Employee was rigging up the cone ring to remove it from the cone. It got bound up at an angle and would not move. EE reset the rigging then started to descend from cone. The cone ring did not have any pressure from the rigging. Descending from the cone onto the platform EE put EE's hand out for support, the ring fell into its resting position just 2"" from bound position.

Employee went to change the cross-conveyor belt using a tool box knife to cut the old belt and pulled it towards themselves, cutting their thumb through their glove.

Employee was helping change cutting edge on skid steer. Employee was holding bolt head with hand when other employee tightened nut with electric impact spinning nut in employee 1's hand cutting EE's finger resulting in 5 stitches.
