While the operator was operating the dozer they were backing up. They placed a hand on the arm rest when they went over a rock the arm rest failed causing a finger to dislocate

Struck against a moving object

Injury occurred on 12-04-24 and employee was evaluated by WC Dr. with no restrictions given. Employee was given restrictions one week later and employee missed scheduled shifts due to restrictions. A haul truck operator was being loaded by wheeled loader when a frost chunk was dumped into the bed. The operator complained of neck and shoulder soreness and headaches.

During Mine Rescue Training of the use of a highline system, member injured right elbow while pulling on the rope.

Injury occurred 7-24-24 as a report only. Employee expressed that they weren't seeing any improvement requested to be seen 8-27-24. Employee continued to work scheduled shifts until 12-4-24. Scheduled surgery 12-5-24. Employee was air lancing in a chute at the crusher, got a sharp pain in their right shoulder

Process Maintenance Tech was walking on a road during a snowstorm then they slipped and when they landed, they ended up sitting on their right ankle. Update: EE did not miss any scheduled shifts nor given restricted duties. EE did not improve & requested to be re-evaluated. EE continued to work with no restrictions until 2-13-25 when surgery was performed to right ankle.

Employee was holding a tagline for a generator being moved by an excavator. The generator swung and pulled the employee forward causing them to fall in front of the excavator track where their foot was caught between the track and the ground.

Maintenance Tech was maneuvering a hoist pump into place when their left thumb got pinched between the housing and a hydraulic hose resulting in a laceration.

While working in drive cabinet, the workers hand contacted a sharp wire-tie, resulting in a cut requiring 3 stitches to the top of middle finger on left hand.

Maintenance mechanic was using 4lb hammer to strike Allen Wrench with extension. When the extension was struck, the Allen wrench ""popped"" back contacting the technician's face resulting in a broken tooth.

IP was being trained in the use of a hose making machine when the trainer was demonstrating how to place dies. While in the process of closing down on the dies, an object struck the IP in the right shoulder.

Process operator was connecting water line to Hydrochloric Acid pump to flush pump for scheduled repairs. The PVC fitting broke and the operator was sprayed in the shoulder and face.

Haul truck operator was backing up to a shovel looking over right shoulder at mirror and hit a bump causing neck pain. Employee was evaluated at doctor and given restricted work and missed workdays.

Mechanic was working on a hose on a haul truck. While using an impact wrench the socket came off striking them in the mouth chipping their front tooth.

Operator was performing a pre shift inspection. While looking up some dirt or dust came down and got in Right eye causing a scratched Cornea. Employee was given prescription medication and missed two shifts of work.

Employee was operating a haul truck being loaded by a shovel, there was some large material and jarred the truck and the operator's back. The operator reported it and did not want to see a doctor at the time and on 12/27/2022 wanted to be seen as they felt it was not getting better. Operator was given restricted duty and prescribed a medication, operator has missed 0 days.

Driller assistant was in the process of tripping drill rods out of the hole. The EE placed the table wrench & break-out wrench in position as per standard procedure. This is when EE realized it was the last of the string. EE reached to remove the breakout wrench & this is when EE's middle finger was pinched between the table wrench & breakout wrench.

Employee was working in the ADR-1 cutting a zip tie with a fixed blade razor knife. It resulted in a laceration on the top of employee's thumb requiring sutures and restricted work.

Employee was on the raised portion of deck on haul truck opening door to get bags out, in doing so they rolled their ankle. Employee was evaluated by MRT and taken to be seen at workman's comp doctor where employee was diagnosed with a sprained ankle. Employee released full duty but has to wear an ankle brace as well as had 6 pt appointment's.

Employee was grinding and cutting in a shoot when EE received steel in EE's eye. Employee was wearing safety glasses and welding hood. Employee had to have the steel removed and drilled by medical professionals antibiotics where prescribed to avoid infection.

Fall from machine

Employee was stepping off a carbon truck when the employee missed or slipped on a rung of the ladder falling off the truck and landing on side. The employee suffered a nondisplaced fracture of the left hip. Employee was given restrictions and put off work.

Employee was getting into a confine space water tank when employee twisted and felt a pop in back followed by a little pain. IP was evaluated at a clinic and given restrictions, PT, and prescribed Lidocaine patches for pain.

Employee was removing an angle Iron brace when the weight shifted smashing the employees Left hand and fingers between a I beam. This resulted in lacerations to fingers requiring sutures as well as fractured fingers. Employee was given restricted work. Medication where prescribed as well

Operator tripped on sidewalk at the Ironclad falling and hitting face on handrail. Dry conditions and was at the end of shift. Employee received sutures for laceration.

