While the operator was operating the dozer they were backing up. They placed a hand on the arm rest when they went over a rock the arm rest failed causing a finger to dislocate

During Technical Ropes Training the employee was beginning to repel of the edge when their knee gave out.

Haul Truck shifted hard causing pain in EE's right shoulder.

Installing a starter in truck. When they finished the job they felt discomfort in their arm

While using an unapproved tool for personal use a laceration was sustained to right hand which resulted in medical treatment.

While employee was drilling a line broke under EE's feet filling up the cab with fumes. Employee seeked medical attention.

Employee was in the process of replacing panel when debris got in their eye.

Injury occurred on 12-04-24 and employee was evaluated by WC Dr. with no restrictions given. Employee was given restrictions one week later and employee missed scheduled shifts due to restrictions. A haul truck operator was being loaded by wheeled loader when a frost chunk was dumped into the bed. The operator complained of neck and shoulder soreness and headaches.

During Mine Rescue Training of the use of a highline system, member injured right elbow while pulling on the rope.

Employee stepped out of the back of a pick-up that was approximately 18 inches and rolled their ankle.

Operator was operating a dozer and haul truck during shift and started experiencing back soreness.

While riding in the passenger van to the equipment ready line, the van hit a bump bouncing the employee up hitting EE's head on the roof of the van causing an injury to EE's back.

Injury occurred 7-24-24 as a report only. Employee expressed that they weren't seeing any improvement requested to be seen 8-27-24. Employee continued to work scheduled shifts until 12-4-24. Scheduled surgery 12-5-24. Employee was air lancing in a chute at the crusher, got a sharp pain in their right shoulder

Process Maintenance Tech was walking on a road during a snowstorm then they slipped and when they landed, they ended up sitting on their right ankle. Update: EE did not miss any scheduled shifts nor given restricted duties. EE did not improve & requested to be re-evaluated. EE continued to work with no restrictions until 2-13-25 when surgery was performed to right ankle.

Maintenance Tech was reporting to work and slipped on ice which resulted in rolling right ankle.

Employee was holding a tagline for a generator being moved by an excavator. The generator swung and pulled the employee forward causing them to fall in front of the excavator track where their foot was caught between the track and the ground.

Loader operator was digging a bucket when the overhanging frost gave way causing a frost chunk to go over the trash rack striking the cab of the loader.

Driller pinched left pinky finger between the drill bit and drill hammer when they moved the hammer to apply a lifting bell. The hammer was positioned horizontal on a rack.

Driller was picking up sample bags from drill holes and felt a pop in right hip.

Maintenance Tech was maneuvering a hoist pump into place when their left thumb got pinched between the housing and a hydraulic hose resulting in a laceration.

While working in drive cabinet, the workers hand contacted a sharp wire-tie, resulting in a cut requiring 3 stitches to the top of middle finger on left hand.

Operator of light vehicle traveled through a dip too fast and caused the operator to raise in their seat and come down on their 2-way radio which was in their vest pocket.

Maintenance mechanic was using 4lb hammer to strike Allen Wrench with extension. When the extension was struck, the Allen wrench ""popped"" back contacting the technician's face resulting in a broken tooth.

Operator put valve on 2 inch water hose and turned on valve and pressure knocked employee off balance and fell and hit head and jammed finger.

Miner was walking from employee parking lot to report to shift and tripped and fell and caught fall with RT hand.

Operator was closing valve and felt back pain later that day.

IP was being trained in the use of a hose making machine when the trainer was demonstrating how to place dies. While in the process of closing down on the dies, an object struck the IP in the right shoulder.

Process operator was connecting water line to Hydrochloric Acid pump to flush pump for scheduled repairs. The PVC fitting broke and the operator was sprayed in the shoulder and face.

Haul truck operator was backing up to a shovel looking over right shoulder at mirror and hit a bump causing neck pain. Employee was evaluated at doctor and given restricted work and missed workdays.

Mechanic was working on a hose on a haul truck. While using an impact wrench the socket came off striking them in the mouth chipping their front tooth.

