Fall from machine

Employee was completing pre-inspection of haul truck, as EE was climbing down, EE's boot treads got stuck on the step and EE lost footing and fell to the ground subsequently injuring wrist, fracture to wrist.

Employee was five to six feet away from another employee who was using air hammer to remove bearing when a piece of metal flew off and hit employee between hard hat and safety glasses. Metal cut forehead and small piece was in the skin.

While completing maintenance on a VSI Crusher and changing teeth, the miner twisted to grab parts and sprained their lower back.

While doing maintenance inside a screen, was cleaning side, holding in place while lying down. After 10-minutes he readjusted his position and straightened his leg and that is when he heard a crack. Was find day of with no pain but the next day it was hurting and swollen.

Employee changing wear parts on a rock crusher and trying to situate a hammer into a vertical shaft impact crusher when he felt a pop in his back. The next day (9-6-13) employee was replacing grease on the track shovel when his back began hurting much worse.

EE was thawing out a catch pan for a water pump. When he reached down to pick up a steel plate out of the catch pan it was still frozen into the catch pan. As ee lifted he strained his lower back (Lumbar Strain and Sacrum Strain).

HE CONTINUED ACROSS THE HAUL ROAD, OVER THE BE RM AND DOWN THE SIDE OF THE HILL. HE SUFFERED CONTUSIONS AND LACERATIONS. HE WAS HELD IN THE HOSPITAL FOR 3 DAYS BECAUSE OF DIZZINESS IN HIS HEAD AND FOR OBSERVATION.

EE WAS PULLING OUT THE LAST SCREEN CLOTH ON THE BOTTOM DECK OF THE SECOND VIBRATING SHAKER WHEN HE CAUGHT HIS FINGER ON THE END OF SCREEN. HIS COWORKERS PULLED THE SCREEN WHICH THEN FELL AND S LICED OPEN THE TIP OF HIS RING FINGER ON HIS LEFT HAND.

EE WAS WALKING UNDER A GUARD ON A CONVEYOR WHEN HE HIT HIS HEAD ON THE GUARD. HE THEN FELL TO THE GROUND. ABOUT 3 WEEKS LATER HE WENT TO SEE HIS PHYSICIAN BECAUSE OF PAIN IN HIS HEAD & NECK.
