The employee was cutting urethane chute liner when the utility knife slipped out of the cut. This resulted in a laceration below the right thumb. He received 9 stitches.

EE STARTED DOWN OFF BOTTOM STEP OF LADDER AND RIGHT FOOT SLIPPED, CAUSING A STRAINED KNEE. EE EXAMINED BY DR AND RETURN TO WORK. EE WENT BACK TO DR 2-9-04 AND WAS PRESCRIBED PHYSICAL THERAPY.
