Employee was completing pre-inspection of haul truck, as EE was climbing down, EE's boot treads got stuck on the step and EE lost footing and fell to the ground subsequently injuring wrist, fracture to wrist.

EE was removing the cutting edge bolds while using impact when a bolt slipped down and cut the employees fingers.

Employee was shoveling when EE felt pain in elbow area, muscle/forearm.

While working on cone crusher, employee was swinging sledge hammer, missed EE's target and hit EE's right pinky finger.

Climbing down ladder of equipment, as EE stepped to the ground EE stepped on a rock and lost footing and fell to the ground, landing on shoulder.

Employee was clearing debris from catwalk and reaching for a rock caught thumb between expanded metal decking and sheave.

Employee was five to six feet away from another employee who was using air hammer to remove bearing when a piece of metal flew off and hit employee between hard hat and safety glasses. Metal cut forehead and small piece was in the skin.

While completing maintenance on a VSI Crusher and changing teeth, the miner twisted to grab parts and sprained their lower back.

While doing maintenance inside a screen, was cleaning side, holding in place while lying down. After 10-minutes he readjusted his position and straightened his leg and that is when he heard a crack. Was find day of with no pain but the next day it was hurting and swollen.

Employee changing wear parts on a rock crusher and trying to situate a hammer into a vertical shaft impact crusher when he felt a pop in his back. The next day (9-6-13) employee was replacing grease on the track shovel when his back began hurting much worse.

EE was thawing out a catch pan for a water pump. When he reached down to pick up a steel plate out of the catch pan it was still frozen into the catch pan. As ee lifted he strained his lower back (Lumbar Strain and Sacrum Strain).

Taking nuts off of the bottom screen. Tripped on catwalk and motor, received a finger laceration on index finger.

Hard hat fell off, bent over to pick it up and hit head on Safety Guard when he sat back up.

Employee fell off ladder.

EE was changing screens on devise 6/18/07. Felt some pain but figured it would go away as it had in the past. Reported 6/25/07

Employee helping welder grind metal with handheld grinder. Employee wearing hard hat and safety glasses. Metal flecks in eyebrow. Employee rubbed eyelid with hand, some metal flecks fall into eye.

Employee operating Cat hoe with hydraulic breaker open at safety screen. A rock chip struck cheek resulting in a minor concussion.

Employee cutting metal. His foot was under metal being cut.

EMPLOYEE WAS HAMMERING. AFTER PASSAGE OF TIME HIS WRIST IS STARTING TO BOTHER HIM. RIGHT WRIST

EE STANDING ON GROUND SEVERAL 4-5 INCH ROCKS ON GROUND FROM SURGE PILE NEXT TO WHERE EE STANDING. AS EE PULLED TO REMOVE RETURN ROLLER, FOOTING SLIPPED, LOST BALANCE RETURN ROLLER IN HAND WAS PULLED FORWARD AS EE FELL, ROLLER PINCHED LEFT THUMB.

EE ENTERED AREA CLOSED.

EMPLOYEE KNEELING UNDER EQUIPMENT WHILE CHIPPING BUMPED HARD HAT ON CROSS PIECE.

EMPLOYEE BACKED DOWN LADDER. HIS HEAVY LUG BOOT SOLE CAUGHT WRUNG WRONG. SLIGHT OVER-EXTENTION OF LEFT KNEE. OK TO WORK - NO CRAWLING/CLIMBING.

STRUCK BY SMALL ROCK BOUNCED BY CRUSHER WHILE OPERATING SKIDSTEER.

EE WAS DESCENDING THE LADDER OF THE WATER WAGON WHEN HE PULLED MUSCLES IN HIS BACK. HE SAID IT HAPPENED PRIOR TO HIS STEPPING FOOT ON THE GROUND

EE WAS REACHING FOR HIS FOOD WHILE LEAVING THE LOADING AREA. HE FAILED TO TURN DOWN THE MAIN HAUL ROAD SINCE HE WAS STILL DISTRACTED BY HIS FOOD. HE CONTINUED ACROSS THE HAUL ROAD, OVER THE BE RM AND DOWN THE SIDE OF THE HILL. HE SUFFERED CONTUSIONS AND LACERATIONS. HE WAS HELD IN THE HOSPITAL FOR 3 DAYS BECAUSE OF DIZZINESS IN HIS HEAD AND FOR OBSERVATION.

EE WAS PULLING OUT THE LAST SCREEN CLOTH ON THE BOTTOM DECK OF THE SECOND VIBRATING SHAKER WHEN HE CAUGHT HIS FINGER ON THE END OF SCREEN. HIS COWORKERS PULLED THE SCREEN WHICH THEN FELL AND S LICED OPEN THE TIP OF HIS RING FINGER ON HIS LEFT HAND.

COLLECTING SAMPLES OF 3/4"" ROCK FROM CONVEYOR BELT.

EE WAS WALKING UNDER A GUARD ON A CONVEYOR WHEN HE HIT HIS HEAD ON THE GUARD. HE THEN FELL TO THE GROUND. ABOUT 3 WEEKS LATER HE WENT TO SEE HIS PHYSICIAN BECAUSE OF PAIN IN HIS HEAD & NECK.
