Employee was preparing a conveyor for transport, when EE's thumb was caught in between the folding legs.

Miner was unloading material alone and was found unresponsive. After being on life support for one week, miner passed away from a brain aneurysm. We didn't feel that the aneurysm was a reportable event/injury but we are submitting the form now out of an abundance of caution.

Employee was cutting a belt when the razor blade contacted his right thigh.

Employee was changing oil in the Jaw Feeder. When he pulled the plug, the oil splashed on to the right hand glove, soaked through the glove causing a 2nd degree burn.

EE WAS PULLING ON CONTROL PANEL FOR CRUSHER. AS IT WAS LIFTED WITH A CHAIN -EE HAD TO PULL THE BOTTOM TO STAND IT UP. FELT SHARP PAIN IN RIGHT ARM (ELBOW)

EMPLOYEE WAS WORKING ON THE ELECTRICAL PANEL BOX ON THE CRUSHER SPREAD. THE WIRE CABLE WAS STUCK. HE PULLED THEM TO LOOSEN THE JAM. WHEN IT CAME LOOSE. EMPLOYEE LOST HIS BALANCE AND FELL FO RWARD HITTING HIS CHIN ON THE EDGE OF THE PANEL BOX CUTTING HIS CHIN.
