWAY FROM THE PRIMARY CRUSHER DECK. ABOUT THE THIRD STEP DOWN, BOTH FEET SLIPPED ON THE STEPS, LEAVING CONTACT WITH HIS HANDS ONLY. EE TIGHTENED HIS GRIP ON THE HANDRAILS TO PREVENT BODY CONTACT WITH THE STAIRS. IN DOING THIS, EE CLAIMS TO HAVE TWISTED HIS LOWER BACK.
A Commercial Truck entered the scale and knocked the drain pan out of position with the tarp rack. Scale house operator climbed the ladder on the side of the truck to put the pan back into place. When the pan dropped back into position, the operator's thumb got pinched between the pan and the chute. Right thumb tip was cut, his nail was removed and he received six stitches.
Employee was walking down a flight of stairs in the Screenhouse and fell. Aparently, the employee missed the first step, fell on his back and slid down the length of the stairway. He dislocated his right shoulder as a result of the fall.
Employee was working on the Pond #12 pump platform. Another employee tossed him a bottle of oil from the bank (The toss was too high). Terry fell backwards trying to catch the bottle and landed on his left hand, breaking the wrist.
A piece of metal apparently entered employee's Left eye after wiping sweat off of his brow with a shirt sleave. He rinsed his eyes in the eye wash station and felt better. The next day he was in pain, so he was taken to the doctor where the object was removed. The cornea was scratched.

EE WAS CLIMBING DOWN A FIXED LADDER IN SCREEN HOUSE & ATTEMPTED TO STEP OFF LADDER TO A STAIRWAY JUST TO LEFT OF LADDER. (THERE WAS A HOSE PILED UP UNDERNEATH LADDER.) WHEN EE PLACED HIS FOOT ON THE STEP BELOW, IT SLIPPED OUT FROM UNDER HIM & HE FELL ON HIS TAILBONE. BECAUSE OF THE HOSE BEING IN THE WAY, EE DID NOT HAVE A SUITABLE PLATFORM TO EXIT THE LADDER.
ELECTRICIAN WAS STEPPING INTO THE BED OF A WORK TRUCK AND STRUCK HIS RIGHT KNEE ON THE TAILGATE OF THE TRUCK. HE WAS EXAMINED BY A DOCTOR THE AFTERNOON OF THE ACCIDENT AND GIVEN A DIAGNOSIS OF ""MUSCULAR CONTUSION"". HE CLAIMED TO HAVE A GREAT DEAL OF PAIN, SO THE DR PRESCRIBED PAIN AND ANTI-INFLAMMATORY MEDICATION. WAS PUT ON LIGHT DUTY FOR A WEEK.
WHILE TROUBLE SHOOTING A 4150 MAIN BREAKER PANEL, EE MAD CONTACT WITH 4160 VOLTS IN THE BUS BAR PANEL NEXT TO THE MAIN BREAKER. EE FAILED TO FOLLOW POSTED PROCEDURES BY VISUAL CONFIRMATION THA T THE KNIFE SWITCHES FOR THE BREAKER WERE DISENGAGED EVEN THOUGH THE MAIN BREAKERE SWITCH WAS IN THE OFF POSITION. THE BREAKER SWITCH SHAFT WAS BROKEN.

EE WAS USING A CHISEL BRAND NEW TO SPREAD LINKS ON A TRACK SYSTEM. THE CHISEL SNAPPED IN HALF AND CUT HIM ABOVE THE RIGHT EYEBROW.
EMPLOYEES WERE FIXING THE ENTRANCE GATE WHEN INJURED RELEASED HIS SIDE, THE OTHER EE SLID THE BLOCK FURTHER AND CAUGHT EE'S INDEX FINGER(LEFT).
EE WAS ADJUSTING SEAT IN HAUL TRUCK. HE OVERADJUSTED THE WRONG WAY AND THE SEAT COLLAPSED. THE SEAT WAS APPROXIMATELY 2 WEEKS OLD. THE INCIDENT HAPPENED IN THE TRUCK LOT BEFORE HE STARTED THE TRUCK.
EE WAS HELPING CHIP CEMENT BUILD UP. HIS PANTS GOT WET AND CEMENT GOT INTO A SMALL ABRASION. HE THOUGHT IT WOULD CLEAR UP BUT IT DIDN'T AND GOT INFECTED. FIRST REQUEST FOR MEDICAL TREATMENT ON 4/12/01.
EE WAS WELDING. HOOD & LENSES CHECKED OUT AND WERE ALL RIGHT BUT WERE REPLACED ANYWAY. ONLY CAUSE DETERMINED WAS WATER REFLECTING ON METAL. EE DID NOT REALIZE PROBLEM UNTIL 2100 HOURS THAT NIG HT.
EE PLACED HIS HAND ON DOOR JAM AND SPLINTER LODGED UNDER HIS FINGER NAIL.