Operator was performing a pre shift inspection. While looking up some dirt or dust came down and got in Right eye causing a scratched Cornea. Employee was given prescription medication and missed two shifts of work.

PSES Operator was washing floors with water. There was some flock material on the ground and the operator did not see it slipping and falling injuring EE's right knee and left wrist.

Wash Bay employee was pulling some hoses when they slipped on some ice causing them to fall and injure elbow.

The employee ran into a door while trying to go through it hitting EE's face resulting in a chipped tooth.

Employee was operating a haul truck being loaded by a shovel, there was some large material and jarred the truck and the operator's back. The operator reported it and did not want to see a doctor at the time and on 12/27/2022 wanted to be seen as they felt it was not getting better. Operator was given restricted duty and prescribed a medication, operator has missed 0 days.

Driller assistant was in the process of tripping drill rods out of the hole. The EE placed the table wrench & break-out wrench in position as per standard procedure. This is when EE realized it was the last of the string. EE reached to remove the breakout wrench & this is when EE's middle finger was pinched between the table wrench & breakout wrench.

Employee was working in the ADR-1 cutting a zip tie with a fixed blade razor knife. It resulted in a laceration on the top of employee's thumb requiring sutures and restricted work.

Employee was on the raised portion of deck on haul truck opening door to get bags out, in doing so they rolled their ankle. Employee was evaluated by MRT and taken to be seen at workman's comp doctor where employee was diagnosed with a sprained ankle. Employee released full duty but has to wear an ankle brace as well as had 6 pt appointment's.

Employee was grinding and cutting in a shoot when EE received steel in EE's eye. Employee was wearing safety glasses and welding hood. Employee had to have the steel removed and drilled by medical professionals antibiotics where prescribed to avoid infection.

Employee was using a table grinder when tool caught and pinched finger

EE was walking on the sidewalk and slipped on ice, hurt their ankle.

Employee was stepping off a carbon truck when the employee missed or slipped on a rung of the ladder falling off the truck and landing on side. The employee suffered a nondisplaced fracture of the left hip. Employee was given restrictions and put off work.

Employee was getting into a confine space water tank when employee twisted and felt a pop in back followed by a little pain. IP was evaluated at a clinic and given restrictions, PT, and prescribed Lidocaine patches for pain.

Employee was removing an angle Iron brace when the weight shifted smashing the employees Left hand and fingers between a I beam. This resulted in lacerations to fingers requiring sutures as well as fractured fingers. Employee was given restricted work. Medication where prescribed as well

Operator tripped on sidewalk at the Ironclad falling and hitting face on handrail. Dry conditions and was at the end of shift. Employee received sutures for laceration.

A mechanic was installing a new dump body on a haul truck. During the process, the dump body shifted downward, trapping the hand between the bed pin and the object detection box, puncturing the center of the employees left hand. The puncture was in the center of the mechanics hand, penetrating through the thickness of the hand.

Maintenance was removing a brake valve in the center joint on shovel 425. Employees lowered the valve down on the frame pedestal, and disconnected the come-along to adjust the rigging. While re-hooking the straps the valve fell to the ground. The hoses caused it to spring back, contacted the ladder below EE's feet. The sudden movement caused a strain to EE's right shoulder.

An employee was driving a bulk truck down the 10% haul road in the snow to load holesa and hit a berm.

EE received a lacerated forehead when contact with the bumper after being struck in the back of head with the tailgate of a pipe truck while bent over picking up a nut previously dropped.

EE working in the weld shop twisted right knee when walking across floor. EE was seen at ER where they were given work restrictions and has missed shifts. EE had reported to others at the start of shift the knee was sore.

At approximately 2:30 pm the crew were in the process of tripping out the drill pipe, the removed rods were being placed in a rod basket, the driller helper placed a rod in the basket and in doing so the rod struck the end of the basket bouncing back toward the driller helper and caught the helper's hand between the end of the rod and the rod basket.

