Employee was assisting changing out a water pump and a section of 10"" diameter pvc pipe slipped from it's supported location and landed on employee's right index finger causing slight fracture to tip of finger bone and laceration that required approximately 4 stitches to close.

Injured employee was cutting a piece of belt to repair a conveyor belt, when the knife the employee was using slipped and employee cut right thigh.
