Alleged back strain while changing 13lb bags in dust collector. Miner worked entire shift and 2 scheduled hours next day, did not report any injury or pain. Called leadman to report back pain on Sunday, 02/15/2009, at 2:30pm. Produced emergency room release on Tuesday, 02/17/2009, showing treatment with no recommended restrictions or missed work days.

Truck driver entered mine property to pick-up load of material. While traveling from truck to scale house stepped through piece of floor grating and lacerated inside of left leg. Driver received ~7 stiches to close laceration. Driver returned to truck that night. Spent night in his truck, and left property @ ~8:00am on 04/26/07.

Employee was stepping down of the bottom step of the 970F endloader, which is approximately 18 inches from the ground. Employee was trying to reach for a concrete wall with his foot which was about 3 feet back from the endloader. Employee failed to reach the concrete wall which caused him to fall backwards and the middle of his back striking the concrete wall.
The 6 x 9 slide gate was pulled out all the way & it fell down, smashing the tip of the employee's right ring finger.
Railcar unloader connveyor started to plug. Employee decided to clear blockage. One employee went to turn off power, other employee started to remove guard. While lifting guard, employee lost balance & stepped on belt which jumped, sucking his right foot into take up tension pulley.

The ee was removing rubber from an impeller. He was using pliers to remove the lining when the pillars slipped causing the ee to strike the lower impeller. The impact on the impeller caused a laceration on the ee's wrist.

THE EE WAS WORKING IN THE BAGGING AREA. HE WAS AT THE PALLET CONVEYOR AND WENT TO BEND OVER & GRAB THE HAND CRANK TO MOVE THE PALLET AND FELT SOMETHING POP IN HIS BACK WHEN HE BENT OVER.

HE HAD BEEN REMOVING PALLETS FROM THE STRETCH WRAP MACHINE AND PUTTING THEM INTO STORAGE. HE WENT TO LINE UP THE MACHINE WHEN HE WALKED BETWEEN THE MACHINE AND PALLET CONVEYER. WHEN IT WAS ACT IVATED IT PINNED THE LOWER PART OF HIS LEG BETWEEN THE PALLET AND THE FRAME OF THE MACHINE. LOWER PART OF HIS LEG BEGAN TO SWELL UP.
THE EMPLOYEE WAS STACKING SAND BAGS AND MISSED ARRANGED ORDER, SO THE PALLET NEEDED TO BE RESTACKED. HE PULLED ANOTHER PALLET OUT OF THE MAGAZINE RACK AND LEANED IT UP AGAINST IT AND WENT TO THE OTHER SIDE OF THE PALLETIZER. WHILE GETTING THE PALLET HE SLIPPED AND FELL AND THE PALLET HIT HIM IN THE FACE AND THE SHOULDER ON THE LEFT SIDE.

THE EE WAS PLACING A PIECE OF METAL GRATING ON A CYCLONE TOWER WHEN IT SLIPPED BETWEEN THE FRAMEWORK IT WAS TO LAY ON, AND BECAME JAMMED. WHEN HE TRIED TO PULL IT OUT FEL AND HEARD A POP IN HI S LOWER BACK.
HE WAS TAKING OFF THREE PIPE END CAPS AND WAS STANDING ON EXTENSION LADDER TO REACH THE ENDCAP EITH A PIPE WRENCH. HE PULLED DOWN ON THE WRENCHAND IT SLIPED OFF THE LADDER AND CUT HIS LEFT TH UMB. HE NEEDED SUTURES.
HE WAS GOING TO UNLOAD A 10 FEET METAL SIGN OST THAT WAS FASTEN TO THE LADDER RACK ON THE PICKUP TRUCK. HE STEPPED UP ON THE TAILGATE OF THE TRUCK AND STOOD UP AND HIT HIS HEAD ON THE SIGN POS T THAT GAVE HIM A SMALL LACERATION.