IP was working in the HG Mill moving a counter weight manually, the counter weight shifted unexpectedly and pinched the IP's right ring finger between the counterweight & its storage rack. This resulted in a small laceration requiring 3 stitches and the IP returning to work with restrictions.

IP initially reported aggravation of shoulder injury from 2011, upon learning that the claim could not be reopened, the IP reported having shoulder discomfort, which began approx. April of 2019, once the discomfort was reported to the company on 9/14/2020; a physician saw the employee, & prescribed restrictions, the employee has returned to work, under these restrictions.

Prior to start of shift on 9/9/20 IP was starting P198 in the North Employee Parking lot, upon exiting the cab of the truck and felt a sharp pain in back. IP refused to manage the injury through worker's compensation and went to personal physician. The IP's doctor did not feel the employee was able to return to work full duty.

A 793F haul truck was being loaded by a 5500 shovel on the shovel's offside. While swinging into the bed of the truck, a rock rolled off the outside edge of the bucket and fell on the dovetail of the truck. This caused the bed to raise and settle back onto the frame. In the process of the bed moving, the IP was unsettled in the cab and ended up chipping their tooth.

IP was aligning a new electric pump motor utilizing a forklift for installation. During the process the motor unexpectedly dropped and struck the employee in the leg. As a result the IP experienced fractures to the left lower leg.

Strong winds were blowing carbon flakes around. One hit the associate in the left eye when EE was laying drip lines with the pad crew.

The haul truck operator was dismounting the truck at the fuel island. When the operator reached the bottom step of the ladder and stepped to the ground, the ladder was higher than anticipated. The operator strained the right leg upon impact. This leg was previously injured at home just days prior. The operator was placed on restrictions until the follow up appointment.

Around 10:40 the employee felt knee pop and pain radiated to EE's heel while climbing up the back ladder of vehicle 5049. EE continued up the ladder to finish loading the truck with emulsion. EE climbed down the ladder and went to the cab to download shot data. EE worked around the shop and office. At 1:00 EE reported the injury and went to the doctor.

Operator entered cab of equipment, closing the door with the right hand and the door made contact with the left hand. Clinic confirmed no fracture. Diagnosed bruised tendon. Referral of three physical therapy treatments.

Employee got windblown dirt in R eye. Washed dirt out with eyewash but eye was very irritated so employee wanted to go to clinic. Clinic confirmed no dirt in eye but prescribed antibiotic eyedrops to prevent infection from developing.

Employee states that employee strained shoulder while stepping from bottom rung of haul truck ladder to the ground. Employee further states employee was facing the ladder and maintaining 3 points of contact. Occ med placed employee on restricted duty for 4 days until MRI concluded no serious injury.

Operator of a production drill was manipulating the air lever while changing the steel to blow loose material out. While pulling the lever EE felt a pop between the index and middle fingers around the knuckles. EE given restrictions of no use of right hand.

EE was in the process of transporting piping for leach operations when they caught their left 2nd finger between pipe and back of loader. Initial clinic visit revealed no fracture and EE returned to full duties. On a follow-up visit on 5/1/19 the PA prescribed 2 physical therapy sessions when EE reported finger felt stiff.

EE was working to replace a leaking gasket on a coolant line when residual pressure caused the line to separate resulting in some coolant entering the EE's eyes. EE requested a medical evaluation during which time was prescribed an antibiotic to prevent possible onset of infection.

IP reported on 3/20 that IP had been experiencing discomfort in lower back for the last several weeks while performing routine tasks in the lab. IP was seen by provider, prescribed physical therapy and given a list of restrictions. Restrictions will not be accommodated.

Performing filter press trouble shooting and was bending down to walk under structure when at same time pry bar caught grating and then contacted nose and upper lip resulting in lacerations requiring sutures to close.

IP was mounting dozer and was pulling self up by the hand hold when EE felt a ""pop"" in EE's right elbow. MRI revealed partial thickness tear of radial bicep tendon. Cannot accommodate restrictions with regular tasks resulting in a lost day injury.

