Employee was struck in chin/jaw with piece of metal being removed from the screen.

Employees were removing the screen frame. It became dislodged quickly and struck an employee in the chin causing a laceration.

Employee's hand slipped while swinging a hammer. The hammer struck EE's face causing tooth damage.

The employee was cutting urethane chute liner when the utility knife slipped out of the cut. This resulted in a laceration below the right thumb. He received 9 stitches.

Employee was stepping off a bench after cleaning a screen and sprained his left ankle.

Employee's finger was caught between a chain and the conveyor.

Employee was using a torch. He pulled back on the torch and his glove caught fire, burning his hand.

Employee slipped while exiting cab and fell to the ground, injuring his right arm. Injury was not recordable at that time. Recent medical evaluation led to surgery 5/12/04.

EE STARTED DOWN OFF BOTTOM STEP OF LADDER AND RIGHT FOOT SLIPPED, CAUSING A STRAINED KNEE. EE EXAMINED BY DR AND RETURN TO WORK. EE WENT BACK TO DR 2-9-04 AND WAS PRESCRIBED PHYSICAL THERAPY.

EE WAS EXPERIENCING PAIN IN RIGHT WRIST.

EMPLOYEE WAS WALKING BACK TO OFFICE FROM CHECKING OUTSIDE SCALE TICKET PRINTER AND SLIPPED, WHEN SHE SLIPPED, SHE FELL AGAINST A PUMP HANDLE INJURING HER RIBS.

EMPLOYEE REPORTED TO MANAGEMENT ON 1-19-03 THAT HE THOUGHT HE HURT HIS BACK ON THE PREVIOUS DAY SHIFT WHILE POSITIONING A 10"" BY 20"" PIECE OF PIPE.

CONVEYER SWUNG TOWARDS CRANE AND BOUNCED OFF OF THIGH.

DIGGING OUT FEEDER CONVEYOR AND CULVERT, BOTH SIDES WERE EXCAVATED. A BIG CRACK IN MATERIAL OPENED UP AND MATERIAL SLID INTO HIS KNEE.

EMPLOYEE CLIMBED ONTO A MACHINE SHED ROOF TO NAIL DOWN LOOSE TIN, EMPLOYEE SLIPPED ON THE ROOF AND FELL TO THE GROUND.