A mechanic was installing a new dump body on a haul truck. During the process, the dump body shifted downward, trapping the hand between the bed pin and the object detection box, puncturing the center of the employees left hand. The puncture was in the center of the mechanics hand, penetrating through the thickness of the hand.

Maintenance was removing a brake valve in the center joint on shovel 425. Employees lowered the valve down on the frame pedestal, and disconnected the come-along to adjust the rigging. While re-hooking the straps the valve fell to the ground. The hoses caused it to spring back, contacted the ladder below EE's feet. The sudden movement caused a strain to EE's right shoulder.

EE received a lacerated forehead when contact with the bumper after being struck in the back of head with the tailgate of a pipe truck while bent over picking up a nut previously dropped.

EE working in the weld shop twisted right knee when walking across floor. EE was seen at ER where they were given work restrictions and has missed shifts. EE had reported to others at the start of shift the knee was sore.

At approximately 2:30 pm the crew were in the process of tripping out the drill pipe, the removed rods were being placed in a rod basket, the driller helper placed a rod in the basket and in doing so the rod struck the end of the basket bouncing back toward the driller helper and caught the helper's hand between the end of the rod and the rod basket.

IP was working in the HG Mill moving a counter weight manually, the counter weight shifted unexpectedly and pinched the IP's right ring finger between the counterweight & its storage rack. This resulted in a small laceration requiring 3 stitches and the IP returning to work with restrictions.

IP initially reported aggravation of shoulder injury from 2011, upon learning that the claim could not be reopened, the IP reported having shoulder discomfort, which began approx. April of 2019, once the discomfort was reported to the company on 9/14/2020; a physician saw the employee, & prescribed restrictions, the employee has returned to work, under these restrictions.

Prior to start of shift on 9/9/20 IP was starting P198 in the North Employee Parking lot, upon exiting the cab of the truck and felt a sharp pain in back. IP refused to manage the injury through worker's compensation and went to personal physician. The IP's doctor did not feel the employee was able to return to work full duty.

A 793F haul truck was being loaded by a 5500 shovel on the shovel's offside. While swinging into the bed of the truck, a rock rolled off the outside edge of the bucket and fell on the dovetail of the truck. This caused the bed to raise and settle back onto the frame. In the process of the bed moving, the IP was unsettled in the cab and ended up chipping their tooth.

IP was aligning a new electric pump motor utilizing a forklift for installation. During the process the motor unexpectedly dropped and struck the employee in the leg. As a result the IP experienced fractures to the left lower leg.

Strong winds were blowing carbon flakes around. One hit the associate in the left eye when EE was laying drip lines with the pad crew.

The haul truck operator was dismounting the truck at the fuel island. When the operator reached the bottom step of the ladder and stepped to the ground, the ladder was higher than anticipated. The operator strained the right leg upon impact. This leg was previously injured at home just days prior. The operator was placed on restrictions until the follow up appointment.

Around 10:40 the employee felt knee pop and pain radiated to EE's heel while climbing up the back ladder of vehicle 5049. EE continued up the ladder to finish loading the truck with emulsion. EE climbed down the ladder and went to the cab to download shot data. EE worked around the shop and office. At 1:00 EE reported the injury and went to the doctor.

Operator entered cab of equipment, closing the door with the right hand and the door made contact with the left hand. Clinic confirmed no fracture. Diagnosed bruised tendon. Referral of three physical therapy treatments.

Employee got windblown dirt in R eye. Washed dirt out with eyewash but eye was very irritated so employee wanted to go to clinic. Clinic confirmed no dirt in eye but prescribed antibiotic eyedrops to prevent infection from developing.

Employee states that employee strained shoulder while stepping from bottom rung of haul truck ladder to the ground. Employee further states employee was facing the ladder and maintaining 3 points of contact. Occ med placed employee on restricted duty for 4 days until MRI concluded no serious injury.

Operator of a production drill was manipulating the air lever while changing the steel to blow loose material out. While pulling the lever EE felt a pop between the index and middle fingers around the knuckles. EE given restrictions of no use of right hand.

EE was in the process of transporting piping for leach operations when they caught their left 2nd finger between pipe and back of loader. Initial clinic visit revealed no fracture and EE returned to full duties. On a follow-up visit on 5/1/19 the PA prescribed 2 physical therapy sessions when EE reported finger felt stiff.

EE was working to replace a leaking gasket on a coolant line when residual pressure caused the line to separate resulting in some coolant entering the EE's eyes. EE requested a medical evaluation during which time was prescribed an antibiotic to prevent possible onset of infection.

IP reported on 3/20 that IP had been experiencing discomfort in lower back for the last several weeks while performing routine tasks in the lab. IP was seen by provider, prescribed physical therapy and given a list of restrictions. Restrictions will not be accommodated.

Performing filter press trouble shooting and was bending down to walk under structure when at same time pry bar caught grating and then contacted nose and upper lip resulting in lacerations requiring sutures to close.