EE was cleaning hydraulic fittings underneath a haul truck when ee lost balance and placed ee right hand out to catch self. In the process ee hand got caught between a hydraulic line and the truck frame. This resulted in a wrist and shoulder strain. When evaluated the EE was not released to return to work.

EE was removing engine doors from a CAT 16M Grader. Handed door down to another employee and felt pain in back.

EE was working on placing concrete and never noticed that EE's pants at the Knee level became saturated with concrete, resulting in concrete burns to EE's Left and Right Knees.

In the process of lifting a sling the IE felt a ""pop"" in the stomach. During medical evaluation the IE was diagnosed with a hernia around the belly button. It has yet to be determined if surgery will be required. The IE was placed on light duty restrictions.

Employee began to experience irritation in the right eye but did not recall a specific event to cause the irritation other than inadvertently contacting a pipe with a bar causing dust to fall from the pipe. During evaluation IE was given prescription medication, taken off work for 1 day and then placed on restrictions.

Technician was removing impellas from assembly, supported with lifting straps and forklift, while positioning for removal, weight shifted causing extremely sharp impellar fin to contact hand with leather work glove on cutting left ring finger

Haul truck operator was traveling down ramp and when transitioned onto the working bench ee felt a ""pop"" in back. The IE continued to feel discomfort and asked to be evaluated by medical provider. As a result physical therapy and work restrictions were issued.

IE was in the process of performing a preshift inspection on the haul truck when IE walked through the park ditch and slipped and fell on backside. Later that day the IE complained of a sore shoulder and backside and was given a restricted duty release after being evaluated by medical provider.

Grader operator entered cab of grader, sat down in the seat and began to take their jacket off when they felt discomfort in their back. The operator was seen by a medical provider the following day and given work restrictions which could not be reasonably accommodated.

IE experienced a heart attack while inspecting a loader bucket GET which resulted in IE collapsing backwards and striking the back of head on the ground. This resulted in a laceration requiring sutures to close. As a result of the heart attack the employee has not returned to work.

Operator was dismounting from a haul truck, had right foot on the ground and when IE placed left foot on the ground stepped on a small rock which resulted in twisting IE left knee. IE experienced pain in left knee and when initial medical evaluation was completed was taken off work pending MRI.

In September of 2017 IE reported a left calf injury while mounting a 793 HT. At that time EE was diagnosed with cellulitis or a skin infection which was not attributed to the injury. The IE has continued to have pain and tightness in EE's calf and on 5/17/2018 was prescribed a treatment of PT for peroneal tendonitis. There is no additional event since the injury in 2017.

While handling a rubber ceramic liner, employee pinched employee's finger between the liner and the chute wall. As a result the employee received a laceration to employee's left ring finger requiring sutures to close.

A production drill was drilling over an unseen void when the drill fell partially into the void. As the extent of the void was unknown at the time the operator remained in the cab until exit could be assisted by mine rescue personnel.

IE was moving an empty garbage bin into place and felt a pain in abdomen. Was diagnosed with a unilateral inguinal hernia and released to restricted duties.

IE was in the process of putting a sling away when it twisted. Felt pain in shoulder which had been surgically repaired in the past. As a precaution IE requested to be seen at ER. No diagnosis given to date, however, employee placed on restrictions.

On 05/03/2017 IE reported that EE had been treated for a sore shoulder from EE's personal physician since February and that EE now thought it may be work related. IE received restrictions on 05/03. On 05/10/2017 IE received results of MRI showing a degenerative condition which could be a result of overuse. IE could not determine any single event which caused injury.

Slipped over pipe ee was working on and through out hand to break fall

Icy Conditions. Employee slipped and fell on ice.

Driver pinched finger under chain while securing a load on trailer. Helper was on one side of truck securing binder while driver was on the opposite side straightening a chain.

Employee was operating a drill when the bit twisted employee's wrist

Technician was repairing air pipe in mill. Due to weakness in shoulder, when residual air from line came out, arm dislocated from the socket. This is the 2nd time this has happened in 2 months. Technician was taken to hospital to have arm reduced into place.

