An employee was helping change a screen on the screen deck. The employee had another employee holding the screen to help out. While trying to climb out of the screen deck, the screen was dropped and hit the employee in the head, resulting in 5 stitches being needed.
Employee was swinging sledge hammer and felt a pinch in EE's elbow.
Cutting rubber skirting with utility knife, cut into self instead of away knife bounced and into upper part right leg, 3 inches along about 1 inch deep requiring 4 stitches. Employee returned to work full duty.
fell out jaw ladder 3 ft off ground twice never reported after either fell on back, 730 and 1030 reported at 12:15 told other employees will tough it out was told to report by other employees
Employee was pre shifting vehicle after safety meeting, and cleaning windows on Volvo A30 haul truck, started to wash the windows, standing next to the fueling spout on the flat are provided for standing, slipped on fuel residue and fell approx 6 feet to the ground on his lower back and butt. Employee also had a headlamp on, the sun was just starting to come up.
While loading boulders in a 30 ton haul truck a rock slipped out of the thumb of the excavator allowing it to fall into the bed of the truck (approx. 5-7 feet) jarring the truck operators neck.

WHILE EXITING THE BASKET OF A ""GENIE"" 45' MANLIFT THE EMPLOYEE LIFTED HIMSELF UP TO EXIT THE BASKET. WHILE DOING SO HIS LEFT SHOULDER POPPED/DISLOCATED OUT OF SOCKET.
SLIPPED ON THE LADDER OF HIS LOADER WHEN CLIMBING DOWN AND HURT HIS KNEE.
EE WAS SHOVELING ROCK WHEN A BOULDER FELL OUT OF THE BUCKET OF THE LOADER, ROLLED DOWN A RAMP AND STRUCK EE IN THE BACK. HE HAS A BROKEN RT. COLLARBONE AND BROKEN LEFT KNEE CAP.
EE JUMPED FROM HIS DOZER AND CAUSED A SLIP DISK IN HIS BACK.