IP was mounting dozer and was pulling self up by the hand hold when EE felt a ""pop"" in EE's right elbow. MRI revealed partial thickness tear of radial bicep tendon. Cannot accommodate restrictions with regular tasks resulting in a lost day injury.

EE was cleaning hydraulic fittings underneath a haul truck when ee lost balance and placed ee right hand out to catch self. In the process ee hand got caught between a hydraulic line and the truck frame. This resulted in a wrist and shoulder strain. When evaluated the EE was not released to return to work.

EE was removing engine doors from a CAT 16M Grader. Handed door down to another employee and felt pain in back.

EE was working on placing concrete and never noticed that EE's pants at the Knee level became saturated with concrete, resulting in concrete burns to EE's Left and Right Knees.

In the process of lifting a sling the IE felt a ""pop"" in the stomach. During medical evaluation the IE was diagnosed with a hernia around the belly button. It has yet to be determined if surgery will be required. The IE was placed on light duty restrictions.

Employee began to experience irritation in the right eye but did not recall a specific event to cause the irritation other than inadvertently contacting a pipe with a bar causing dust to fall from the pipe. During evaluation IE was given prescription medication, taken off work for 1 day and then placed on restrictions.

Technician was removing impellas from assembly, supported with lifting straps and forklift, while positioning for removal, weight shifted causing extremely sharp impellar fin to contact hand with leather work glove on cutting left ring finger

Haul truck operator was traveling down ramp and when transitioned onto the working bench ee felt a ""pop"" in back. The IE continued to feel discomfort and asked to be evaluated by medical provider. As a result physical therapy and work restrictions were issued.

IE was in the process of performing a preshift inspection on the haul truck when IE walked through the park ditch and slipped and fell on backside. Later that day the IE complained of a sore shoulder and backside and was given a restricted duty release after being evaluated by medical provider.

Grader operator entered cab of grader, sat down in the seat and began to take their jacket off when they felt discomfort in their back. The operator was seen by a medical provider the following day and given work restrictions which could not be reasonably accommodated.

IE experienced a heart attack while inspecting a loader bucket GET which resulted in IE collapsing backwards and striking the back of head on the ground. This resulted in a laceration requiring sutures to close. As a result of the heart attack the employee has not returned to work.

Operator was dismounting from a haul truck, had right foot on the ground and when IE placed left foot on the ground stepped on a small rock which resulted in twisting IE left knee. IE experienced pain in left knee and when initial medical evaluation was completed was taken off work pending MRI.

In September of 2017 IE reported a left calf injury while mounting a 793 HT. At that time EE was diagnosed with cellulitis or a skin infection which was not attributed to the injury. The IE has continued to have pain and tightness in EE's calf and on 5/17/2018 was prescribed a treatment of PT for peroneal tendonitis. There is no additional event since the injury in 2017.

While handling a rubber ceramic liner, employee pinched employee's finger between the liner and the chute wall. As a result the employee received a laceration to employee's left ring finger requiring sutures to close.

A production drill was drilling over an unseen void when the drill fell partially into the void. As the extent of the void was unknown at the time the operator remained in the cab until exit could be assisted by mine rescue personnel.

IE was moving an empty garbage bin into place and felt a pain in abdomen. Was diagnosed with a unilateral inguinal hernia and released to restricted duties.

IE was in the process of putting a sling away when it twisted. Felt pain in shoulder which had been surgically repaired in the past. As a precaution IE requested to be seen at ER. No diagnosis given to date, however, employee placed on restrictions.

On 05/03/2017 IE reported that EE had been treated for a sore shoulder from EE's personal physician since February and that EE now thought it may be work related. IE received restrictions on 05/03. On 05/10/2017 IE received results of MRI showing a degenerative condition which could be a result of overuse. IE could not determine any single event which caused injury.

Employee was operating a C5 568 roller compacting 2:1 slopes traveling up near the top of the slope when the engine shut down. The roller began to slide down the slope uncontrollable and gaining speed and when the roller reached the bottom the roller over turned on to its side.

She reported to her shifter but the injury was not reported to Safety/ HR until 3/3/2015 when her wrist began to really hurt her. Up to that point she thought she was okay. She sent her to our medical provider on 3/4/2015

Employee was drilling a hole from the bottom up into a panel. His loose hand was too close to the drill bit and the slack in his glove got caught on the bit causing his hand to get pulled towards the bit. The employee's left middle finger was lacerated as a result.

The connector was hanging steel girders approx. 40' above the ground. The crane operator was swinging in a load girders. The worker had his back turned to the load, and due to poor communication between the worker, the other connector, and the crane operator, the load swung in and struck the back of his knee, which caused the front of it to hit the column in front of him.