While descending a ladder off the pipe truck the employee stepped on a rock and hyper-extended left knee.

Installing pipe in mill. Employee holding pipe overhead, stepped forward to adjust strop, right shoulder popped out of joint.

Employee twisted leg and resulted in EE falling and hitting EE's knee on a rock. Causing contusion and bruising to knee.

Operator was coming down from HT and EE's right foot slipped off the rung of the flexible ladder and EE fell to the ground, landing on EE's right foot.

Employee was driving up the ramp and began to feel light-headed and dizzy. EE stopped truck and called supervisor and got out of truck until help could arrive

Climbing off left tandem on 16M, foot slipped on catwalk, hand slipped and catwalk cut glove and right ring finger requiring stitches.

Driving was dismounting haul truck and the ladder began to wobble causing his knee to buckle. Grabbing the handrail to prevent falling he strained his lft shoulder

Sprained wrist while shoveling out filter press

Haul Truck Driver backed into the Bucket of Loader causing the truck to come to an abrupt stop jarring the drivers back.

As concrete was being pumped to area for placement, concrete was splashed into the right eye causing a corneal abrasion.

Snow pack conditions employee slipped twisting his foot fell and broke fibula above right ankle.

Stepping off of ladder and missed bottom step causing foot to roll ankle resulting in sprain.

Mechanic was working on Rubber Tire Dozer coming down he missed a step and started to fall. He caught his arm in the handrail and caused a separation in his shoulder

Employee was operating a C5 568 roller compacting 2:1 slopes traveling up near the top of the slope when the engine shut down. The roller began to slide down the slope uncontrollable and gaining speed and when the roller reached the bottom the roller over turned on to its side.

Employee was removing a lower roller bolt, and when they sat the bolt on the ground the sleeve fell. Catching the employee's right index finger between the bolt head and sleeve. Causing a tuft fracture, two stiches, and prescription medication.

Smashed Two Fingers on Left Hand While Replacing on/off Valve on Flow Blocking Device Pipe slipped and smashed his between the flanges

Checking filters in Lime Silo. Pressure caused lime dust to blow in his eyes

Employee was removing zip-ties off equipment with a pocket knife and cut his finger. The employee was given first aid and released to return to work. On June 1, 2015 his finger began to draw up. He was taken to the Dr. and surgery was required to repair cut tendon

Driver strained wrist holding onto steering wheel trying to brace himself in bad road conditions

Driving Lube truck jarred his back while driving in pit because of rough conditions due to weather conditions

Team Member was welding pipe on the pad & during the process the welding head shifted and smashed his right hand.

Slipped on ice and snow threw her arm out to protect her fall fracturing right wrist

Driving snow plow truck. Blade caught on something in road and broke. The impact threw the driver into the steering cutting his nose

Leak from tank containing caustic material; tank was being rinsed out for repairs. Some of the water got on the pants leg of employee burning the calves of both his legs. This incident has been rated as a LTI

Slipped on ice hurt his shoulder Employee did not report this incident until Sunday, February 22

Slid hand down handle of squeegee & caught a splinter in his hand.

Frost chunk hit his truck while being loaded and jarred the operator around inside his truck.

Employee cut her thumb using a hook blade to splice some cable

Employee was cleaning lime from chute and debris got behind his safety glasses into his eye.

Employee Syncope and Collapsed

While adjusting forks on forklift he bumped his leg on the edge of fork cutting his leg.

Employee was burying side slope on the 10,000 bench when she was disconnecting the eyelet of a bobcat from the winch and she tweaked her wrist. She reported to her shifter but the injury was not reported to Safety/ HR until 3/3/2015 when her wrist began to really hurt her. Up to that point she thought she was okay. She sent her to our medical provider on 3/4/2015

Employee was climbing up a scaffold ladder and when he looked up got some dirt in his eye. Employee had his eye flushed out on site but it still hurt. Went to physician and determined a scratch on his eye. A prescription eye drop was given to him.