SUPERVISOR WAS HELPING ANOTHER DEPT TO DISABBEMBLE A PUMP. THE BRAKE ASSEMBLY BREAKING WERE STUCK. WHEN THE ASSEMBLY WAS LOOSENED IT WENT TO REVERSE CATCHING HIS THUMB DUE TO UNKNOWN PRESSUR E IN THE BASE OF THE PUMP.
EMPLOYEES WERE CHANGING ROLLERS. AS THEY TIPPED A ROLLER TO REMOVE IT, IT HIT AN EMPLOYEE'S LEFTTHUMB, PINCHING IT BETWEEN THE ROLLER & THE BELTFRAME.
EE WAS REMOVING A HOIST FRICTION HE DID NOT WEAR GLOVES & WAS CUT RESULTING IN 6 STITCHES
EE STOPPED HIS HAUL TRUCK TO TALK TO ANOTHER DRIVER. HE LEFT TRUCK IN GEAR AND WHILE HE WAS STANDING ON HIS CATWALK, HIS HAUL TRUCK ROLLED INTO THE OTHER HAUL TRUCK. HE WAS PINNED AND GRABBED ONTO THE BED OF THE OTHER HAUL TRUCK, DROPPED TO THE GROUND, RAN AFTER HIS HAUL TRUCK, CLIMBED UP AND STOPPED HIS HAUL TRUCK. HIS INJURIED OCCURRED AT IMPACT.
EMPLOYEE WAS MAKING REPAIRS ON AN AIR CONDITION-ING SYSTEM ON A LOADER. HE THOUGHT HE'D TURNED THE DISCONNECT TO THE OFF POSITION; HIS KNEE HITTHE REMOTE START WHILE MAKING THE REPAIRS & THE ENGINE STARTED.
EMPLOYEE WAS REMOVING BOLTS TO CHANGE OIL ON AN ARTICULATING TRUCK AND WAS HOLDING UP THE BELLY PAN WHEN HE LET THE PAN DROP, HE DIDN'T MOVE HIS HAND OUT OF THE WAY ON TIME.
EE SAYS THAT WHILE HE & 2 OTHER EE'S WERE ATTEMPTING TO REMOVE A TOOL BOX MODE OF 3/8"" PLATE, HEWAS HOLDING IT & HURT HIS BACK. HE FRIST BROUGHTBACK INJURY TO OUR ATTENTION 1ST WK OF JUNE. HE WASNT' SURE IF IT WAS WORK RELATED OR JUST AN O LD BULL RIDING INJURY. HE DECIDED TO GO ON HIS OWN BECAUSE HE WASN'T SURE. HE RETD 6/16 WITH NO RESTRICTIIONS. THE BEGINNNING OF SEPT HE HAD TO
EE RAN INTO THE BACK OF A PAN TRK (CONVERTED INTO WATER WAGON) WITH A 55TON HAUL UNIT. WHEN HE REARENDED THE PAN, THE HAUL UNIT BOUNCED CAUSINGA MUSCLE STRAIN IN THE LOWER BACK IT APPEARS THAT HE FELL ASLEEP (CONVERSATION WITH WIFE RE VEALED HE HAD BEEN UP ALL NIGHT THE NIGHT BEFOREWITH A SICK CHILD)