Laceration to left hand while cutting piece of metal

Employee was lifting trash bags from the bed of a pickup truck to a dumpster when employee felt some back discomfort. At the time, the employee reported that she did not need medical attention. On 9/15/14 the employee requested medical attention.

Employee was drilling a hole from the bottom up into a panel. His loose hand was too close to the drill bit and the slack in his glove got caught on the bit causing his hand to get pulled towards the bit. The employee's left middle finger was lacerated as a result.

Clearing obstruction from chute . A rock fell on floor& he bent to pick it up. On standing he bumped his head on apron door and hit his lip on the rock chipping a tooth

The connector was hanging steel girders approx. 40' above the ground. The crane operator was swinging in a load girders. The worker had his back turned to the load, and due to poor communication between the worker, the other connector, and the crane operator, the load swung in and struck the back of his knee, which caused the front of it to hit the column in front of him.

Employee was driving Haul Truck and felt a pop in his right shoulder.

On 5/9/2014 employee reported that she twisted her knee on 5/5/2014. She did not report this event to personnel when it occurred as required by company policy. We have investigated this accident and do not believe this event constitutes a workplace injury. Brahma only files this 7000-1 out of an abundance of caution in order to preclude MSHA enforcement.

While working the affected employee was struck by a piece of falling conduit that was not secured properly from a distance of approx. 12'

Employee was shaking out steel to load on forklift. Employee attempted to lift a heavy piece of steel approx 400 lbs as he reached down to lift & turn employee heard a popping sound in his lower back. *Note-Employee was diagnosed w/lower back fracture on 5/13/14 MRI

A grating covers a concrete tank. When employee walked across the grating one section (approx. 3'x5') slid and fell into the tank. He also fell in the tank from 17' on to the concrete below. He fell on to his right side and fractured 7 ribs, his elbow and shoulder.

Employee's were tripping out PVC when the under reamer slipped. Hitting the employee in that back of the arm. First report was employee hyperextended rt elbow.

Employee was working off a 4' ladder tightening & replacing a hydraulic line with a 1"" 7/16 wrench. Right hand was being used to tighten the line while left hand was being used to support position. Employee states that as he was tightening the line he felt a twitch in left shoulder & heard a pop, he then climbed down ladder & realized his L shoulder was dislocated.

EE was removing grating clips when a piece struck him in the mouth causing lip laceration requiring suture.

Leaving his shift for the day, employee stepped down form the steps onto solid ground and felt his knee give or twist. He went home thinking he had strained the knee. His knee began to hurt and swell. He came in the next day and reported to his supervisor and was referred to our health care provider. No rocks or other material was found on the solid surface

Employee was lifting LN Welding machine up with rope from 1st level of scaffolding. Welder got hung up on scaffolding, he pushed welder away while pulling up on rope & felt pain in lower back

Employee was starting to climb a vertical ladder when he slipped on first step and fell 2' down and landed on his right knee on top of strong back causing a laceration on his knee

EE conducting a pre-inspection walk around when a chunk of mud fell on top of his head, went to hospital, and has to go to physical therapy.

Employee standing on drill table slipped and fell backwards hitting his head.

Upward motion with concrete hose. Attempting to empty out the hose before concrete set-up. Approx weight of section of hose 75-100 lbs - Employee jerked hose up from ground position causing a sudden pain in his shoulder. Employee heard popping sound from shoulder.

Slip & fall. Initial incident - from snow packed over. Employee fell on right elbow straining elbow. Employee continued to work. Elbow became more painful from the repetitive motions of the work causing more strain to elbow.

EE was walking between work locations, slipped on ice and strained his left knee

EE was cleaning out a service truck, while emptying the trash he cut his hand on some glass

EE was performing a pre-walk around on his Haul Truck, he slipped on the ice, his legs went separate ways and he broke his right leg in two places.

EE was getting his haul truck loaded when a large rock hit the bed of the truck, jarring his back sending pain and tingling down his left leg

Crews were working on form setting using hard tools (hammer) to place 2x4's between forms. Employee was driving 2x4 into place when injured employee walked behind employee swinging hammer. Employee was not aware of other swinging hammer and was hit on face by back swing.

EE was attempting to remove slack out of a hoist chain on a drill, he pinched his right middle finger, amputating the end of his finger. He was taken to the ER and they sewed his tip back on.

During stretch and flex activity EE over stretched and experienced a sharp pain in groin area.

Removing a boom pin from a shovel. Forklift was used to straighten the boom. A come along was used to keep the tension on the pin straight. When the boom straightened out, the pin came out and struck the mechanic on the foot, causing a broken tibia

EE was using a pry bar to help guide a 12"" header pipe, it slipped and hit him in the left leg. He was transported to ER they took X-ray's and nothing was broken. On 10/14/2013 he called and needed to see an authorized physician. Possible small fracture to foot. Needs some physical therapy.

We were washing the steel. We finished 1 section and boomed all the in. I then started turning the boom to wash the next column. Once I started turning I felt the lift starting to tip, we hit the column.

Washing the columns, the boom was extended so boomed all the way in. Once the boom was all the way in we started rotating the boom to get closer to the wall. At the moment of the rotation we were falling towards column.

Both EE wrists hurt while driving the haul truck. This has been going on for some time now, but has not reported it until 8/26/13

Mechanic was climbing down ladder, missed a step and fell 2-3 feet on his foot and broke his ankle

EE was performing general clean up work around crusher plant. EE proceeded to climb to upper level of plant to continue cleaning off cat walk. As EE completed his duties he proceeded to exit the cat walk by descending the stationary ladder. As he was descending he experienced an episode of dizziness & lite headiness causing him to lose his grip & falling to the ground. EE fell 6'

While operators Haul Truck was being loaded with rock, it jarred the truck causing discomfort in neck and back.

While installing a Crusher sheave taper lock bushing, hitting the hammer and it ""bounced"" back and fracturing his nose.

Exiting haul truck (740) employee stepped on the lower rung & lost balance causing him to fall backward off of the step. Falling back onto a 3' - 4' rock. Employee suffered a blow to his lower back.

Walking up slope with spooler slipped on surface covered w/dust. Lost balance tried to catch himself with spooler, all of his weight was on spooler/outstretched right arm causing dislocation of shoulder.

Wet / muddy / slippery area. Employee was navigating on access ramp to pump house when he slipped and fell hitting his right shoulder on the ground.

EE states that 2 weeks prior that she is having pain in her elbow from mopping, sweeping, and lifting. Sent to medical provider, and she is receiving physical therapy with no restrictions at this time.

EE was descending a ladder when he stepped onto the floor, his left knee twisted and he felt a pop.

EE fell walking up front steps, falling on right hand, and hurting right shoulder. Did not go to doctor until March 1, 2013

Operator was exiting a haul truck when he missed a step and fell from the machine. Outside temperature was -4 degrees, causing frost on the handrails making them slippery. Operator stated that his wrist was hurt.

Operator was backing up a ramp in his blade, bounced over a rock and jammed his lower back. We sent him to an authorized provider on 1/28/13. He tried to control the pain with ibuprofen for the past couple of weeks. He is receiving physical therapy a couple times a week.

EE was rolling up a hydraulic hose, the hose jerked resulting in pulling muscles, in shoulder, chest and thoracic lumbar

Haul Truck was being loaded with bigger rocks causing the truck to be jarred. This caused the driver to jamb his lower back and has numbness in left leg.

Mechanic was using a crane to reinstall a track on a dozer, he got his finger smashed between the chain and the blade tilt cylinder, causing trauma to all 4 fingers on left hand.

EE was shoveling rocks, when he felt a pinching pain in his lower back. Originally he thought maybe a hernia, dr stated strain in lower back and prescribed physical therapy.

While EE was cleaning a plugged chute, with falling rock, he stepped back onto a falling rock, spraining his right ankle.

EE got left ring finger caught in a door that closed on her, she ended up with a couple of stitches

EE was cutting chain link, material slipped causing hands to come together. Hit rt middle finger again, hit this finger on 10/25/12 He had to go to the dr to have it drained on 10/31/12

EE doing repetitive motion causing inflamed neck, pain through arm too. EE did not report this until 11/19/12, claiming this is from a previous injury. EE is going to physical therapy for this injury. Logs core and looks through microscope

mechanic was removing a hyd line, leaning over a drill in an awkward position. While pulling he felt a sharp pain in left lower back, and pain went down his leg to his knee. Notified employer on 9/9/12 did not go to medical provider 9/10/12. He went home and did some stretches, ee stated he felt better on next day 9/9/12 sent for physical therapy

DRILLER HELPER WAS STANDING ON THE FOOT CLAMP AND THEN THE DRILLER LOWERED THE HEAD WITH THE ROD SHOOT ON IT AND THE HEAD CAME DOWN ON THE EMPLOYEES FOOT.

He reported on 8/13/12 still waiting info from doctors...EE hit rt knee on ladder of haul truck then noticed swelling days later, went to personal dr. and then to ER. On the 13th we sent him to our provider once he notified us.

A valve on a caustic pipe failed and sprayed the employee in the face with liquid caustic soda. A small burn to the right eye occurred and was treated by the authorized provider with antibiotic ointment.

EE was stepping out of his service truck, and stepped on a rock, rolling his left ankle

EE was shoveling at the tail of the Load out Bin on his knees. After 15-20 minutes he stood up and felt a pop in his right knee.

EE was installing brackets on underside of LOB deck and was in a boom truck while performing this task. The bucket sprung forward causing EE to hit his head on a support beam, causing a compression fracture of several vertebrae of the thoracic spine.

An equipment operator using his pocket knife to remove a tie-wrap holding a lamp on his hard hat when the knife slipped and lacerated his left thumb. He received 4 stitches at Pikes Peak Hospital Emergency Room.

EE stepped out of dozer onto a rock and twisted his left ankle. He did not notify us until 12/22/11

I was working on the crusher waiting to seal unit, cleaning up unit. Pulled magnet off, assumed that the magnet was not magnetized, when I walked by the magnet with a sprocket, it pulled my hand against it and smashed my left index finger.

While installing a ped pivot pin in a Euclid dump truck, pushing the pin, it slid in all at once trapping right fore finger between the pin cap and pin bus.

Lab Tech was lifting blast holes out of rack (approx 25 lbs) and hurt her left arm.

Lab employee was pulling a pan out of the mill and her hand started to hurt.

EE was attaching chain binders onto pipe tub, with forward momentum his hand slipped and binder was not fully locked so binder bounced back and the handle of the binder struck the EE on the forehead above the left eye.

EE was driving a haul truck with the windows down and some dust blew in his eye causing a cornea abrasion. He notified us two days later and went to provider, they gave him medicated eye drops. He had no days lost.

Operating an IT to stem blast holes, felt a sprain in shoulder to spine

Employee claims that his wrist (right) is strained from over use/Ganglion Cyst

The bucket on a Haul Truck fell down, causing the driver to hit is head on the steering wheel. The driver went to ER, they did a cat scan and he has some strain on the cervical spine. No days lost.

Employee was pulling on a pallet jack that had sample racks on it and felt her shoulder pop. Did not report it for a few days. She is going to physical therapy, and missing no work. No lost time.

A relief driller was lifting drill cutting samples into a rack on a sample truck, and felt a pain in his shoulder

Employee was picking up cones around edge of shot muck piles, fell, resulting in a twisted right knee. This was a medical treatment until 5/31/11 when she had surgery. This was not reportable until 5/31/11

Operator was walking out to get a sample from the barren header when he slipped on ice. While trying to catch himself his right leg locked straight, causing strain to his knee.

Employee was carrying a 5-gallon bucket of silica when she felt a pain in her low back and left hip.
